Podán historický přehled mortalitních studií s inhibicí systému angiotenzin-aldosteron u nemocných s chronickým srdečním selháním. Od studie CONSENSUS po studii PARADIGM-HF se ukazuje, že zlatým standardem léčby jsou inhibitory ACE/blokátory AT1 receptorů pro angiotenzin II – sartany, spolu s blokátory mineralokortikoidních receptorů. Přímý blokátor reninu aliskiren a duální blokátor enalapril s inhibicí neprilysinu se ukázaly jako neúčinné, na druhé straně nový duální inhibitor valsartan + inhibitor neprilysinu LCZ 696 je novým nadějným léčebným přípravkem pro budoucnost léčby chronického srdečního selhání., An historical survey is presented of mortality trials on angiotensin-aldosteron system inhibition in patients with chronic heart failure. From the CONSENSUS trial up to the PARADIGM-HF trial, ACE inhibitors/angiotensin II receptor antagonists (AT1-blockers, ARBs, sartans), along with mineralocorticoid receptor blockers, have been the gold standard of treatment. Podáváme přehled současných názorů na kombinační léčbu a na postavení fixních kombinací v léčbě hypertenze podle doporučení ESH/ESC a ČSH z roku 2013. Mezi nejčastěji doporučované dvojkombinace patří blokátor renin-angiotenzinového systému (ACEI nebo sartan) a blokátor vápníku, dále blokátor renin-angiotenzinového systému a diuretikum a blokátor vápníku a diuretikum. V roce 2014 se na českém trhu objevila fixní kombinace ACE inhibitor (perindopril), blokátor vápníku (amlodipin) a diuretikum (indapamid). Ve studii the PIANIST u 4 731 nedostatečně kontrolovaných hypertoniků fixní trojkombinace perindopril, amlodipin a indapamid vedla ke snížení krevního tlaku o 28,3/13,8 mm Hg a k dostatečné kontrole hypertenze u 92 % nemocných. Výhoda fixních kombinací je především ve větší komplianci nemocného, a tím v lepší kontrole hypertenze. Trojkombinaci k dobré kontrole tlaku potřebuje asi třetina hypertoniků., We present an overview of the present views on combination treatment and on fixed combinations in the treatment of hypertension according to guidelines of ESH/ESC and ČSH from 2013. The most frequently recommended dual combinations include a blocker of the renin-angiotensin system (ACEI or sartan) and a calcium channel blocker, and further a blocker of the renin-angiotensin system and a diuretic and a calcium channel blocker and a diuretic. In 2014 a fixed-dose combination of an ACE inhibitor (perindopril), a calcium channel blocker (amlodipine) and an diuretic (indapamide) appeared on the Czech market. Within the PIANIST study including 4 731 insufficiently controlled hypertensives, a fixed-dose triple combination of perindopril, amlodipine and indapamide led to a decrease in blood pressure by 28.3/13.8 mm Hg and to a sufficient control of hypertension in 92 % of patients. The advantage of fixed combinations primarily consists in greater compliance of patients and thereby in a better control of hypertension. Léčba diabetes mellitus 2. typu je v současnosti účinná, ale bohužel ještě není ideální. Novou skupinu perorálních antidiabetik představují DPP4 inhibitory, tzv. gliptiny. V roce 2015 byla ukončena již 3. velká mortalitní klinická studie s gliptiny u pacientů s diabetes mellitus 2. typu a kardiovaskulárním onemocněním. Jedná se o studie TECOS, SAVOR a EXAMINE s léky sitagliptin, saxagliptin a alogliptin. Studie zahrnuly téměř 37 000 nemocných. Tyto studie prokázaly kardiovaskulární bezpečnost DPP4 inhibitorů, přesto ale zůstává otevřená otázka výskytu srdečního selhání. Prokázaná je i účinnost na snižování glykemie, resp. glykovaného hemoglobinu a bezpečnost z pohledu onemocnění pankreatu. Klíčová slova: bezpečnost – gliptiny – účinnost, The treatment of diabetes mellitus type 2 is effective, but still is not optimal. DPP4 inhibitors (gliptins) are a new group of peroral antidiabetic drugs. The third clinical mortality study with gliptins in patients with diabetes mellitus type 2 was finished in 2015. The studies are known under acronym TECOS, SAVOR and EXAMINE and the tested drugs are sitagliptin, saxagliptin and alogliptin. The studies included about 37 000 patients. The studies confirmed the cardiovascular safety of the DPP4 inhibitors, but the question about increased heart failure remains open. The effectiveness of lowering glycaemia (glycated haemoglobin) was confirmed and also the pancreatic safety is confirmed.
