Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Cíl. Zhodnotit první zkušenosti s využitím 18F-FCH-PET/CT v hodnocení účinků embolizační léčby hepatocelulárního karcinomu (HCC). Metoda. Byl hodnocen soubor 5 zobrazení 18F-FCH-PET/CT u mužů nemocných s transarteriální embolizaci (průměrný věk 69,6 let v rozpětí 59-83 let). Vyšetření byla provedena po aplikaci 18F-fluorocholinu ( 18F-FCH) v dávce 2 MBq/kg, součástí vyšetření bylo plně diagnostické dvoufázové MDCT. Byla zhodnocena vaskularizace ložiska a akumulace radiofarmaka. Výsledky. U všech nemocných šlo o dobře diferencovaný HCC. Ve čtyřech případech po embolizační terapii (1krát prostá embolizace pro krvácení, ve třech případech TACE) bylo nalezeno reziduum, ve třech případech byla rezidua tkáně HCC spíše hypervaskularizovaná, ale akumulace FCH byla zvýšená v menším rozsahu, naopak u hypovaskularizované tkáně ukázala vysoká metabolická aktivita viabilitu nádoru. U dvou nemocných byla nalezena nová ložiska. Závěr. 18F-FCH-PET/CT poskytuje komplexní informace o vaskularizaci a metabolické aktivitě HCC a zpřesňuje informace o viabilitě nádorového postižení., Aim. To evaluate an early experience with 18F-FCH-PET/CT in the therapy response assessments after trans-arterial embolisations of hepatocellular carcinoma (HCC). Method. The sample 18F-FCH-PET/ CT examinations was assessed in five men (mean age 69.6 years range 59-83) suffered from HCC after trans-arterial embolisation. All examinations were performed after intravenous administration of 18F-gluorocholine in the dose of 2 MBq per kilogram of body weight, the full diagnostic dual-phase MDCT forms the integral part of the examination. The vascularisation and the radio-pharmaceutical accumulation were assessed. Results. The well differentiated HCC was present in all patients. Signs of the recurrence were found in four men, hyper-metabolic all of them, in three, joined with hyper-vascularisation. But the increased 18F-FCH uptake was depicted in area less than one third of hypervascularized tissue. Hypo-vascularized hyper-metabolic viable tissue of HCC was found in one patient. Newly developed HCC foci were found in two patients.
