Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Úvod: Sympatická kožní odpověď (SSR) je jednoduchý a široce dostupný test sudomotorických funkcí. Jedná se o polysynaptický reflex s variabilní aferentací a eferentní částí, zprostředkovanou tenkými nemyelinizovanými vlákny typu C. Cílem práce bylo zhodnocení validity SSR v diagnostice neuropatie s postižením tenkých senzitivních nervových vláken. Metodika: SSR byla vyšetřena na horních a dolních končetinách u 69 pacientů s bolestivou periferní neuropatií. U 33 pacientů této skupiny šlo o izolovanou neuropatii tenkých nervových vláken (NTV). Třicet šest pacientů mělo kombinované postižení vláken tenkých a silných. Nálezy byly srovnány se souborem 89 zdravých kontrol. Hodnocena byla výbavnost odpovědí elektrickým proudem a prudkým inspiriem a jejich reprodukovatelnost, amplituda a latence. Výsledky: Hodnocení latencí ani amplitud odpovědí neprokázalo jasný přínos pro diagnostiku postižení tenkých nervových vláken. Nejspolehlivější typ SSR abnormity je tedy nevýbavnost odpovědí. Postižení tenkých autonomních vláken však bylo v našem souboru pomocí SSR prokazatelné pouze u malé části pacientů se senzitivní neuropatií – senzitivita dosáhla necelých 10 % u pacientů s čistou NTV a 33 % u pacientů s kombinovaným postižením vláken tenkých a silných (u nichž je současně pravděpodobnější těžší stupeň postižení vláken tenkých). Závěr: Při zohlednění nízké senzitivity není SSR optimální metoda využitelná jako jediný diagnostický test neuropatie tenkých vláken., Introduction: Sympathetic skin response (SSR) is a simple and widely available test of sudomotor functions. Afferent part of this polysynaptic reflex is variable, while the efferent direction of the reflex arch is via thin unmyelinated C fibres. No reliable study of the SSR diagnostic validity in patients with sensory small fiber neuropathy (SFN) has so far been published. In clinical practice, however, we repeatedly meet patients, who have been diagnosed with SFN based solely on abnormal SSR. The aim of the study was to evaluate diagnostic validity of SSR in patients with sensory small fibre neuropathy. Methods: SSR was recorded from palms and soles of 69 patients with painful sensory neuropathy (33 of them with pure SFN and 36 with mixed small and large nerve fibre dysfunction) using electrical stimulation and inspiratory gasp stimuli. Small nerve fiber involvement was confirmed by reduced intraepidermal nerve fiber densities in skin biopsy samples of all cases. The results were compared with those of 89 healthy controls. The outcome for health controls were also used to establish age-stratified normative data. We assessed electric stimuli and sudden inspiration response recall end their reproducibility, amplitude and latency. Results: Both the SSR latencies and amplitudes showed very low diagnostic validity in small fibre neuropathy patients. The absence of SSR response represented the most reliable abnormality. However, using this parameter, dysfunction of small autonomic nerve fibres has only been found in a small part of our sensory neuropathy patients: sensitivity did not exceed 10% in pure SFN patients and 33% in those with mixed small and large nerve fibre dysfunction (where more pronounced small sensory nerve fibre involvement has previously been found). Conclusion: Considering its low sensitivity, SSR should not be used as the only test to confirm sensory small fibre neuropathy. Úvod: Důležitou součástí diagnostického algoritmu neuropatické bolesti jsou jednoduché dotazníkové nástroje založené na přítomnosti typických popisných charakteristik (tzv. deskriptorů). V českém jazykovém prostředí však není k dispozici validovaná verze žádného z mezinárodně akceptovaných dotazníků. Cíl: Cílem práce bylo vytvoření české jazykové verze jednoho z nejpoužívanějších dotazníků zaměřených na diagnostiku neuropatické bolesti a charakteristiku jednotlivých jejích subtypů, a to tzv. NPSI (Neuropathic Pain Symptom Inventory) a její validace. Soubory a metodika: Dotazník byl přeložen metodou „forward‑backward translation“. Pro potřeby validace byla touto českou jazykovou verzí následně vyšetřena skupina pacientů s neuropatickou bolestí periferního (n = 66) nebo centrálního (n = 30) původu (v rámci polyneuropatie či roztroušené sklerózy) a skupina nemocných s bolestí nociceptivní v důsledku pokročilé artrózy kyčelního nebo kolenního kloubu (n = 70). Za účelem hodnocení opakovatelnosti testu byl u části pacientů (n = 19) dotazník administrován dvakrát s odstupem přibližně tří hod. Výsledky: Srovnání jednotlivých hodnocených položek, dílčích škál i celkového NPSI skóre pomocí ROC analýzy prokázalo vysokou schopnost diskriminace mezi neuropatickou a nociceptivní bolestí (p < 0,001), ale nízkou schopnost odlišení neuropatické bolesti centrálního a periferního původu (p > 0,05). Opakovaná administrace testu potvrdila vysokou opakovatelnost testu (ICC, Intraclass Correlation Coefficient = 0,86 pro sumární skóre). Pomocí clusterové analýzy bylo identifikováno šest odlišných symptomových profilů. Závěr: Vytvořená česká verze dotazníku NPSI je schopna spolehlivě diskriminovat neuropatickou a nociceptivní bolest a identifikovat odlišné symptomové profily bolesti při akceptovatelné opakovatelnosti testu. Dotazník lze tedy doporučit jako vhodný a snadno použitelný nástroj v diagnostickém algoritmu neuropatické bolesti, využitelný i pro longitudinální sledování., Introduction: Simple questionnaires based on so called “neuropathic pain descriptors” are the most frequently used screening tools in the diagnosis of neuropathic pain, especially by non‑specialists. However, no validated Czech version of any of the internationally accepted nueropathic pain questionnaires is available. Aim: The aim of this study was to develop and validate a Czech version of the Neuropathic Pain Symptom Inventory (NPSI), a questionnaire that allows the user to characterize subtypes of neuropathic pain, is sensitive to change, and has discriminatory power. Patients and methods: The NPSI was translated into the NPSIcz using the forward‑backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 66) or central origin (n = 30) due to polyneuropathy or multiple sclerosis, and in a group of individuals suffering from nociceptive pain (due to severe osteoarthritis of the knee or hip joint; n = 70). The NPSIcz was repeated within three hours in 19 individuals to determine test‑retest reliability. Results: The NPSIcz showed good diagnostic validity in discriminating between patients with neuropathic and nociceptive pain (p < 0.001) but low ability to discriminate between patients with central and peripheral neuropathic pain (p > 0.05). Repeated administration confirmed acceptable test‑retest reliability (intra‑class correlation coefficient of 0.86 for the NPSIcz sum score). Cluster analysis identified six different pain profiles. Conclusions: The NPSIcz is able to discriminate between neuropathic and nociceptive pain, to identify different pain profiles, and has acceptable test‑retest reliability. It can, therefore, be recommended as an easy‑to‑administer questionnaire suitable for the diagnosis of neuropathic pain, including long‑term follow up.
