Paliativní péče klade důraz na kvalitu života nemocného a saturaci jeho bio-psycho-sociálních a spirituálních potřeb. V zahraničí se používají nástroje pro hodnocení potřeb pacientů v onkologické či paliativní péči, které pomáhají zdravotnickým pracovníkům porozumět specifickým potřebám pacientů, které by mohly být předmětem profesionální individualizované péče. Cílem tohoto přehledového článku je analyzovat měřící nástroje pro hodnocení potřeb pacientů v paliativní péči, porovnat je s doporučenými postupy pro tvorbu měřících nástrojů v paliativní péči a s českou verzí dotazníku PNAP. V elektronických databázích (Medline, Scopus, Proquest Public Health a Web of Knowledge) bylo nalezeno a dále podrobněji analyzováno 12 měřících nástrojů. Český dotazník PNAP pokrývá velkou část nejčastěji měřených potřeb i doporučení plynoucích ze směrnic pro měření potřeb v paliativní péči, které byly vytvořeny na základě vědeckých důkazů., Palliative care emphasizes quality of life of the ill and the saturation of his bio-psycho-social and spiritual needs. Abroad are used tools for assessing needs of patients in oncological or palliative care, which helps to health workers to understand the specific needs of patients that could be subject to professional individualized care. The aim of this review is to analyse measuring tools for assessing needs of patients in palliative care, to compare them with recommended procedures for creation of measurement tools in palliative care and with Czech version of the questionnaire PNAP. In the electronic databases (Medline, Scopus, Proquest Public Health and Web of Knowledge) was recovered and further analysed in detail 12 measuring tools. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. Cíl: Cílem je zjistit, který hodnotící nástroj je v odborné literatuře doporučován jako nejvhodnější pro posouzení komplikací v dutině ústní dětí, ve věku 2-18 let léčených chemoterapií a/nebo radioterapií. Metodika: Analýza příspěvků vyhledávaných v licencovaných a volně přístupných databázích (Medline, Scopus, Proquest Public Health, Ebsco, Cochrane Collaboration) za období 1985-2012. Výsledky: K posouzení komplikací v dutině ústní dětí léčených chamoterapií bylo identifikováno šest vhodných hodnotících nástrojů - Oral Mucositis Daily Questionnaire, Oral Mucositis Assessment Scale, Western Consortium Cancer Nursing Research Staging System for Stomatitis, Oral Assessment Guide, Children's International Mucositis Evaluation Scale a Oral Mucositis Rating Scale. Většina z nich, je modifikací hodnotících nástrojů určených dospělé populaci. K nejnovějším nástrojům náleží Children's International Mucositis Evaluation Scale (ChIMES), která je od počátku vyvíjena pro dětské pacienty a je v současnosti ověřována v klinické praxi. Škála zahrnuje subjektivní hodnocení bolesti v ústech, potíže s polykáním, omezení při jídle, omezení při pití a objektivní hodnocení léze. Zahrnuto je dále užívání léků proti bolesti v ústech nebo krku, v hodnoceném dni. Nejčastěji užívaná byla ve studiích hodnocena Oral Assessment Guide (OAG), která byla všemi autory doporučována k užití. Závěr: OAG je vhodný hodnotící nástroj pro dětskou populaci. Jejím pozitivem jsou kromě možnosti stanovení stadia komplikací v dutině ústní také doporučené intervence pro jednotlivá stadia poškození dutiny ústní. Posouzení je vhodné k použití kvalifikovanou sestrou., Aim: The aim is to find out which assessment tool is recommended in the scientific literature as the most suitable for an assessment of complications in the oral cavity in children, from the age of 2 to 18, who undergo chemotherapy and/or radiotherapy. Methods: Analysis of papers searched in licenced and freely available databases (Medline, Scopus, Proquest Public Health, Ebsco, Cochrane Collaboration) in the period of 1985-2012. Results: For an assessment of complications in the oral cavity in children undergoing chemotherapy, six suitable assessment tools were identified - Oral Mucositis Daily Questionnaire, Oral Mucositis Assessment Scale, Western Consortium Cancer Nursing Research Staging System for Stomatitis, Oral Assessment Guide, Children's International Mucositis Evaluation Scale a Oral Mucositis Rating Scale. Most of them are a modification of assessment tools for an adult population. One of the newest assessment tool is Children's International Mucositis Evaluation Scale (ChIMES), which has been developed for paediatric patients from the very beginning. At present the scale is being verified in clinical practice. The scale includes a subjective assessment of pain in the mouth, problems with swallowing, limitations at eating and drinking, and an objective lesion assessment. It also includes taking analgesics to releive the pain in the mouth and the throat on the day when an assessment is made. Oral Assessment Guide (OAG) is the most frequently used assessment tool in the studies and is recommended by all authors for the use. Conclusion: OAG is a suitable assessment tool for a children´s population. Its positive features are a possibility to determine a stage o f complications in the oral cavity as well as recommended interventions for individual stages of the damage to the oral cavity.
