Cíl: Cílem pilotní studie bylo zjistit, zda dochází v průběhu klokánkování ke snížení srdeční a dechové frekvence u nedonošených novorozenců, zjistit zda klokánkování ovlivní periferní saturaci krve kyslíkem a délku hospitalizace nedonošených novorozenců na Jednotce intenzivní a resuscitační péče o novorozence. Dále zjistit, zda klokánkování ovlivňuje interakci matka - dítě. Metodika: Metodou studie byl experiment. Technikou sběru dat k posouzení efektu klokánkování bylo měření fyziologických funkcí. Interakce mezi matkami a jejich dětmi byly měřeny dotazníkem - Maternal Postnatal Attachment Scale (MPAS). Ke statistickému zpracování byl využit t-test pro dva výběry, neparametrický Wilcoxonův test, chí test a Fisherův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z fyziologických funkcí byl signifikantně potvrzen vzestup tělesné teploty. Při porovnání intervenční a kontrolní skupiny před klokánkováním nebyl v interakcích matek k jejich dětem zaznamenán žádný signifikantní rozdíl. Před propuštěním (resp. po klokánkování) byl zájem o dítě u matek klokánkovaných dětí významně vyšší (p = 0,0354) oproti matkám, které své dítě neklokánkovaly. Ve všech položkách MPAS byla úroveň připoutání ke klokánkovanému dítěti před propuštěním vyšší než u neklokánkovaných novorozenců. Závěr: Klokánkování prokázalo pozitivní efekt na nedonošené novorozence v oblasti vzestupu tělesné teploty, zvýšeného zájmu o potřeby novorozence a silnějšího připoutání matky k novorozenci., Objective: The pilot study aimed at determining whether kangaroo mother care decreases heart and respiration rates in preterm newborns, affects saturation of peripheral oxygen and length of newborns’ hospital stay at a neonatal intensive care unit, and influences mother-child interaction. Methods: The method of study was an experiment. To assess the effect of kangaroo mother care, physiological functions were measured. Mother-child interaction was assessed using the Maternal Postnatal Attachment Scale (MPAS). Statistical analyses were carried out with a two-sample t-test, non-parametric Wilcoxon test, chisquare test and Fischer’s exact test. The statistical tests were evaluated at a significance level of 5%. Results: Of the physiological functions studied, only body temperature was confirmed to be significantly increased. There was no significant difference in mother-child interaction prior to kangaroo care between the study and control groups. After kangaroo care (i.e. prior to hospital discharge), interest in their babies was significantly higher (p=0.0354) in mothers providing kangaroo care than in the controls. In all MPAS items, pre-discharge levels of maternal bonding were higher in the group of children provided with kangaroo care than in the controls. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care.
