INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Úvod: Jedná se o retrospektivní analýzu souboru nemocných s mnohočetným myelomem ošetřených ve FN Hradec Králové v období od 1/2007 do 6/2010 pomocí perkutánní vertebroplastiky. Materiál a metodika: U 13 nemocných (3 muži a 10 žen), medián věku 65 let, bylo celkem ošetřeno 18 obratlových těl v oblasti thorakolumbální páteře. Sledovali jsme bolesti pomocí VAS (visual analoque scale), četnost komplikací, funkční výstup nemocných (stejný, zlepšený, zhoršený) a spotřebu analgetik (zvýšená, stejná, snížená). Nemocní byli ošetření na angiografickém kompletu transpedikulárním či posterolaterálním přístupem. Výsledky: Provedení perkutánní vertebroplastiky mělo statisticky významný vliv na pokles bolesti po výkonu (p = 0,002). Přitom jsme neprokázali žádnou symptomatickou komplikaci, k asymptomatickému úniku kostního cementu mimo obratlové tělo došlo u 11 % nemocných. Zlepšení úrovně života udávalo 100 % nemocných. Snížení spotřeby analgetik jsme prokázali u 92 % nemocných a u zbývajících 8 % zůstala spotřeba analgetik stejná. Závěr: Perkutánní vertebroplastika je miniinvazivní zákrok s minimem komplikací a rychlým nástupem analgetického efektu u nemocného., Introduction: We present retrospective analysis of a group of patients with multiple myeloma treated in FN Hradec Králové from 1/2007 to 6/2010 by percutaneous vertebroplasty. Material and methods: A total of 18 vertebral bodies were treated in thoracolumbar spine region in 13 patients (3 men and 10 women), median age of 65 years. We watched the pain using the VAS (visual analoque scale), frequency of complications, functional outcome of patients (same, improved, worsened) and consumption of analgetics (increased, same, decreased). Patiens were treated at angiographic suit using transpedicular or posterolateral access. Results: Vertebroplasty has a statistically significant effect on the pain relief after procedure (p = 0,002). We didn´t find symptomatic complications, asymptomatic leakage of bone cement outside the vertebral body was observed in 11 % of patients. Improving the standard of living was reported by 100 % of patients. Reducing consumption of analgetics has been confirmed in 92 % and 8 % of patients remained the same consumption of analgetics. Bortezomib (Velcade) je lék ze skupiny inhibitorů proteasomu. Představuje zcela nový a jedinečný léčebný postup v léčbě mnohočetného myelomu (MM). Tato práce přináší retrospektivní analýzu léčebné účinnosti a nežádoucích účinků u nemocných s MM léčených v referenčních centrech CMG v České republice. Celkem bylo hodnoceno 424 nemocných MM léčených bortezomibem od června 2004 do června 2010. Medián délky sledování byl 12,4 měsíců. Průměrný věk nemocných byl 66 let (34,1–86,5). Jednalo se především o nemocné s relabujícím MM. Celkem 48,3 % (205/424) nemocných mělo bortezomib v rámci druhé léčebné linie. Léčebnou odpověď bylo možné hodnotit u 83,3 % (353/424) nemocných. Léčebné odpovědi (? 50% pokles MIg) bylo dosaženo u 52,7 % (186/353), z toho bylo u 14,2 % (50/353) nemocných dosaženo kompletní remise. Nejzávažnější toxicitou byla periferní neuropatie stupně 3 a vyšší u 14 % (49/358) nemocných. Trombocytopenie stupně 3 a vyšší byla u 28,8 % (103/357) nemocných. Infekční komplikace stupně 2 a vyšší byly u 33,6 % (121/359) nemocných. V průběhu léčby bortezomibem nedošlo v našem souboru nemocných ke zvýšení frekvence výskytu trombo embolických komplikací. Mimořádný přínos bortezomibu v klinické praxi je dán rychlým nástupem léčebné odpovědi a vysokým počtem dosažených kompletních remisí. Většina jeho nežádoucích účinků se zpravidla daří zvládat pomocí vhodných profylaktických opatření, případně krátkodobým přerušením a následným snížením dávky podle platných doporučení., Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of the total 424 patients, 205 patients had bortezomib as second line therapy (48,3 %). Assessment of terapeutic response was possible in 83,3 % of treated patients (353/424). Therapeutic response (defined as a ? 50 % decrease in the serum MIG) was achieved in 52,7 % (186/353). Among mentioned responses, complete response was obtained in 14,2 % (50/353). The most severe toxicity observed was peripheral neuropathy, grade 3 or higher was developed in 14 % (49/358). There was recorded a thrombocytopenia grade 3 or higher in 28,8 % (103/357), further occurance of infections grade 2 or higher in 33,6 % (121/359) and eventually we could observe increased number of thromboembolic disease as well. Rapid onset of therapeutic responses and high percentage of obtained complete responses are the main reasons of extraordinary clinical benefit of bortezomib.
