Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Limited financial resources and persistent risks of lifestyle do not allow to slow down the increase of primary and subsequent neoplasms in the near future among cancer survivors not only in gynaecology., Edvard Geryk, Radim Štampach, Viera Bajčiová, Teodor Horváth, and Literatura
V průměru za 4,9 roku u mužů a 5,5 roku u žen následovalo 35,1 % nádorů kůže a melanom, 16,9 % nádorů trávicích, 13 % dýchacích a 8,4 % močových cest. Očekávaný nízký počet pokročilých klinických stadií u následných nádorů se nepotvrdil: u mužů zahrnoval 43,8 % nádorů dutiny ústní, 41,5 % dýchacích, 34,2% trávicích, 10,6 % močových cest a 15,7 % pohlavních orgánů; u žen 39,4 % dýchacích, 30,6 % močových, 30 % trávicích cest a 28,2 % pohlavních orgánů, 25 % prsů, 25 % dutiny ústní. Bohužel mnoho klinických stadií nebylo v registru nahlášeno. Před následnými leukemiemi bylo nejvíce primárních karcinomů kůže a melanomu u 43,6 % případů jako ukazatel maligního stavu. Do října 2007 přežívalo u mužů jen 12,4 % primárních a 10,3 % následných leukemií, respektive 19,7 % a 14 % u žen. Kolik dalších novotvarů se vyskytne u osmi tisíc přežívajících s leukemií, očekávaných v roce 2015? Rozbory vícečetných novotvarů by mohly osvětlit mechanizmy jejich vzniku a poskytnout náhled na dispenzární péči a preventivní opatření, ze kterých budou mít prospěch rostoucí počty onkologicky nemocných v remisi., A total of 30,166 cases of leukemia, based on the Czech Cancer Registry in 1976–2005, there were notificated 3,937 multiple cases associated with the ocurrence of other neoplasms. Of them there were 2,450 (62.2 %) in males and 1,487 (37.8 %) in females, i.e. 14.7 % cases in males and 11 % in females of total registered leukemia. There were diagnosed 2,064 other neoplasms after 1,715 primary leukemias and 2,642 other primary neoplasms before 2,222 subsequent leukemias. A total of 14 Czech regions were distributed 35.5 % multiple leukemias in the three regions (Southern and Northern Moravia, Prague). In the average interval 4.9 years in males and 5.5 years in females followed 35.1 % cancer of skin and melanoma, 16.9 % digestive, 13 % respiratory and 8.4 % urinary system. The expected low representation of advanced clinical stages in subsequent cancers was not confirmed: in males 43.8 % cancer of oral cavity, 41.5 % respiratory, 34.2 % digestive, 10.6 % urinary and 15.7 % genital organs; in females 39.4 % respiratory, 30.6 % urinary, 30 % digestive and 28.2 % genital organs, 25 % breast, 25 % oral cavity. Unfortunatelly a lot of clinical stages were not registered. Before the subsequent leukemias the most frequent were 43.6 % primary cancers of skin and melanoma as a sign of malignant status. Up to October 2007 in males survived only 12.4 % of primary and 10.3 % subsequent leukemias, respective 19.7 % and 14 % in females. How many other neoplasms will be diagnosed among 8,000 surviving with leukemia, estimated in the Czech Republic in 2015? The analyses of multiple neoplasms should be possible to illuminate the mechanisms of susceptibility to multiple cancers and provide insights into the long-term medical care and preventive interventions that will benefit the growing population of cancer survivors. of subsequent cancers., Edvard Geryk, Radim Štampach, Jiří Kozel, Petr Dítě, Milan Konečný, and Literatura
