Počet seniorů absolvujících i delší a náročnější cesty i ve vyšším věku stoupá. Chronické choroby společně s fyzickými i psychickými nároky cesty činí seniora vulnerabilním. Geriatr i praktický lékař by měli být schopni vyhodnotit rizika, která mohou vést k dekompenzaci stavu v průběhu vlastní cesty, a v součinnosti s cestovatelem samotným je minimalizovat. Jsou rozebírána rizika kardiovaskulární vzhledem k dlouhým letům a pobytu v horkém prostředí - komplikace ischemické choroby srdeční, zvláště některé souvislosti výskytu infarktu myokardu během cesty, pro který byly vypozorovány tyto rizikové faktory: krátkodobé plánování cesty, neobvyklá destinace, nižší úroveň vzdělání, život v partnerském svazku, cestování autem oproti ostatním dopravním prostředkům, bydlení ve stanu nebo přívěsu/ campingovém voze oproti bydlení v hotelu. Z hlediska časového se akutní komplikace objevovaly nejčastěji v prvních dvou dnech cesty. Tromboembolické komplikace jsou diskutovány s ohledem na nové aspekty economy class syndromu. Pozornost je věnována arytmiím, komplikací hypertenze, oběhovému selhání, průjmovým onemocněním a jejich možným důsledkům. Na závěr jsou uvedeny dvě kazuistiky a tabulka s doporučeními pro minimalizaci rizik cesty., The number of seniors undertaking longer and more demanding journeys at an advanced age is increasing. Chronic illnesses together with the physical and mental demands of travelling make seniors vulnerable. Geriatricians and general practitioners should be able to assess risks that could lead to the deterioration of a condition during travel and co operate with travellers to minimise such risks. The article analyses the cardi ovascular risks associ ated with a long flight and a stay in a hot climate - complications in ischemic heart disease, especially certain correlates of myocardial infarcti on during travel, for which the identified risk factors were short-term planning of the journey, unusual destinations, a low level of education, life in a couple, travel by car in comparison with other forms of transport, accommodation in a tent or trailer/ camper van compared to accommodation in a hotel. In terms of time, acute complicati ons occurred most frequently in the first two days of travel. Thromboembolic complications are discussed with regard to the new aspects of economy class syndrome. Attention is also given to arrhythmia, complications of hypertension, circulation failures, diarrhoe a and their possible consequences. Je konstatována nutnost zabývat se psychickým stavem starších nemocných a je poukázáno na dobrý efekt podaných antidepresiv., The author describes 2 cases of depression in older patients which is caused by the situations typical just for the seniors - loss of partner and cumulating of diseases. Autoři prezentují případ 73leté ženy léčené v posledních 15 letech pro arteriální hypertenzi diuretiky (tiazidy, amiloridem a indapamidem) a současně antidepresivem trazodonem. Po 6 letech této léčby začala pacientka trpět manifestní osteoporózou s vícečetnými frakturami i po banálním traumatu a současně byla po celou tuto dobu dokumentována těžká hyponatremie bez adekvátní suplementace. Osteoporóza a hyponatremie u této pacientky spolu s největší pravděpodobností těsně souvisely – autoři diskutují možné patofyziologické mechanizmy na úrovni renálních tubulů, osteoblastů a osteoklastů, hypofyzárně-skeletální osy, syndrom neadekvátní sekrece adiuretinu (SIADH) a možný vliv porušené acidobazické rovnováhy., The authors present a case-report of a 73years old woman treated for arterial hypertension within past 15 years with diuretics (thiazides, amiloride and indapamide) and concomitantly with antidepressive trazodone. The patient developed severe osteoporosis after 6 years of such a treatment with multiple fractures even after minor trauma; during whole this period severe hyponatremia without adequate supplementation was documented. Most probably, there exists tight relationship of osteoporosis and hyponatremia in this patient – the authors discuss possible pathophysiological mechanisms at the level of renal tubules, osteoblasts and osteoclasts, hypopituitary-skeletal axis, syndrome of inadequate adiuretin secretion (SIADH) and possible influence of acid-base balance disturbances., and Pavel Polák, Petr Husa, Hana Matějovská Kubešová
