V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. The number of primary PC increased to 282 cases in 1996 and then decreased, of subsequent PC permanently increased to 705 in 2005. Nearly half of PC contributed to the age group 70–79 year, third the age 50–69 year. The proportion of 1,115 (21.3 %) synchronous and 4,121 (78.7 %) metachronous subsequent neoplasms was 1:3.7. The average time between the primary and subsequent neoplasms was 4.1 years. The most frequent from 5,236 subsequent neoplasms after primary PC there were 28,6 % cancers of skin, 26,7 % gastrointestinal cancers, 17,8 % of urinary and 10,9 % in respiratory tract. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. From patients with primary PC survived 22,4 % and died 77,6 %, from subsequent PC survived 31,6 % and died 68,4 % men up to 17. 10. 2007. Výsledky na bázi dat Národního onkologického registru ČR z období 1976–2005 uvádí: a) Z 125 262 primárních novotvarů (52,1 % mužů, 47,9 % žen) bylo evidováno v časných klinických stadiích 37,1 % nádorů u mužů a 46,6 % u žen, v pokročilých stadiích 9,3 % u mužů a 9,5 % u žen. Neznámá stadia převažovala nad ostatními stadii až do roku 1994. Za celé období převažovali muži ve stadiu IV nad ženami. Nejvíce časných stadií se vyskytlo v Praze, pokročilých stadií v kraji Plzeňském. Po vyloučení neznámých stadií bylo z 54 187 duplicit u mužů 63,8 % v časných stadiích, u žen 71,3 %, v pokročilých stadiích u mužů 18,4 % a 15,8 % u žen. Z ostatních 10 292 multiplicit bylo 16,2 % časných stadií u mužů a 11,7 % u žen, v pokročilých stadiích 1,6 % u mužů a 1,2 % u žen. b) Z 165 050 následných novotvarů (54,4 % mužů, 45,6 % žen) – u případů po primárních stadiích I a II. bylo evidováno 21,7 % časných klinických stadií u mužů a 27 % u žen, 5 % pokročilých stadií u mužů a 6 % u žen – u případů po primárních stadiích III a IV bylo evidováno 2,5 % časných stadií u mužů a 3,2 % u žen, 1,8 % pokročilých stadií u mužů a 2,2 % u žen. Pozoruhodných bylo u mužů 12 418 následných nádorů v pokročilých stadiích (13,8 % ze všech 89 796 následných), u žen 10 189 (13,5 % ze všech 75 254 následných) po primárních nádorech stadia III a IV. Mezi následnými pokročilými stadii převažovalo – u mužů 36,9 % nádorů trávicích a 33,1 % dýchacích cest, 10,3 % pohlavních orgánů, 8,1 % močových cest – u žen 37 % nádorů trávicích cest, 21 % prsů, 17,2 % rodidel, 12,8 % dýchacích a 4,5 % močových cest. Je varující, že z vícečetných onemocnění byl každý sedmý nádor gastrointestina u mužů a pátý u žen po primárním novotvaru zjištěn v některém pokročilém klinickém stadiu. c) Ze 41 161 přežívajících k 17. 10. 2007 bylo 54,7 % časných stadií u mužů a 58 % u žen, 5 % pokročilých stadiích u mužů a 5,2 % u žen, neznámých stadií 40,3 % u mužů a 36,9 % u žen. Z 84 101 zemřelých bylo 30 % časných stadií u mužů a 39,8 % u žen, 11 % pokročilých stadií u mužů a 12,1 % u žen, neznámých stadií 59 % u mužů a 48,1 % u žen. Z výsledků lze doporučit, aby přehledy vícečetných diagnóz a jejich stadia se staly součástí ročních statistických přehledů novotvarů u české populace., The results based in the Czech Cancer Registry in 1976–2005: a) Of the 125,262 primary neoplasms (52.1 % males, 47.9 % females) there were registered 37.1 % of early clinical stages in males and 46.6 % in females, 9.3 % of advanced stages in males and 9.5 % in females. After exclusion of unknown stages, there were of 54,187 duplicities 63.8 % cases of early stages in males and 71.3 % in females; 18.4 % of advanced stages in males and 15.8 % in women. Of other 10,292 multiplicities there were 16.2 % of early stages in males and 11.7 % in females, 1.6 % of advanced stages in males and 1.2 % in females. b) Of the 165,050 subsequent neoplasms (54.4 % males, 45.6 % females) – in cases following the primary stages I and II there were registered 21.7 % of early stages in males and 27 % in females, 5 % of advanced stages in males and 6 % in females, – in cases following the primary stages III and IV there were registered 2.5 % of early stages in males and 3.2 % in females, 1.8 % of the advanced stages in males and 2.2 % in females. There were uncommon the number of 12,418 subsequent tumors of advanced stages in men (13.8 % of total 89,796 subsequent ones) and 10,189 in women (13.5 % of total 75,254 subsequent ones), following the primary cancers of stages III, IV. Between these subsequent and advanced stages predominated – in males 36.9 % cancers of gastrointestinal and 33.1 % respiratory tract, 10.3 % genital organs, 8.1 % urinary tract, – in females 37 % cancers of gastrointestinum, 21 % breast, 17.2 % genital organs, 12.8 % respiratory and 4.5 % urinary tract. It is worrying that of total multiple cases every the seventh gastrointestinal cancer in males and the fifth in females were detected at any advanced clinical stages after primary cancer. c) Of 41,161 survived up to October 2007 there were 54.7 % of early stages in males and 58 % in females, 5 % of advanced stages in males and 5.2 % in females, unknown stages 40.3 % in males and 36.9 % in females. Of 84,101 deaths there were 30 % of early stages in males and 39.8 % in females, 11 % of advanced stages in males and 12.1 % in females, unknown stages 59 % in males and 48.1 % in females. Za posledních 46 let bylo u české populace evidováno 56 828 onemocnění nádorem pankreatu (PC) dg. C25, s incidencí 19,1 na 100 tis. mužů a 18,2 na 100 tis. žen v roce 2007. Prevalence PC vzrostla mezi roky 1989–2005 u mužů o 45,9 % na 629, u žen o 119,1 % na 241. Z 3 632 vícečetných nádorů PC, evidovaných za období 1976–2005 v registru, bylo u mužů 321 (1,5 %) primárních a 1 581 (7,4 %) následných, u žen 241 (1,2 %) primárních a 1 489 (7,7 %) následných, které představovaly 11 % ze všech nově hlášených PC. Duplicitní PC byl početnější u žen, multiplicitní u mužů. Počet primárních PC se zvýšil na 34 případů a následných PC na 258 v r. 2005. Téměř polovina vícečetných PC u mužů a třetina u žen byla evidována ve věku 35–64 let. Z 3 632 vícečetných PC se více vyskytlo v kraji Severomoravském 11,8 %, Jihomoravském 11,1 % a v Praze 8,1 %. Průměrný interval mezi primárním PC a jinými následnými novotvary (dg. C00–D48) byl 3,2 roku u mužů a 2,1 roku u žen, poměr synchronních a metachronních případů byl 1:0,7 u mužů a 1:0,5 u žen. Nejpočetnější po primárních PC byly u mužů nádory trávicích, dýchacích a močových cest a pohlavních orgánů, u žen nádory trávicích cest, pohlavních orgánů, močových cest a prsu. Následným PC nejčastěji předcházely u mužů primární nádory kůže, trávicích, dýchacích a močových cest a pohlavních orgánů, u žen nádory kůže, pohlavních orgánů, trávicích cest a prsu. Z 562 primárních PC zemřelo k říjnu 2007 celkem 526 (93,6 %) nemocných a přežívalo jen 23 mužů a 13 žen. Český screeningový program a dispenzární péče se nejeví dostatečný vzhledem k riziku vícečetných PC, ale také ostatních nádorů GIT a zejména kolorekta. A total of 3,632 multiple PC registered in 1976–2005 in the Czech Cancer Registry there were 321 (1.5 %) primary and 1 581 (7.4 %) subsequent in males, 241 (1.2 %) primary and 1 489 (7.7 %) subsequent in females, of which were 11 % multiple PC of all new registered PC. Double PC were higher in females, multiple in males. The number of primary PC increased to 34 cases, of subsequent to 258 in 2005. Almost half of multiple PC in males and third of females were registered in the age group of 35–64 years. Of 3.632 multiple PC were distributed mostly in the Northern 11.8 % and Southern 11.1 % Moravia regions and in Prague 8.1 %. The average interval between primary PC and subsequent other neoplasms (ICD-10: C00–D48) was 3.2 years in males and 2.1 years in females, the ratio of synchronous to metachronous cases was 1:0.7 in males and 1:0.5 in females. The most frequent cases following primary PC in males were cancers of digestive, respiratory, urinary and genital tract, in females these were cancers of digestive, genital and urinary tract and breast. The most frequent cancers preceding the next subsequent PC included in males primary cancers of the skin, digestive, respiratory, urinary and genital tract, in females cancers of the skin, genital and digestive tract and breast. Up to October 2007, of 562 primary PC died 526 (93.6 %) patients and survived only 23 males and 13 females. The Czech screening programme and medical surveillance is decisive in view of risk of multiple PC and digestive, especially colorectal cancers.
