Cíl: Cílem studie bylo vyhodnotit vztah mezi hodnotami indexu IgG a počtem oligoklonálních IgG pásů (OCB) v mozkomíšním moku (CSF) pacientů s roztroušenou sklerózou (SM). Materiál a metodika: Soubor 150 pacientů sestával ze 41 mužů a 109 žen (ve věku od 18 do 68 let, průměrný věk byl 36,6 ±10,1 let). U mozkomíšního moku odebraněho lumbální punkcí byla posouzena intratekální syntéza s využitím indexu IgG a stanovením OCB. Počet alkalických OCB v CSF byl stanoven metodou izoelektrické fokusace. K vyhodnocení statistické významnosti výsledků byla rovněž použita Pearsonova korelační analýza, x2 test homogenity, Mannův-Whitneyův test, párový t-test (parametrický) a jednovýběrový Wilcoxonův test (neparametrický). Výsledky: Pozitivní korelace mezi IgG indexem a počtem OCB nebyla nalezena. Ani Mannův-Whitneyův test neprokázal podstatný rozdíl v hodnotách indexu IgG u pacientů s OCB > 2 a < 2. Závěr Studie nepotvrdila korelaci mezi hodnotami IgG indexu a počtem OCB v CSF u pacientů s roztroušenou sklerózou., Objective: The aim of this study was to assess the correlation between IgG index values and the number of the oligoclonal IgG bands (OCB) in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS). Material and Methods: The set of 150 patients consisted of 41 males and 109 females (aged 18–68, mean 36.6 ± 10.1 years). The CSF collected by a lumbar puncture was examined evaluating intrathecal synthesis using the IgG index and determining OCB. The number of alkaline OCB in the CSF was assessed using the method of isoelectric focusation. Pearson's correlation analysis, and homogeneity χ2 test , Mann-Whitney test, paired-sample t-test (parametric) and Wilcoxon signed-ranks test (nonparametric) were used to evaluate the statistical significance of the results. Results: No positive correlation between the IgG index and the number of OCB was found. Mann-Whitney test also did not demonstrate any significant difference of the IgG index values in patients both with the OCB number ≥ 2 and < 2. Úvod: Adenokarcinom pankreatu (PDAC) je jedním z nejagresivnějších maligních onemocnění. Jeho špatná prognóza je dána kombinací různých faktorů, mezi něž patří agresivní biologie onemocnění, nespecifické či chybějící příznaky v časných stadiích, jejich nedocenění, protrahovaná diagnostika a zpoždění v zahájení léčby. Většina pacientů je diagnostikována již v pokročilém stadiu onemocnění. Medián přežití u těchto pacientů se pohybuje mezi 2 a 11 měsíci. Nejčastějšími důsledky lokálně pokročilého onemocnění, které si vynutí intervenci, jsou obstrukce žlučových cest a duodena. Cílem práce byla analýza přežívání pacientů s radikálně inoperabilním PDAC, kteří podstoupili paliativní chirurgický výkon či exploraci s biopsií, stanovení vlivu faktorů pacienta, nádoru a léčebné modality na délku přežití. Metody: Jedná se o retrospektivní analýzu souboru pacientů s inoperabilním PDAC, kteří podstoupili neradikální chirurgickou intervenci v letech 2006–2014. Předmětem analýzy byly věk, histopatologické nálezy, chirurgická a onkologická léčba a doba přežití. Výsledky byly statisticky zpracovány a vyhodnoceny pomocí softwaru IBM SPSS Statistics verze 22 (USA). Výsledky: Soubor tvořilo celkem 184 pacientů (105 mužů, 79 žen) s radikálně inoperabilním PDAC. Průměrný věk byl 64 let a převažovali pacienti s onemocněním ve stadiu IV. Průměrná délka přežití byla 7,04 měsíce, medián 4,7 měsíce. Závěr: Byl zjištěn statisticky signifikantní vliv pohlaví, stadia onemocnění, přítomnosti vzdálených metastáz v době operace a podání onkologické léčby na přežívání těchto pacientů. Průměrná délka i medián přežití pacientů s radikálně inoperabilním tumorem v našem souboru koresponduje s výsledky podobných studií., Introduction: Pancreatic cancer (PDAC) is one of the most aggressive malignancies. Its poor prognosis is due to a combination of various factors, mainly aggressive biology of the tumour, non-specific symptoms in early stages, their underestimation, prolonged time to diagnosis and late onset of treatment. The majority of patients are diagnosed in an advanced stage of the disease. Median survival of these patients ranges from 2−11 months. The most common consequences of locally advanced disease that require intervention include obstruction of the duodenum and biliary obstruction. The purpose of our study was to analyze the survival of patients with radically inoperable PDAC undergoing palliative surgery or exploration with biopsy, and to evaluate the influence of patient and tumour factors and treatment modalities on survival. Methods: In our retrospective study we included all patients with radically inoperable PDAC undergoing a non-radical surgical intervention between 01 January 2006 and 31 December 2014. Patient age, histopathological findings, surgical and oncological treatment and survival were included in the analysis. The results were statistically processed and evaluated using IBM SPSS Statistics software version 22 (USA). Results: 184 patients with radically inoperable PDAC, 105 males and 79 females, were included in our study. Mean age of the patients was 64 years and most patients presented with stage IV of the disease. Mean survival time was 7.04 months and median 4.7 months. Conclusion: We determined a statistically significant influence of the following factors on patient survival: sex, stage, presence of distant metastases at the time of surgery and oncological treatment administration. Cíl studie: Pro monoklonální gamapatie (MG) je typická přítomnost molekul monoklonálního imunoglobulinu (MIG) nebo jejich fragmentů v séru a/nebo v moči. Nejnovějším testem rozšiřujícím spektrum možností vyšetření MIG je systém HevyLiteTM, principiálně založený na užití dvojice specifických protilátek proti junkčním epitopům mezi doménami těžkého a lehkého řetězce imunoglobulinu. Cílem práce bylo zavedení systému HevyLiteTM a provedení pilotní analýzy ve skupině nemocných s MG typu IgA. Název a sídlo pracoviště: III. interní klinika, Fakultní nemocnice Olomouc. Materiál a metody: Vyšetřený soubor tvořilo 24 nemocných s mnohočetným myelomem (MM) a 7 jedinců s monoklonální gamapatií nejistého významu (MGUS). Pro stanovení sérových hladin IgAκ, IgAλ byl použit systém HevyLiteTM (The Binding Site, UK) a analyzátor BN II (Siemens Healthcare Diagnostics). Výpočtem byl určen poměr IgAκ/IgAλ. Výsledky: U nemocných s aktivní formou MM byly zjištěny vysoce patologické hladiny dominantního MIG, se supresí hladin alternativního imunoglobulinu IgA a s výrazným ovlivněním poměru IgAκ/IgAλ. Ze 6 nemocných s dosažením kompletní remise byl u 5 zaznamenán normální poměr IgAκ/IgAλ, jeden nemocný vykazoval supresi obou IgA typů se změnou poměru IgAκ/IgAλ. U všech jedinců s MGUS byly zjištěny abnormální hladiny i poměr IgAκ/IgAλ, u jedince s low-risk typem byla hodnota poměru IgAκ/IgAλ pouze nadhraniční. Při srovnání výsledků stanovení M-proteinu metodou HevyLiteTM a elektroforézou, Spearmanova korelační analýza potvrdila velmi těsnou korelaci hladin M-proteinu jak v případě IgAκ (r = 0,946, p < 0,0001), tak IgAλ (r = 0,872, p = 0,0001). Součty hladin dominantního i alternativního IgA imunoglobulinu významně korelovaly s celkovou hladinou imunoglobulinu IgA stanoveného nefelometricky (r = 0,994, p < 0,0001). Závěr: Je patrné, že systém HevyLiteTM nadějně doplňuje soubor vyšetření, používaných standardně při sledování MG, zejména pak v případech s velmi nízkou koncentrací MIG či jen pouhé pozitivity imunofixační elektroforézy. Pro definitivní zhodnocení přínosu vyšetření pro klinickou praxi je nutné získání dalších zkušeností., Objective: Typical features of monoclonal gammopathy (MG) include the presence of monoclonal immunoglobulin (MIG) molecules or their fragments in the serum and/or urine. The latest test which extends the range of MIG examination possibilities is the HevyLiteTM system, using principally a couple of specific antibodies against junctional epitopes between the domains of heavy and light immunoglobulin chains in constant regions. The study aimed at implementing the HevyLiteTM system and performing a pilot analysis in the group of IgA-type MG patients. Settings: Department of Internal Medicine III, University Hospital Olomouc. Material and methods: The studied group consisted of 24 patients with multiple myeloma (MM) and 7 individuals with monoclonal gammopathy of undetermined significance (MGUS). The HevyLiteTM system (The Binding Site, UK) was used to determine the serum levels of IgAκ, IgAλ as well as the BN II analyser (Siemens Healthcare Diagnostics). The ratio of IgAκ/IgAλ was determined by calculation. Results: The patients in the active disease stage revealed highly pathological levels of the dominant MIG, with suppressed levels of the alternative IgA immunoglobulin, and significant influence on the IgAκ/IgAλ ratio. 5 out of the 6 patients who had achieved a complete remission showed a normal IgAκ/IgAλ ratio, 1 patient had suppression of both IgA with a modified IgAκ/IgAλ ratio. All individuals with MGUS revealed abnormal levels as well as ratio of IgAκ/IgAλ, one patient with a low-risk disease type had the IgAκ/IgAλ ratio value slightly above the threshold. When comparing the results of M-protein determination by the method of HevyLiteTM and electrophoresis, Spearman‘s correlation analysis confirmed a very close correlation of M-protein levels regarding IgAκ (r = 0.946, p < 0.0001) as well as IgAλ (r = 0.872, p = 0.0001). The summations of the dominant as well as alternative IgA immunoglobulin levels substantially correlated with the general level of IgA immunoglobulin determined nephelometrically (r = 0.994, p < 0.0001). Conclusion: It seems, that the HevyLiteTM system promisingly complements the set of examinations used routinely by MG monitoring, in particular by very low concentrations of MIG, or the mere positivity of the immunofixation electrophoresis.
