Úvod: Delirantní stavy u pacientů vyššího věku jsou častou a závažnou psychickou poruchou. Jsou většinou krátkého trvání, avšak recidivující. Cíl: Analýza příčin delirantních stavů u pacientů. Soubor: Bylo sledováno 31 pacientů (18 žen a 13 mužů) ve věku od 65 do 92 let, před a po ortopedickém zákroku - totální endoprotéza kyčle čí kolene. 30 operací bylo elektivních, 1 akutní. Metodika: Pacienti byh hodnoceni pomocí CAM (Confusion Assessement Method) se zaměřením na problém deliria a MMSE (Mini Mental State Examination) se zaměřením na problém demence. Výsledky: Delirium bylo prokázáno u 4 pacientů, z toho byl u 3 pacientů potvrzen abúzus alkoholu, ve 4. případě šlo o akutní zvládáni stavu pacienta. Závěr: Problematika delirií bývá často opomíjena a je nutná pečlivá anamnéza a zvážení všech rizikových faktorů, zejména etylizmu, které mohou vést k rozvoji delirantních stavů, a je vhodně zvážit premedíkaci neuroleptiky před zákrokem., Introduction: Delirious states in elderly patients are a wide-spread major psychic disorder. They are mostly short-lasted, but recurrent. Goal: Analysis of causes of delirious states in patients. Group: 31 patients (18 women and 13 men) aged 65 to 92, before and after orthopedic intervention - hip or knee total endoprosthesis 30 surgeries were elective; 1 was acute. Results: Delirium was confirmed in 4 patients, in 3 of whom alcohol abuse was proved; the fourth patient was an acute case. Conclusion: Deliria are an often neglected condition. Careful diagnostics is to be applied and all risk factors, especially ethylism, which might cause delirious States are to be taken into account. Vývoj teorií stárnutí reflektuje posun v lidském poznání; teorie novější jsou postupným zdokonalováním teorií starších. V posledních několika letech až desetiletích se rovina odhalovaných souvislostí zanořuje stále hlouběji do subcelulárních a molekulárních struktur. Přes mnoho společných rysů zůstávají jevy pro některé tkáně, orgány a orgánové soustavy specifické. V článku je předložen přehled změn způsobených stárnutím v buňce samotné obecně, dále potom v myokardu, v buňce kosterního svalstva, v chrupavce a celém pohybovém systému. Pozornost je věnována relativně novému' pojmu - sarkopenii, která má dalekosáhlý dopad na celkový stav a hlavně zachování soběstačnosti seniora. Specifické rysy má proces stárnutí v neuronu a mozkové tkáni, opět s konsekvencemi zásadního významu. Zkušenost říká, že obranyschopnost organismu klesá s postupujícím věkem, ale poznatky odhalující podstatu tohoto de facto empiricky zjištěného jevu přicházejí jen pozvolna. Na stav imunitního systému úzce navazuje kondice respiračního systému, který se ve stáří stává jedním z nejzranitelnějších článků celého organismu. Změny trávicího systému ve stán mohou vést k pozvolnému rozvoji vitaminových karencí i proteinoenergetické malnutrice. Poněkud opomíjeným rizikem ve stáří je tendence k hyperkoagulaci, a tím k četným tromboembolickým komplikacím., The development of ageing theories reflects the advancement in human knowledge; newer theories are progressive improvement of older theories. In the last few years up to decades the level of discovered imphcations submerges constantly deeper to subcellular and molecular structures. Despite many common features there are still phenomena that are specific for certain tissues, organs and organ systems. The article presents a review of changes caused by ageing in the cell itself, in general terms, then in the myocardium, in the skeletal muscle cell, in the cartilage and in the whole locomotive system. The attention is given to the relatively novel term - sarcopenia - that has far-reaching impact to the general state of health and mainly to the maintenance of senior self-sufficiency. Ageing process in neurone and brain tissue has its own specific features, again with consequences of essential significance. The experience teaches that stamina of the body decreases with advancing age but knowledge reveahng nature of this phenomenon, in fact found empirically, comes only slowly. The condition of respiratory system that becomes one of the most vulnerable parts of the whole body in the old age is closely connected with the immune system condition. Changes of the digestive tract in the old age can lead to gradual development of vitamin deficiency diseases and protein-energetic malnutrition.
