Harmonizace výsledků měření v klinických laboratořích je zcela nezbytná k dosažení klinické efektivity laboratorního vyšetřování, jeho finanční efektivity a k redukci rizika pacientů v diagnostických a terapeutických procesech, způsobeného chybami měření. Podstatou harmonizace je dosažení hodnot referenčních intervalů a rozhodovacích diagnostických limitů, nezávislých na použité metodě měření a na laboratoři měření provádějící. Nástrojem harmonizace výsledků je standardizace metod měření, realizovaná návazností rutinních metod na referenční postupy a materiály. Data, popisující realizaci a možnosti harmonizace jsou uvedena zejména na obecně přístupných stránkách IFCC, JCTLM, NIST, IRMM. Je pojednáno o Stockholmské deklaraci kvality standardizačních procesů, o nevhodně malé šíři standardizace, o nedostatečné úrovni její dokumentace ze strany výrobců., Harmonization of laboratory results is necessary for approaching the clinical and economical effectiveness in laboratory examinations and also to reduction of patients risk in diagnostic and therapeutic processess due to measurement errors. Harmonization of results in clinical laboratories is based on the standardization of measurement methods and standardization is realized by traceability chains from reference procedures and certified reference materials to calibration of routine methods. The main goal of harmonization is establishment of commonly valid values of reference intervals and/or diagnostic decision limit, values independent on the used measurement nethods and location of laboratories. Communication introduces describing data on the harmonization, produced namely by IFCC, JCTLM, NIST, IRMM-EU. Six sigma je nejúspěšnějším nástrojem řízení kvality výroby a procesů současnosti. Intenzita jeho používání v průmyslu včetně produkce in vitro laboratorních diagnostik je velmi vysoká. Oproti tomu je její používání v klinických laboratořích laboratorní medicíny na samém počátku. Předmětem sdělení je souhrn základních poznatků a informací, které by měly být součástí souboru potřebných znalostí odborníků pracujících v klinických laboratořích. Je uvedena kalkulace hodnot sigma ze zjištěného počtu chyb, závad a neshod v procesech a ze vztahů mezi hodnotami kontrolních limitů, bias a přesnosti. Výsledky Six sigma mohou být používány k hodnocení preanalytických, postanalytických i vlastních analytických procesů laboratorních vyšetření. Při korektním provedení kalkulací lze hodnoty sigma považovat za univerzální kritéria hodnocení indikátorů kvality a za účinný kvantitativní nástroj ke srovnání reálné úrovně kvality jak mezi laboratořemi, tak i mezi výrobními odvětvími. Six sigma může velmi účinně a názorně hodnotit relace mezi analytickými parametry měření a klinickými požadavky na jejich výsledky. Stejně tak dobře může posuzovat chybování neanalytických procesů ve vztahu k míře rizika zdravotní péče. It is very often used in industry including the produce of laboratory in vitro diagnostics. The use in clinical laboratories is however at present day only rare. Summary of basic information and knowledge on this method is the objective of our presentation. We introduce calculation of sigma values from number of defects (errors) of processes and also its calculation from tolerance limits, bias and precision for assessment of analytical results. The Six sigma concept may be regarded as universal criterion for quantifi cation of quality indicators and also for comparison of quality level between laboratories. Six sigma can serve as tool for assessing the relation between analytical effi ciency and clinical requirements and for assessing the extraanalytical phases as sources for risk of healthcare of patients.
