Cíl: Cílem sdělení je připomenout možná úskalí paralelního hodnocení výsledků elektroforézy séra a sérových hladin volných lehkých řetězců (VLŘ) imunoglobulinu. Stanovení sérových hladin VLŘ se v posledních letech stalo nedílnou součástí algoritmu vyšetření využívaných v diagnostice a monitorování nemocných s monoklonální gamapatií. Zkušenosti mnoha pracovišť a následně publikovaná doporučení napomáhají vhodné indikaci vyšetření a interpretaci nálezů, avšak v každodenní praxi se často setkáváme s výsledky diskrepantními či atypickými, což si vyžaduje obezřetnou interpretaci, ověření stanovení a úzkou spolupráci zúčastněných odborníků., Aim: The aim of the presented paper is to remind of possible pitfalls of parallel assessment of serum electrophoresis and serum free light chain levels (FLC) of the immunoglobulins. Evaluation of serum levels of FLC has become an inherent part of the algorithm in the diagnostics and monitoring of patients with monoclonal gammopathy. Experience from several departments followed by published recommendations help to determine appropriate indication and interpretation of the results.
