Kardiorenální syndrom je onemocnění srdce a ledvin, kdy akutní či chronické onemocnění jednoho orgánu vede k akutnímu či chronickému postižení orgánu druhého. Srdeční selhání vede k poklesu srdečního výdeje a aktivaci sympatického nervového systému a vazokontrikčních působků, tím postupně dochází ke snížení prokrvení ledvin, vazokonstrikci renálních cév, poklesu glomerulární filtrace s postupným rozvojem renálního selhání. Dalšími faktory jsou zvýšení venózního tlaku vedoucí k renální venózní kongesci, zvýšení hladin cirkulujících cytokinů a snížená odpověď ledvin na natriuretické peptidy. Jsou rozebrány patofyziologické mechanizmy, charakteristika nemocných, úloha některých léků a vztah proteinurie a anémie u KR syndromu. Heart failure with decreased cardiac output and activation of the sympathetic nervous system and vasocontrictory substances leads to hypoperfusion of the kidneys, decrease of glomerular filtration rate and eventually development of renal failure. Other factors include increased venous pressure leading to renal venous congestion, increased levels of circulating cytokines and decreased renal response to natriuretic peptides. In this review, we assess the pathophysiological mechanisms, patient characteristics, role of certain drugs, and the relationship of the anemia and proteinuria in cardiorenal syndrome. Úvod: Prognostický význam laboratorních parametrů asociovaných s postižením jater byl prokázán v mnoha studiích, nicméně jen málo prací sleduje prediktivní vliv jaterních enzymů u akutních koronárních syndromů (AKS). Cílem naší práce bylo zjistit prognostický vliv abnormalit funkčních jaterních testů u pacientů hospitalizovaných pro AKS s manifestním srdečním selháním. Metodika a výsledky: Z celkového souboru 8 818 pacientů v registru AHEAD (Acute Heart Failure Database) bylo vybráno 359 pacientů s diagnózou AKS a dostupnými hodnotami aspartátaminotransferázy (AST), alaninaminotransferázy (ALT), alkalické fosfatázy (ALP) a gamaglutamyltransferázy (GGT). Výskyt abnormálních hodnot zkoumaných laboratorních parametrů v našem souboru byl následující – AST u 71,3 %, ALT u 55,2 %, ALP u 11,7 % a GGT u 31,8 % pacientů. S krátkodobou (30denní) celkovou mortalitou byly významně asociovány abnormální hodnoty AST a ALT, zatímco dlouhodobou celkovou mortalitu signifikantně ovlivňovaly abnormality AST, ALP a GGT. Závěr: I přes vývoj moderních biomarkerů myokardiální ischemie zůstává role funkčních jaterních testů díky jejich snadné proveditelnosti a cenové dostupnosti v diagnostice infarktu myokardu nezastupitelná. Jak však naznačují výsledky naší studie, neméně důležitý je i jejich prediktivní význam., Introduction: The prognostic significance of the laboratory parameters associated with hepatic impairment has been proven in many studies. However, only a few of them focused on the predictive value of liver enzymes in acute coronary syndromes (ACS). The aim of our study was to evaluate the prognostic significance of abnormal liver function tests in patients hospitalised with ACS and concomitant acute heart failure. Methods and results: Of 8,818 patients included in the AHEAD (Acute Heart Failure Database) registry, 359 patients had ACS on admission and complete records on aspartate aminotransferase (AST), alanine aminotransferase (ALT), g‑glutamyltransferase (GGT), and alkaline phosphatase (ALP) serum levels. The incidence of abnormalities was as follows – AST in 71.3%, ALT in 55.2%, ALP in 11.7% and GGT in 31.8%. A significant association with short‑term (30-day) mortality was found in AST and ALT. A significant influence of AST, ALP and GGT was found in the case of long‑term mortality. Conclusion: Despite the development of modern myocardial ischaemia biomarkers, the role of liver function tests due to their high accessibility and affordability remains irreplaceable.
