Náplní sdělení je souhrnná informace o Hevylite (HLC – heavy/light chain) analytické metodě rozšiřující dosavadní možnosti diagnostiky, typizace a monitorování průběhu maligních monoklonálních gamapatií (MG) a dalších B-buněčných lymfoproliferativních stavů včetně primární systémové AL-amyloidózy, primární Waldenströmovy makroglobulinémie a potenciálně i nehodgkinských lymfomů. Jsou popsány východiska této nové techniky a metodické aspekty automatizované kvantitativní imunochemické analýzy využívající vysoce specifické, avidní, polyklonální ovčí HLC protilátky s významným potlačením zkřížené reaktivity, jejichž terčem jsou unikátní junkční epitopy umístěné mezi konstantními doménami těžkých (HC) a lehkých (LC) řetězců molekul imunoglobulinu (Ig). Použité HLC protilátky umožňují separátní rozpoznání a kvantifikaci lehkých řetězců k a l v rámci jednotlivých tříd imunoglobulinů, tedy IgGk a IgGl, IgAk a IgAl i IgMk a IgMl, včetně stanovení poměru dominantního a suprimovaného Ig, tj. poměr Ig k/l a tím i posouzení klonality hodnoceného procesu.Tato nová metoda poskytuje reproduktibilní výsledky i v případě nízké hodnoty monoklonálního imunoglobulinu (MIg) a volných lehkých řetězců v séru ( sVLŘ), nebo i v situaci „překrytí“ M-proteinu např. transferinem, haptoglobinem nebo C3 složkou komplementu v rámci standardní elektroforézy bílkovin séra. K přednostem Hevylite metody patří skutečnost, že stanovení HLC Ig k/l poměru koriguje variabilitu katabolismu IgG i ostatních tříd MIg a změny hematokritu i plazmatického objemu. Jde o metodiku citlivější, nežli je standardní gelová elektroforéza bílkovin séra s obdobnou, případě i vyšší senzitivitou, nežli je imunofixační elektroforéza séra s výhodou poskytnutí i kvantitativně vyjádřených výsledků měření. Vyšetření HLC rozšiřuje dosavadní možnosti nejen v oblasti diagnostiky, ale i ve sféře monitorování průběhu maligních MG s možností včasné detekce remise, relapsu či progrese nemoci a nepostrádá ani prognostický potenciál. Skutečné postavení Hevylite metody ovšem vyplyne teprve z výsledků dalších, prospektivních randomizovaných studií a ze zkušeností reálné klinické praxe., The aim of the presented study is to display a general information about the Hevylite (HLC – heavy/light chain) analytical method, spreading present possibilities of diagnostics, typization and course monitoring of malignant monoclonal gammopathies (MG) and other B-cell lymphoproliferative diseases including primary systemic AL-amyloidosis, primary Waldenström macroglobulinemia and potentially also non-hodgkin lymphomas. The study describes the basis of this new method, methodical aspects of automatic quantitative immunochemical analysis using high specific, avid, polyclonal ovine HLC anti-bodies with substantial suppression of crossreactivity, and their targets are unique junction epitopes located between constant domains of heavy (HC) and light (LC) chains of immunoglobulin molecules (Ig). Used HLC anti-bodies allow separate recognition and quantification of light chains k and l within the separate immunoglobulin classes, then IgGk and IgGl, IgAk and IgAl also IgMk and IgMl, including assessment of relationship of dominant and suppressed Ig, i.e. Ig k/l ratio, thereby the clonality of assessed process examination. This new method provides reproducible results also in case of low levels of monoclonal immunoglobulin (MIg) and free light chains in serum (sFLC), or in condition of M-protein „overlap“ e.g. by transferrin, haptoglobin or C3 complement compound. The advantage of Hevylite method is that assessment of HLC Ig k/l ratio amends the catabolism variability of IgG and other MIg classes, and also the changes of haematocrit and plasmatic volume. The method is more sensitive than standard gel electrophoresis of serum protein with analogous, perhaps even higher sensitivity than serum immunofixation electrophoresis, with advantage of quantitative analytical representation of results. HLC assessment extends current possibilities not only in the field of diagnostics, but also in monitoring of malign MG with chance of the early detection of remission, relapse or progression of the disease and also has a prognostic potential. Náplní sdělení je, na pozadí rozboru diagnostiky a léčby vlastních tří nemocných, podání současných poznatků o poměrně vzácném a v běžné klinické praxi opomíjeném onemocnění, vyznačujícím se depozicí lehkého řetězce imunoglobulinu v tkáních životně důležitých orgánů. Onemocnění z depozice lehkých řetězců imunoglobulinu postihuje především ledviny a vede k progresivní poruše funkce s konečným orgánovým selháním. Kromě etiopatogeneze, odlišení idiopatické formy od formy provázející mnohočetný myelom a vzácně i jiné B-lymfoproliferativní stavy, je nastíněna problematika klinických projevů, diagnostiky, diferenciální diagnostiky a terapie včetně přínosu biologické léčby bortezomibem a vysokodávkované chemoterapie s podporou autologních kmenových buněk, vedoucí k podstatnému zlepšení kvality i celkové délky života nemocných s touto prognosticky stále závažnou nemocí., The aim of the paper within the background of the diagnostics and treatment of three patients is the summary of recent knowledge about a relatively rare disease which is characterized by the deposition of immunoglobulin light chains in vital tissues and organs, especially the kidneys, leading to progressive deterioration of their function with end stage organ failure. Sdělení podává současný pohled na úlohu jednotlivých zobrazovacích metod v diagnostice mnohočetného myelomu (MM) a jeho odlišení od monoklonální gamapatie nejistého významu (MGNV). Jsou rozvedeny možnosti i úskalí rozpoznání jednotlivých projevů myelomové kostní nemoci při použití konvenční radiografie skeletu, výpočetní tomografie (CT), magnetické rezonance (včetně celotělové formy MR), 99mTc-MIBI scintigrafie a FDG-PET/CT, sdružující vyšetření s pomocí pozitronové emisní tomografie a multidetektorové výpočetní tomografie. Je popsán přínos nových zobrazovacích metod pro stratifikaci MM (stážovací systém Durie-Salmon Plus), monitorování vývoje nemoci včetně hodnocení léčebné odezvy, rozpoznání relapsu/progrese a extramedulárního šíření nemoci. Jsou rovněž zhodnocena interpretační úskalí jednotlivých zobrazovacích technik a principy jejich individuální volby podle specifické klinické situace., The paper presents recent opinion on individual imaging methods in the diagnostics of multiple myeloma (MM), and its differentiation from monoclonal gammopathy of undetermined significance (MGUS). We assess the possibilities and difficulties in the differentiation of individual manifestations of myeloma bone disease using conventional radiography, computed tomography (CT), magnetic resonance imaging (including whole body MR), 99mTc-MIBI scintigraphy, and FDG-PET/CT which combines positron emission tomography and multidetector computed tomography. We describe the contribution of new imaging methods for MM stratification (Durie-Salmon Plus staging system), monitoring of the course of MM including therapeutics response, recognition of relapse/progression, and extramedular progression of the disease.
