The article is focused on the key problems of long-term care provided to disabled seniors above 65 years of their age and disabled children in the Czech Republic from informal carers' point of view. The research is based on 25 semi-structured interviews with informal carers. The results show a serious shortage of (social and health care) services generally as well as for informal carers alone, shortage of useful information and medical devices. In some cases, insufficient coordinative and informative role of general practitioners and paediatricians is also a problem. Tento článek se zaměřuje na dostupnost zdravotní péče pro migranty v Německu v širších souvislostech současné migrace. Rozebírá legální nárok jednotlivých kategorií migrantů na zdravotní péči: migrantů s legálním pobytovým statusem, žadatelů o asyl a iregulérních migrantů. Zvláštní pozornost je věnována přístupu dětí ke zdravotní péči. Článek rovněž předkládá vybrané příklady dobré praxe ke zlepšení dostupnosti zdravotní péče a k začleňování migrantů do mainstreamového zdravotnického systému v Německu. - mecku, The article is focused on healthcare availability for migrants in Germany regarding broad context of current migration. The article represents an overview of the development of Czech health promotion in the last two decades. Current “state of art” and health promotion in international context are mapped. Major behavioral risk factors are described and analybesity in comparison to EU countries. Their impact on the Czech population health is discussed in terms of relevant diseases. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. Článek v širším historickém kontextu pojednává o dlouhodobě připravované reformě zdravotnictví v Nizozemsku, která byla završena v roce 2006. Analyzuje příčiny, které k reformě vedly a seznamuje s reformní strategií a jejím postupem. S odstupem několika let mapuje dopady reformy na dostupnost zdravotní péče, chování poskytovatelů i zdravotních pojišťoven a na pacienty. Nastiňuje ekonomické důsledky reformy a některé etické souvislosti., The article is focused on the long-planned health care reform in the Netherlands completed in 2006 in a broader historical context. The aim of the text is: firstly, to analyse the causes of the reform and secondly, to discuss reform strategy as well as its progress. After several years since reforms implementation there are mapping their impact on the access to health care and behaviour of health providers, insurance companies and patients.
