Úvod: Informace o prevalenci orgánových komplikací diabetu včetně syndromu diabetické nohy v České republice je možné získat z dokumentů Ústavu zdravotnických informací a statistiky. Cíl: Posoudit vývoj vysokých amputací a drobných výkonů na noze v letech 2010–2014 v reprezentativním vzorku populace pacientů s diabetes mellitus vedených v databázi Všeobecné zdravotní pojišťovny (VZP) České republiky. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10. Následně jsme identifikovali osoby, které v letech 2010–2014 současně prodělaly chirurgický výkon na noze z důvodu diagnózy diabetické nohy. Výsledky: Absolutní počet amputačních výkonů na končetině vykazuje stacionární úroveň. Závěr: Předkládaná analýza je prvním zhodnocením vývoje výkonů na diabetické noze v ČR. Je zřetelný setrvalý stav v počtu výkonů na diabetické noze, a to jak vysokých amputací, tak drobných výkonů na noze. V kontextu s absolutním nárůstem pacientů, léčených pro diabetes mellitus, je stacionární stav známkou relativního poklesu, což je příznivé především v oblasti amputace dlouhých kostí. Klíčová slova: amputace – diabetes mellitus – syndrom diabetické nohy, Introduction: Information about the incidence of organ-affecting complications of diabetes, including the diabetic foot syndrome, can be obtained from the documents of the Institute of Health Information in the Czech Republic. Goal: Assessment of the development of high amputations and minor surgical procedures on the lower limb from 2010 to 2014 in a representative sample of the population of patients with DM kept in the General Health Insurance Company of the Czech Republic database. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or any antidiabetic therapy prescribed (ATC group A10) in the period of 2010 – 2014. A set of patients who had an agent from A10 group prescribed at least once in the given year was extracted for analysis. In the next step we identified individuals, who in the period of 2010 – 2014 also underwent a surgical procedure on the lower limb due to diabetic foot. Results: An absolute number of lower limb amputations remains at a stationary level. Conclusion: The submitted analysis presents the first assessment of the development of surgical treatment of diabetic foot in the Czech Republic. The amount of surgical procedures on the diabetic foot remains stable, regarding both high amputations and lower limb minor surgical procedures. In the context of an absolute increase of patients treated for diabetes mellitus, the stationary state is an indication of a relative decrease, which is favourable in particular with regard to the amputation of long bones. Kolorektální karcinom je druhým nejčastějším maligním onemocněním ve vyspělých zemích. Vzhledem k tomu, že proces kancerogeneze sporadického karcinomu probíhá relativně pomalu, je možné tomuto onemocnění úspěšně předcházet. Národní screening kolorektálního karcinomu byl v České republice zahájen v roce 2000 a od této doby prochází kontinuálním vývojem s cílem zvýšení efektivního pokrytí cílové populace. V současnosti je k dispozici dvouetapový screeningový program (test na okultní krvácení do stolice a v případě pozitivity kolonoskopie), nebo jednoetapový program (primární screeningová kolonoskopie). Kvalita programu je hodnocena pomocí indikátorů kvality – zejména záchytu adenomů (adenoma detection rate, ADR) a podílu totálních kolonoskopií (cecal intubation rate, CIR). Díky screeningovému programu dochází v ČR k dlouhodobé stabilizaci incidence a mírnému poklesu mortality na kolorektální karcinom. K dalšímu snižování incidence a mortality onemocnění je nutná vyšší účast cílové populace za pomocí adresného zvaní cílové populace a mediální kampaně., Colorectal cancer is the second most common malignant disease in developed countries. Given that the process of sporadic cancer carcinogenesis is relatively slow, it is possible to successfully prevent this disease. Czech National Colorectal Cancer Screening Program was launched in 2000 and since that time is in continual development in order to increase the effective coverage of the target population. It is currently available as two-step screening program (stool test for occult blood and colonoscopy) or one-step program (primary screening colonoscopy). Quality of the program is evaluated using performance indicators – namely adenoma detection rate (ADR) and cecal intubation rate (CIR). The screening program in the Czech Republic enables long-term stabilization of incidence and a slight decrease in mortality from colorectal cancer. Úvod: Stanovení strategie léčby karcinomu rekta vyžaduje komplexní a multidisciplinární přístup, na základě kterého je indikována racionální léčba. K primární resekci jsou indikována časná stadia karcinomu rekta, naopak lokálně pokročilé tumory by měly být předléčeny některým z režimů předoperační (chemo)radioterapie. Hlavním cílem této práce bylo zmapovat strategii léčby pacientů s karcinomem rekta v České republice a zejména sledovat výskyt kompletní patologické odpovědi při neoadjuvantní léčbě. Metody: Jde o retrospektivní multicentrickou klinickou studii, která zahrnuje data všech pacientů s karcinomem rekta, kteří byli operováni v zúčastněných centrech v období od 1. 1. 2011 do 31. 12. 2012. Požadovaná data byla zadávána do online registru PATOD C20. Byly stanoveny tři okruhy otázek: 1. charakteristika pracoviště a spolupráce s onkologií; 2. analýza léčby pacientů s karcinomem rekta; 3. detailní analýza skupiny pacientů s kompletní patologickou odpovědí. Analýza byla provedena s ohledem na charakter jednotlivých pracovišť, tj. univerzitní centra s KOC, neuniverzitní centra s KOC a centra mimo KOC. Výsledky: Do studie bylo zařazeno celkem 21 chirurgických pracovišť České republiky, která poskytla data 1860 pacientů. Strategie léčby karcinomu rekta byla určována mezioborovými semináři v 19 centrech (90,5 %). Statisticky významné rozdíly byly zjištěny mezi jednotlivými centry v indikaci neoadjuvantní léčby (p<0,001), provedených resekcí rekta s anastomózou (p=0,048) a resekcí bez anastomózy (p=0,022). Kompletní patologická odpověď byla nalezena celkem u 61 (8,7 %) pacientů. Postižení lymfatických uzlin (ypN+) bylo nalezeno v případě vymizení tumoru ve stěně rekta (ypT0) u celkem 7 (9,7 %) pacientů. Závěr: Studie PATOD prokázala, že péče o karcinom rekta je v ČR velmi heterogenní. Univerzitní a velká centra při KOC, i přes vytvořené nejlepší podmínky, této výhody plně nevyužívají. The primary rectal resection is indicated in early stages, while locally advanced tumors should be pretreated by one of the modes of neoadjuvant (chemo) radiotherapy. The main goal of this study was to explore the therapeutic strategy in patients with rectal cancer in the Czech Republic. The second aim was to determine the incidence of the pathological complete response after neoadjuvant therapy. Methods: This is a retrospective multicenter clinical study, which includes data from all patients with rectal cancer who were treated at participating centers in the period from 01/01/2011 to 31/12/2012. The required data has been passed into the online registry PATOD C20.Three issues have been set up: 1. Characteristics of the center and cooperation with the oncological department; 2. Characteristics of the treatment of patients with rectal cancer; and 3. Detailed analysis of the group of patients with complete pathological response. The analysis was performed with regard to the nature of individual departments, i.e. departments of surgery in university hospitals with complex oncological centres, departments of surgery within complex oncologic centers, and departments of surgery outside complex oncologic centers Results: In total, 21 departments of surgery in the Czech Republic provided data about 1860 patients with rectal cancer for the study. The treatment strategy for rectal cancer was determined at multidisciplinary seminars at 19 centers (90.5%). Statistically significant differences between the centers were found in the indication for neoadjuvant treatment (p<0.001), rectal resection with anastomosis (p = 0.048), and resection without anastomosis (p = 0.022). Complete pathological response was found in 61 (8.7%) patients. Positivity of mesorectal lymph nodes (ypN+) was found in the case of ypT0 stage in 7 (9.7%) patients. Conclusion: PATOD study showed that therapy of rectal cancer is highly heterogeneous in the Czech Republic. Despite the best conditions provided, university hospitals and large departments within complex oncologic centers do not fully utilize this benefit. Po téměř deseti letech od zahájení screeningu kolorektálního karcinomu (KRCA) v České republice došlo ke změně jeho programu. Hlavní epidemiologické ukazatele KRCA, incidence a mortalita, pro nás zůstávají nepříznivé. Sekundární prevence KRCA zatím nenaplňuje očekávání. Úprava programu spočívá ve volbě vlastní screeningové metody a modifikaci jejího intervalu. Zvýšení adherence populace ke screeningu se očekává od nové alternativy, primární screeningové kolonoskopie. Kvalitativní změnu přináší imunochemický test na okultní krvácení, který se jak u praktických lékařů a gynekologů, tak u pacientů těší stále větší oblibě. Prohlubuje se spolupráce mezi jednotlivými odbornostmi participujícími na screeningu. Prostřednictvím médií i jiných marketingových akcí se zvyšuje informovanost české populace. Data o výkonech poskytovatelů za rok 2009 naznačí, zda screening v nové podobě je krokem správným směrem. Efekt screeningu ovšem budeme moci zhodnotit až s odstupem několika let., Almost ten years after the introduction of colorectal cancer (CRC) screening in the Czech Republic, there has been a change in the programme. The major epidemiological indicators of CRC, i. e. incidence and mortality, still remain unfavourable. Secondary prevention of CRC has not fulfilled the expectations so far. The change of the programme consists in choosing an individual screening method and modifying its interval. Primary screening colonoscopy, which is a new alternative, is expected to increase the adherence of the population to the screening. A qualitative change has been brought by the use of an immunochemical faecal occult blood test that is becoming increasingly popular with general practitioners and gynaecologists as well as patients. The cooperation among the individual disciplines involved in the screening programme has become more in-depth. Through the media and other marketing strategies, the awareness of the Czech population has been rising. The 2009 data on the procedures done by providers will suggest whether this new form of screening is a step in the right direction.
