A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. The patient underwent multiple consecutive operations and interventions having, among others, finally replaced the entire thoracic aorta with the use of the hybrid elephant trunk technique. and T. Autoři popisují případ 78leté ženy s masivní hemoptýzou, zdrojem které byla hypertrofická bronchiální tepna anomálně odstupující z a. thoracica interna. CT angiografie provedená na multidetektorovém CT přístroji ektopickou bronchiální tepnu zobrazila. Následná selektivní embolizace dané tepny akrylátovým lepidlem vedla k zástavě hemoptýzy. Nemocná neměla recidivu krvácení během 24měsíčního sledování. Anomální odstup bronchiální tepny není vzácný a proto je nutné na něj myslet zejména v případech, kdy není patrná žádná tepna zásobující bronchy v normální anatomické lokalizaci. CT angiografie provedená na multidetektorovém CT přístroji má vysokou úspěšnost v detekci odstupu bronchiálních tepen a slouží k navádění při jejich katetrizaci a embolizaci., The authors describe a case of a 78-year-old woman with massive hemoptysis, which was from to hypertrophic bronchial artery anomalously originating from the internal thoracic artery. CT angiography performed on multidetector CT shows anomalous origin of the right bronchial artery. The bronchial bleeding was treated with embolization. The patient had no recurrent bleeding during 24 month follow-up. Ectopic origin of bronchial arteries is not rare, therefore it is necessary to consider it especially in cases when no bronchial artery arises between the upper margin of the fifth thoracic vertebra and the lower margin of the sixth thoracic vertebra. Osteochondrom (exostóza) je nejčastějším benigním kostním tumorem. Ve většině případů jde o solitární lézi. Mnohočetná hereditární osteochondromatóza představuje vzácné autozomálně dominantně dědičné onemocnění charakterizované růstem vícečetných exostóz především na metafýzách dlouhých kostí, lopatách kosti kyčelní, žebrech, obratlích a na lopatkách. Cévní komplikace mnohočetné osteochondromatózy jsou velice vzácné, přičemž nejčastěji postihují popliteální tepnu.
