Při dlouhodobém hladovění je nedostatečný přívod energie kompenzován z vlastních zdrojů organizmu. Zásoby glykogenu jsou rychle vyčerpány, produkce inzulínu klesá. Zpočátku slouží jako zdroj energie především bílkoviny, později jsou přednostně utilizovány tuky. V každém případě dochází však do určité míry k vyčerpání libové tělesné hmoty s vývojem deplece iontů, především uložených intracelulárně, t.j. fosfátů, kalia a magnézia. Jejich plazmatické hladiny zůstávají během malnutrice, díky uvolňování z buněk, v referenčních mezích. Deplece postihuje i významné metabolické kofaktory, kterými jsou stopové prvky a vitaminy, především thiamin. Po zahájení realimentace a přívodu glukózy se cukry stávají opět primárním zdrojem energie a katabolismus tuků a bílkovin klesá. Uvolňuje se sekrece inzulínu a při nástupu anabolismu se tok uvedených iontů obrací zpět do buněk. Může dojít k extrémnímu poklesu jejich plasmatických hladin a k akutnímu nedostatku thiaminu. V těžších případech vzniká realimentační syndrom (RS). V souvislosti s tím může dojít k závažným dysfunkcím řady orgánů, včetně srdečního selhání. Práce uvádí postupy doporučené pro rozpoznání rizika malnutrice i rizika vývoje RS. Jsou popsány jeho možné pestré klinické příznaky. Jsou uvedeny postupy klinického i laboratorního monitorování při zahájení realimentace u nemocných, kteří jsou v riziku vývoje RS. Dále jsou uvedeny postupy při úpravě iontových deplecí při manifestaci RS, jejich agresivita je vztahována k plasmatickým hodnotám fosfátu, kalia a magnézia., During long lasting starvation the insufficient supply of energy is compensated from own sources of organism. The store of glycogen is soon exhausted and the production of insulin decreases. During initial period the main source of energy used is all body protein. Later on lipids are preferred. However to some extent the lean body mass is always depleted. This depletion is accompanied by decreased intracellular pool of phosphates, potassium and magnesium. Thanks to gradual release from catabolic cells during malnutrition, their plasma levels remain in reference ranges for long time. The depletion affects also the important metabolic factors, such as the trace elements and vitamins, thiamine especially. During realimentation sugars become the main source of energy again and catabolism of fat and protein is interrupted. The restored secretion of insulin and start of anabolism turn the shift of the afore mentioned ions back into the cells. Their plasma levels as well as the levels of thiamine may extremely fall in short time. As consequence of these changes the refeeding syndrome (RS) may develop. The severe multiorgan dysfunction may develop including the cardiac failure. The paper presents the proceedings recommended for the malnutrition risk screening and also for the detection of risk of RS. The possible diverse clinical manifestations of this syndrome are presented. The article follows the description of clinical and laboratory monitoring at the start of realimentation in severely malnourished high risk patients. 4986. Reálné možnosti využití datových zdrojů pro hodnocení kvality péče o pacienty s diabetem v České republice: Národní diabetologický informační systém

Diabetes mellitus je závažným zdravotnickým a ekonomickým problémem současnosti. Terapie je vnímána primárně jako prevence pozdních komplikací diabetu. Většina farmakoekonomických analýz uvádí, že náklady spojené s terapií diabetes mellitus jsou z větší části spotřebovány na terapii komplikací. Projekt Národního diabetologického informačního systému vychází ze základního principu automatizovaného sběru dat, která jsou k dispozici v České republice v elektronické podobě a která současně popisují kvalitu a kvantitu péče poskytovaného pacientů s diabetes mellitus i s jejími výsledky. Výhodou postulovaného Národního diabetologického informačního systému je minimalizace selektivní ztráty dat, komplexní pohled na epidemiologii, terapii a její výsledky, možnost sledování trendů, zahrnutí nákladů spojených s hospitalizací a sociálními dávkami. Rizikem zůstává správná interpretace a možnost systémové chyby. Jestliže by se podařilo v České republice podobný systém uskutečnit, bude to první takto komplexní plně automatizovaný informační systém na světě. Závěr: Data popisující incidenci a prevalenci diabetes mellitus, strukturu terapie, náklady přímé na zdravotní péči i indukované na sociálních platbách jsou v naprosté většině k dispozici v elektronické podobě. Vzniká tak možnost založení Národního diabetologického informačního systému, který umožní kontinuální vyhodnocování kvality péče o pacienty s diabetes mellitus včetně farmakoekonomických a sociálních důsledků. Therapy is primarily perceived as prevention of the late complications of diabetes. Most pharmacoeconomic analyses state that the cost associated with the therapy of diabetes mellitus is largely spent on the therapy of its complications. The project of the National Dia­betes Information System is based on the essential principle of automated collection of data which is available in di­gital form in the Czech Republic and which describes the quality and quantity of the care provided for patients with diabetes mellitus and its results. Benefits of the posited National Diabetes Information System embrace minimizing of selective loss of data, a comprehensive view of epidemiology, therapy and its results, possibility of following trends, inclusion of costs related to hospitalization and social benefits. Risks involve correct interpretation and a possible system error. If we succeeded in implementing such system in the Czech Republic, it would be the first fully automatic information system on such a comprehensive scale worldwide. Conclusion: The data describing the incidence and prevalence of diabetes mellitus, therapy structure, direct healthcare costs and induced costs of social payments is in the largest part available in digital form. This creates a basis for the foundation of the National Diabetes Information System which will allow for continuous quality assessment of care for patients with diabetes mellitus, including the related pharmacoeconomic and social impacts. 4987. Reasons for delaying the first birth for mothers aged 35 and more

Východiska: Východiskem výzkumného šetření byla Psychologická studie motivů pozdního rodičovství autorky Ireny Bímové. Cíl: Hlavním cílem výzkumného šetření bylo zjistit důvody oddálení narození prvního dítěte u matek po 35. roce. Metody: Výběrovým kritériem pro zařazení do výběrového souboru byl věk primipary 35 let a více. Jako technika sběru dat byl použit polostrukturovaný dotazník vlastní konstrukce. Počet respondentů byl 92. Data byla získávána na gynekologicko-porodnických a novorozeneckých odděleních v období od ledna 2013 do ledna 2014. K analýze získaných dat byl použit Fischerův přesný test. Výsledky: Nejčastějším důvodem oddálení narození prvního dítěte u matek po 35. roce byl výběr vhodného partnera. Tuto variantu uvádí 42,4 % matek. Druhým nejčastějším důvodem byly problémy s otěhotněním u 32,6 % matek. Součástí výzkumu bylo zpětné pozitivní a negativní hodnocení věku matek při narození prvního dítěte po 35. roce. Zpětně hodnotilo pozitivně věk porodu 45,7 % žen, negativně hodnotilo věk 54,3 % žen. Ženy, které zpětně hodnotily věk porodu 1. dítěte negativně, uvedly signifikantně častěji jako důvod oddálení porodu „Problémy s otěhotněním“ (p = 0,014). Ženy, které zpětně hodnotily věk porodu 1. dítěte pozitivně, uvedly signifikantně častěji jako důvod oddálení porodu „Preferenci nezávislého způsobu života“ (p = 0,039). Závěry: Důvody oddálení narození prvního dítěte v pozdějším věku spadají zejména do oblasti sociální, psychologické a zdravotní. Výzkum může vést k zamyšlení nad problematikou trendu pozdního rodičovství. Aim: e aim of the research was to determine the reasons for postponing the first birth for mothers aer 35 years of age. Methods: e group selection criterion was a primipara age 35 and more. A semi-structured questionnaire of an original design was used to collect the research data. e number of respondents was 92. Data were obtained on obstetric and neonatal departments from January 2013 to January 2014. To analyze the data was used Fisher's exact test. Results: e most frequent reason for birth postponement amongst mothers aged 35 and more was the search for a suitable partner. is option was selected by 42.4% mothers. e second most frequent answer involved issues with conception, this being reported by 32.6% mothers. Additionally, the survey research included a retrospective positive or negative evaluation of the mothers’ age when they had their first child aer the age of 35. In retrospect, the age for the first birth was seen positively by 45.7% women and negatively by 54.3% women. Women evaluating the age for the first birth negatively significantly more oen reported the reason „Problems with conception“ (p = 0.014). Women evaluating the age for the first birth positively significantly more oen reported „A preference of an independent way of life“ as the reason behind their birth postponement (p = 0.039). 4988. Recalcitrant pruritus as primary manifestation of synchronous Hodgkins lymphoma and Langerhans cell histiocytosis

Description:
We present a 73 year old female with intractable pruritus and nonspecific cutaneous rash for a period of 9 months. She had recieved symptomatic therapy with no improvement. A complete examination revealed axillary and abdominal lymphadenopathy. A biopsy confirmed the diagnosis of Hodgkins lymphoma with Langerhans cell histiocytosis. She received 5 cycles of chemotherapy with resolution of pruritus and reduction in axillary and abdominal lymphadenopathy. The patient presented 6 months later with relapse and succumbed to the illness. Simultaneous occurrence of Langerhans cell histiocytosis and Hodgkins lymphoma may lead to misdiagnosis.
