Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. In this period the number of primary cases increased twice, of subsequent ones nearly 30 times during five periods. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract. Z 30 166 případů leukemií, evidovaných v českém registru nádorů v letech 1976–2005 bylo 3 937 vícečetných případů spojeno s výskytem jiných novotvarů. Z nich se 2 450 (62,2 %) vyskytlo u mužů a 1 487 (37,8 %) u žen, tj. 14,7% případů u mužů a 11 % u žen ze všech evidovaných leukemií. Po 1 715 primárních leukemiích bylo diagnostikováno 2 064 jiných novotvarů a před 2 222 následnými leukemiemi 2 642 jiných primárních nádorů. Ze 14 krajů se ve třech vyskytlo 35,5 % vícečetných leukemií (Jihomoravský a Severomoravský kraj, Praha). V průměru za 4,9 roku u mužů a 5,5 roku u žen následovalo 35,1 % nádorů kůže a melanom, 16,9 % nádorů trávicích, 13 % dýchacích a 8,4 % močových cest. Očekávaný nízký počet pokročilých klinických stadií u následných nádorů se nepotvrdil: u mužů zahrnoval 43,8 % nádorů dutiny ústní, 41,5 % dýchacích, 34,2% trávicích, 10,6 % močových cest a 15,7 % pohlavních orgánů; u žen 39,4 % dýchacích, 30,6 % močových, 30 % trávicích cest a 28,2 % pohlavních orgánů, 25 % prsů, 25 % dutiny ústní. Bohužel mnoho klinických stadií nebylo v registru nahlášeno. Před následnými leukemiemi bylo nejvíce primárních karcinomů kůže a melanomu u 43,6 % případů jako ukazatel maligního stavu. Do října 2007 přežívalo u mužů jen 12,4 % primárních a 10,3 % následných leukemií, respektive 19,7 % a 14 % u žen. Kolik dalších novotvarů se vyskytne u osmi tisíc přežívajících s leukemií, očekávaných v roce 2015? Unfortunatelly a lot of clinical stages were not registered. Before the subsequent leukemias the most frequent were 43.6 % primary cancers of skin and melanoma as a sign of malignant status. Up to October 2007 in males survived only 12.4 % of primary and 10.3 % subsequent leukemias, respective 19.7 % and 14 % in females. How many other neoplasms will be diagnosed among 8,000 surviving with leukemia, estimated in the Czech Republic in 2015?
