Cíl: posoudit možnosti hodnocení N-stagingu karcinomu prsu pomocí 18F-FDG-PET/CT. Metodika: V souboru 26 žen vyšetřených před operací prsu pomocí 18F-FDG-PET/CT byl porovnán výkon zobrazení pomocí dvou přístrojů se tříprstencovým detektorem PET (15 žen) a se čtyřprstencovým detektorem PET (11 žen), kdy ve druhé skupině bylo použito techniky rekonstrukce obrazu time-of-flight a point spread function a současně snížení dávky aplikovaného radiofarmaka na 2, 5 MBq/kg ve srovnání s 4 MBq/kg. Výsledky: V celém souboru bylo dosaženo pro hodnocení uzlinových metastáz v axile senzitivity 85%, specificity 65%, u nemocných vyšetřených pomocí zařízení s time-of-flight a point spread function bylo dosaženo významně lepších výsledků se senzitivitou 100 % a specificitou 80 %. Závěr: Použitím rekonstrukce obrazu time-of-flight a point spread function dosahuje lepších výsledků v hodnocení uzlinových axilárních metastáz i za současného snížení dávky aplikovaného radiofarmaka na 2,5 MBq/kg., Aim: to evaluate the possibilities of the N--staging assesment of the breast carcinoma using 18F-FDG-PET/CT. Method: In 26 women examined before surgery using 18F-FDG-PET/CT, the performance was compared when the three-ring PET system was used (15 women) and four-ring PET system with image reconstruction using time-ofiflight and point spread function even with the radiopharmaceuticals dose reduction to 2.5 MBq/kg from 4 MBq/kg. Results: The sensitivity to detect axillary lmph nodes metastases reached 85%, specificity of 65% in the whole sample, in patients examined by the system enabling the reconstruction time-of-flight and point spread function, the sensitivity was improved significantly to 100% with specificity of 85%. Cíl. Zhodnotit možnosti hybridního zobrazení a frekvenci používaných výkonů v detekci okultního muskuloskeletálního pyogenního zánětu jako příčiny sepse. Metoda. Retrospektivně byly zhodnocena hybridní zobrazení indikovaná během 3 let k detekci příčiny septického stavu nejasného původu, vyšetření byla vyhledána mezi všemi provedenými 18F-FDG-PET/CT a SPECT/CT. Výsledky. Bylo provedeno celkem 27 vyšetření 18F-FDG-PET/CT z indikace sepse nejasného původu, kdy v šesti případech byla nalezena spondylodiscitida, pyogenní artritida ve třech případech, svalový absces ve dvou případech a infekce endoprotézy ve čtyřech případech. Muskuloskeletální příčina sepse tvořila 55,5 % případů. Bylo provedeno celkem 16 vyšetření SPECT/CT, kdy v deseti případech byly použity techneciem značené fragmenty myších protilátek a v šesti případech leukocyty značené 99mTc-HMPAO. Ve dvou případech byla prokázána akutní spondylodiscitida, v jednom případě chronická spondylitida a v jednom případě mnohočetné abscesy svalů původem v infekci endoprotézy. Muskuloskeletální příčina tvořila 25 % nálezů. Závěr. Za dobu současného používání PET/CT a SPECT/CT v detekci okultních infekcí tvoří větší díl vyšetření pomocí 18F-FDG-PET/CT, neboť nabízí vícečetné výhody před zobrazeními leukocytárními., Aim. To asses a role and frequency of indications of hybrid imaging methods in the detection of occult pyogenous musculoskeletal inflammation as a cause of the septic state of unknown origin. Method. The retrospective evaluation of the hybrid imaging indicated due a sepsis of unknown origin during three years was performed. All procedures of 18F-FDG-PET/CT and the SPECT/CT using labeled leucocytes were enrolled into the assessment. Results. 27 procedures of 18F-FDG-PET/ CT were performed due to the searching of the cause in cases of the sepsis of unknown origin; the spondylodiscitis was found in 6 cases, pyogenous arthritis in 3 cases, muscular abscesses in 2 cases and 4 cases of pyogenous infection of implanted joint prosthesis. Muskuloskeletal inflammations formed 55,5% of all findings. 16 procedures of SPECT/CT were performed, 10 using 99mTc-labeled antigranulocyte antibodies, and 6 using 99mTc-HMPAO labeled leukocytes. In 2 cases, the acute spondylodiscitis was found, one case of chronic spon-dylitis and one case of soft tissue abscesses due to the infection of endoprosthesis were found. Muskuloskeletal inflammations formed 25% of all findings. Conclusion. Cíl: Zhodnotit první zkušenosti s prováděním transarteriální radioembolizace (TARE) jaterních nádorů mikročásticemi s obsahem 90-yttria Metodika: V průběhu 6 měsíců jsme provedli u šesti nemocných TARE s aplikací skleněných mikročástic TheraSphere®. U čtyř nemocných šlo o metastatické postižení jater kolorektálním karcinomem, u dvou nemocných hepatocelulárním karcinomem. U nemocných jsme sledovali peri- a postprocedurální průběh, výskyt komplikací, WHO performance status a časný efekt léčby. Výsledky: Ani u jednoho z nemocných jsme nezaznamenali závažné komplikace léčby, při sledování účinků léčby bylo po 6 týdnech zjištěno, že u jedné nemocné došlo k rapidnímu zlepšení WHO performance status o tři kategorie, u ostatních byl status zachován, ani u jednoho nemocného v časném horizontu nebyla zaregistrována jaterní progrese onemocnění. Závěr: První zkušenosti s TARE ukazují i u nemocných s neresekabilním jaterním postižením dobrou toleranci léčby a slibné výsledky terapie., Aim: To evaluate an early experience with transarterial radioembolisation of liver tumors with microspheres with 90-yttrium. Method: During 6 monnths, we performed 6 months TARE with the application of glass-microspheres TheraSphere®. In 4 patients.colorectal carcinoma metastases were treated, in 2 hepatocellular carcinoma respectivelly In patients, the peri- and post-procedural disease development was surveyed according complications, WHO performance status an early effect of therapy. Results: No severe complication was noted, during 6 weeks was noted only one extreme improvement of WHO performance status with decrease by 3 grades, in rest of patients the performance status was unchanged. No progression in liver was noted within 6 weeks.
