Předpokládá se, že v příštích dvou dekádách stoupne počet diabetiků ve světě až na 300 milionů. Z toho se odhaduje, že nejméně u 9 z 10 případů se bude jednat o diabetes 2. typu (DM2T). Metabolický syndrom, který je charakterizován inzulínovou rezistencí, je časnou a častou formou manifestace diabetes mellitus. Diabetes 2. typu je v současnosti hlavní klinickou výzvou u dospělé populace, protože se společně se syndromem inzulínové rezistence zvláště podílí na kardio¬ vaskulární mortalitě a morbiditě. Prevalence DM rapidně roste se stoupajícím zastoupením starých osob v populaci. Progresivní povaha diabetů 2. typu znamená, že účinná terapie by měla být zahájena co nejdříve po stanovení diagnózy s cílem dosáhnout metabolické stability. Perorální antidiabetika (PAD) zůstávají nejčastější farmakoterapeutickou intervencí u diabetů 2. typu. Probrány jsou jednotlivé skupiny PAD-, inzulínové senzitizéry, inhibitory alfaglukozidázy, inzulínová sekretagoga: sulfonylureová i nesulfonyulreová (deriváty meglitinidu). Pozornost je věnována perspektivním přípravkům ( mj. thiazolidindionům), které vzdor všem limitacím představují nové možnosti v terapii DM2T v dohledné budoucnosti. Probrány jsou také možnosti kombinační terapií PAD (např. možnost kombinace PAD a inzulínu) a indikace inzulinoterapie u DM2T. Při posuzování nejlepších terapeutických postupů je třeba pamatovat na to, že jak inzulínová rezistence, tak i inzulínový deficit jsou daleko nejčastější příčinou DM2T., Within the next two decades the number of patients with diabetes worid-wide is expected to rise to 300 million. It is estimated that out of this number at least 9 out of 10 cases will be diabetes type 2 (DM2T). The metabolic syndrome, characterised by insulin resistance, is an early and frequent manifestation of diabetes. At present, DM2T is a major clinical challenge in adult population, as it contributes significantly, along with the syndrome of insulin resistance, to cardiovascular morbidity and mortality. The prevalence of diabetes is increasing rapidly with the growing percentage of elderly people in the population. The progressive nature of DM2T implies that effeaive therapy should be initiated as soon as possible following the diag¬ nosis with the aim to stabilise metabolism. Oral antidiabetic drugs remain the most frequent pharmacological intervention in DM2T. Individual classes of oral drugs are discussed: insulin sensitisers, alfaglucosidase inhibitors, insulin secretagogues: sulphonylureas and non-sulphonylureas (meglitinid derivatives). The authors discuss perspective drugs (thiazolidindiones among others) which, despite a number of limitations, present new opportunities for the treatment of DM2T in foreseeable future. Furthermore the possibilities of combination therapies using oral drugs (also the possibility of combining oral drugs and insulin) as well as indications for insulin therapy in DM2T are discussed. Diabetes mellitus 2. typu (DM2T) je provázen celou řadou odchylek, které zvyšují pravděpodobnost rozvoje aterosklerózy. Výzkumy posledních let ozřejmily jak význam vztahu diabetů a aterosklerotických komplikací, tak jejich komplikovanost. Kardiovaskulární choroby jsou příčinou úmrtí 3/4 diabetiků. V prevenci kardiovaskulárního rizika samotná korekce hyperglykemie zklamala očekáváni. Jedinými dosavadními intervencemi, jež se ukázaly účinné a snižují výskyt makrovaskulárních komplikací ve spojení s diabetem, jsou ty, jež příznivě modifikují koexistující rizikové faktory KV-chorob. Proto komplexní léčba diabetů neznamená jen příslušnou léčbu nefarmakologickou, ale kromě antidiabetické léčby a edukace musí zahrnovat i léčbu hypertenze, dyslipoproteinemií, včetně zákazu kouření. Obezita provází diabetes 2. typu v 80-90 % a podílí se jak na metabolických, tak na kardiovaskulárních komplikacích diabetů. Úspěšná a trvalá redukce váhy snižuje všechna zdravotní rizika. Obrat ve farmakoterapii přinesly nové typy antiobezitik, která jsou bezpečgčná i při dlouhodobém podávání (sibutramin, oriistat). Metabolický syndrom - inzulínová rezistence i DM2T jsou provázeny aterogenní konstelaci metabolických rizikových faktoru, a navíc se problematika KV-onemocnění přesouvá u diabetiků i nediabetiků do vyššího věku. Z těchto důvodů se věnuje i větší pozornost terapii dyslipoproteinemie. Léčebná schémata se posouvají k vyšší agresivitě terapie (při současném zachování bezpečnosti). Probrány jsou standardní postupy léčby dyslipoproteinemie u diabedků 2. typu: své již jisté místo maji s atiny a fibráty v sekundární i primární prevenci KV-postižení. Rovněž striktní kontrola krevního tlaku u osob s diabetem vede ke snížení makrovaskulárních komplikací. Za antihypertenziva první volby u hypertenzních diabetiků jsou v současné době považovány inhibitory angiotenzin konvertujícího enzymu, selektivní betablokátory, Ca2-blokátory, alfa-1-blokátory. Zvláštní postavení ve strategií s agresivními terapeutickými cílí mají v léčbě hypertenze u diabetiků ACEI a ARB. U všech starších diabetiků zvažujeme zařazeni preventivního užívání aspirinu do základního terapeutického schématu., Diabetes mellitus type 2 (DM2T) is accompanied by a number of alterations increasing the risk of development of atherosclerosis. Recent research has shed more light both on the importance of the relationship between diabetes and atherosclerotic complications and the inherent complexity. Cardiovascular disease is a cause of death in some 75% of patients with diabetes. With regard to the prevention of cardiovascular risk, correcting hyperglycaemia itself has not met our expectarions. The only interventions so far shown to be efficient and to reduce the incidence of cardiovascular complications associated with diabetes are those that have a favourable effect while modifying coexisting cardiovascular risk faaors. Thus, comprehensive therapy of diabetes does not only mean adequate non-pharmacological management but, in addition to antidiabetic therapy and education, it should also include therapy of hypertension, dyslipoproteinaemia, and cessation of cigarette smoking. Obesity accompanies diabetes in 80-90%, thus participating both in metabolic and cardiovascular complications of diabetes. Successful and sustained weight reduaion reduces all health-related risks. New classes of antiobesity drugs present a turnaround in pharmacological management, providing safety even with long-term use (sibutramin, oriistat). The metabolic syndrome of insulin resistance as well as DM2T are accompanied by an atherogenous constellation of metabolic risk faaors while, in addition, the issue of cardiovascular disease in both diabetic and nondiabetic patients is shifted towards higher age. For this and other reasons, the management of dyslipoproteinaemia receives currently more attention. The management patterns move towards more aggressive therapies (while maintaining adequate safety). The authors discuss standard procedures for the treatment of dyslipoproteinaemia in patients with diabetes type 2: statins and fibrates already occupy their steady position both in primary and secondary prevention of cardiovascular injury. In addition, rigid control of blood pressure in patients with diabetes contributes to the reduaion of maaovascular complications. At present, inhibitors of angiotensin converting enzyme, seleaive beta-blockers, calcium channel blockers and alpha 1-blockers are considered to be the first-line antihypertensive drugs in patients with hypertensive patients with diabetes. ACEI and ARB occupy a special position in strategies with aggressive therapeutic goals for the treatment of high blood pressure in patients with diabetes.
