Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Introduction: It has been suggested that the prevalence of neurodegenerative diseases in small, isolated European communities might be higher than in the general population. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. Úvod: Vysoká incidence kolorektálního karcinomu představuje v České republice aktuální a populačně závažný zdravotnický problém. Polovina všech nemocných je věku nad 70 let. Možnosti chirurgické i nechirurgické léčby v této skupině závisejí na faktorech, jen minimálně vyjádřitelných zavedenou onkologickou a anesteziologickou klasifikací (cTcNcM, ASA). Cílem práce je ukázat vliv věku na využití léčebných modalit do protokolu léčby a na její výsledky a na jiné možnosti hodnocení únosnosti k léčbě. Metody: Analýza údajů z databáze NOR z pětiletého období 2008–2012 zpracované Institutem biostatistiky a analýz MU. Výsledky: Ve všech sledovaných parametrech byl prokázán rozdíl mezi nemocnými pod a nad 70 let v neprospěch starších. Ve věku pod 70 let nebylo léčeno 11,2 % nemocných, ve věku na 70 let 25,2 %. Závěr: K rozhodování o způsobu vedení léčby včetně volby operace by mohla přispět klasifikace pomocí komplexního geriatrického vyšetření., Introduction: High incidence of colorectal cancer in the Czech Republic is an actual and demographically significant health issue. Half of all of the patients is older than 70 years. Both surgical and non-surgical treatment options in this group of patients depend on factors that are difficult to measure only by current oncological and anesthesiological classifications (cTcNcM, ASA). The objective of this paper is to measure the impact of age on the use of various treatment modalities within the protocol and their results, and also to suggest alternative options for therapy tolerance assessment. Methods: Analysis of data over a five-year period from the NOR database prepared by the Institute of Biostatistics and Analyses, Masaryk University. Results: In all parameters a difference was demonstrated between patients below the age of 70 and those above the age of 70 years. Older patients were disadvantaged. Only 11.2% of patients younger than 70 years were not treated, whereas 25.2% over the age of 70 years were not treated. Cíl: Popsat profil nemocných s akutním srdečním selháním (ASS) z pohledu přednemocniční péče v České republice. Metodika: Data pochází z registru Zdravotnické záchranné služby (ZZS) Jihomoravského kraje z období leden 2010–prosinec 2011. Byl proveden konsekutivní sběr nemocných s podezřením na ASS. Vyloučeni byli nemocní po kardiopulmonální resuscitaci nebo zemřelí před příjezdem ZZS nebo pacienti s dominantně extrakardiální příčinou dušnosti. Srovnáni jsou nemocní transportovaní do nemocnice oproti pacientům léčeným ambulantně. Cílovým parametrem je 30denní celková mortalita. Výsledky: Z celkového počtu 131 798 nemocných bylo podezření na ASS vysloveno ve 1 964 (1,5 %) případech. Konečná diagnóza ASS byla nakonec stanovena u 1 310 pacientů, kteří byli transportováni do nemocnice s předpokladem hospitalizace. Ambulantně bylo ošetřeno 35 nemocných. Průměrný věk celé populace byl 78 let, nejčastější komorbiditou byla hypertenze (67,0 %), u téměř poloviny jsou do doby předání do nemocnice přítomny známky plicního edému (46,4 %). Ambulantně léčení nemocní jsou častěji muži (71,4 % vs 49,1 %), mají menší výskyt komorbidit a nevykazují známky těžší respirační insuficience (kyslíková saturace 96 % oproti 87 %, žádné známky plicního edému). Nejčastěji aplikovaným lékem v přednemocniční péči u pacientů s podezřením na ASS je furosemid (74,0 %). Jednoměsíční mortalita souboru dosahovala 20,7 %. Závěr: ASS představuje 1,5 % všech volání ZZS pro akutní potíže, většina nemocných je transportována do nemocnice s předpokladem hospitalizace. Časná mortalita nemocných je vysoká. Klíčové slova: akutní – srdeční selhání – úmrtnost – zdravotnická záchranná služba, Purpose: To describe the profile and management of patients with acute heart failure (AHF) in pre‑hospital care in the Czech Republic. Methods: Registry data of the Emergency Medical System (EMS) of the South Moravian region of Czech Republic were used. Patients with suspected AHF were enrolled consecutively during the period of January 2010–December 2011. Excluded were patients who died or had cardiac arrest before the arrival of EMS and those who presented with dyspnea of dominantly extra-cardiac etiology. We compared patients who were transported to the hospital with those treated in an outpatient setting. Overall mortality within the first 30 days after the initial EMS contact was assessed as a main endpoint. Results: From the 131,798 of patients who called EMS, 1,964 (1.5%) of cases were suspected of having AHF. The diagnosis of AHF was determined in 1,310 patients who were transported to the hospital; 35 were treated in Outpatients. The mean age of the study population was 78 years, the most common comorbidity was hypertension (67.0%) and almost one half of the study population had signs of pulmonary edema before being admitted to hospital (46.4%). Patients not transported to hospital were more frequently of male gender (71.4% vs. 49.1%), with lower incidence of comorbid conditions and did not have signs of respiratory distress (oxygen saturation 96% vs. 87% in those transported to the hospital; no signs of pulmonary edema). The most common drug used in pre‑hospital care in patients with suspected AHF was furosemide (74.0% patients). The 30-day mortality rate of the whole study population reached 20.7%. Conclusion: AHF represents 1.5% of all emergency calls due to acute symptoms. The vast majority of the patients are transported to the hospital with the assumption of hospitalization. Early mortality rates of these patients are high. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Úvod: Observační, prospektivní a neintervenční projekt SOLOSTAR měl za cíl získat informace z klinické praxe v České republice o spokojenosti pacientů s používáním předplněného inzulinového pera SoloStar. Metodika: 1 805 pacientů s diabetem 1. nebo 2. typu bylo po zahájení používání pera SoloStar sledováno 3–4 měsíce. Spokojenost pacientů s perem SoloStar a s jeho jednotlivými parametry byla posuzována ve skupině všech pacientů a v podskupinách rozdělených dle demografických parametrů včetně zdravotního omezení. Pacienti také porovnávali pero SoloStar s pery používanými před vstupem do projektu. Výsledky: 98,3 % pacientů ohodnotilo celkovou spokojenost s perem SoloStar jako „výbornou“ a „dobrou“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Na celkovou spokojenost s perem SoloStar nemělo statisticky významný vliv žádné z demografických kritérií, a to ani zrakový handicap či omezení manuální zručnosti. Parametry pera SoloStar byly u více než 90 % pacientů hodnoceny jako „výborné“ a „dobré“. Lépe byly některé parametry hodnoceny zejména pacienty bez předchozí zkušenosti s aplikací inzulinu. Pacienti, kteří používali jiný typ aplikátoru před zařazením do projektu, byli více spokojeni s perem SoloStar než s předchozím aplikátorem a většina z nich ohodnotila používání pera SoloStar (93,1 %) a aplikaci inzulinu perem SoloStar (83,5 %) v porovnání s původním perem jako „mnohem jednodušší“ a „jednodušší“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Závěr: Používání pera SoloStar je v běžné klinické praxi v České republice spojeno s vysokou spokojeností pacientů, a to včetně pacientů se zdravotním omezením. Pacienti navíc pero SoloStar upřednostňovali před inzulinovými pery používanými před vstupem do projektu. Methodology: 1 805 patients suffering from type 1 or 2 diabetes who began using the pen SoloStar were observed for 3 to 4 months. Satisfaction of patients with SoloStar and with its particular features was evaluated in the group of all patients and in subgroups defined by demographic parameters including the type of handicap. Patients also compared SoloStar with the pens used before the entry into the survey. Results: 98.3 % of patients rated overall satisfaction with the pen SoloStar as “excellent” and “good” (the highest and second highest rating on a 5-point scale). Demographic criteria, including visual handicap and manual dexterity handicap, did not have any statistically significant effect on the rating of the overall satisfaction with SoloStar. Features of SoloStar were rated as “excellent” and “good” by more than 90% of patients. Higher rating of some of the features of SoloStar was awarded mostly by patients without prior experience with insulin application. Patients who used other insulin applicators before entering into the survey were more satisfied with the pen SoloStar than with the previously used applicator and most of them rated the use of SoloStar (93.1 %) and insulin application with SoloStar (83.5 %) as “much easier” and “easier” (the highest and second highest rating on a 5-point scale) in comparison with the previously used pen. Conclusion: The use of the pen SoloStar in a clinical practice setting in the Czech Republic is associated with high levels of satisfaction of patients, including handicapped patients. In addition, patients preferred SoloStar over insulin pens used before the entry into the survey. Cíl: Cílem našeho článku je předložit normativní data pro fonemickou verbální fluenci (VF), pro hlásky K, P, S, i sémantickou VF, pro kategorie zvířata a zelenina. Úvod: Testy verbální fluence jsou jedna z nejrozšířenějších metod hodnocení kognitivní výkonnosti. V českém prostředí dosud neexistuje kvalitní normativní studie testů VF se zaměřením na populaci starších osob, u kterých se tato metoda používá nejčastěji. Metodika: Testy VF jsme administrovali v rámci neuropsychologické baterie 540 zdravým osobám (292 žen, 248 mužů) nad 59 let. V části fonemické VF jsme použili hlásky K, P, S a celkový skór všech hlásek (K + P + S) jako analogii anglického originálu F, A, S v Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). V rámci sémantické VF jsme hodnotili výkonnost ve dvou kategoriích, zvířata a zelenina. Výsledky: Věk má statisticky významný (p < 0,001) slabý až středně silný vliv na výkon ve VF, reprezentovaný součtem skórů K + P + S (r = –<span class=""></span>0,236), zvířata (r = –<span class=""></span>0,359) a zelenina (r = –<span class=""></span>0,264). Rovněž počet let vzdělání ovlivňuje slabě až středně silně výkon ve VF. Statisticky významně (p < 0,001) pouze ve skórech K + P + S (r = 0,297) a zvířata (r = 0,357). Mezi věkem a skórem zelenina vztah není signifikantní (r = 0,028; p = 0,523). Ve skóru zelenina existuje signifikantní (p < 0,001) rozdíl mezi muži a ženami. Dále předkládáme normativní data pro věkové skupiny 60–<span class=""></span>75 let, 70–<span class=""></span>85 let a 80–<span class=""></span>96 let. Závěr: Výsledky naší studie prokazují statisticky významný vliv věku na všechny uvedené zkoušky VF. Fonemická VF a kategorie zvířata jsou závislá i na výši vzdělání, v kategorii zelenina není výkonnost závislá na vzdělání, ale na pohlaví., Aim: The aim of the study was to report normative data on letter (LF; letters K, P, S) and semantic fluency (SF; animals and vegetables). Introduction: Verbal fluency (VF) is one of the most frequently used neuropsychological methods for the assessment of cognitive performance in clinical and experimental neuropsychology. However, representative normative data for the Czech population of older and very old adults are so far lacking. Methods: We administered VF as part of neuropsychological battery to 540 (292 women, 248 men) healthy older adults (60–<span class=""></span>96 years of age). In LF, the letters K, P, S and their total score were used as analogous to the original Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). In SF, we evaluated performance in two categories –<span class=""></span> animals and vegetables. Results: Age was significantly (p < 0.001) related to a sum of K + P + S (r = –<span class=""></span>0.236) as well as animals (r = –<span class=""></span>0.359) and vegetables (r = –<span class=""></span>0.264). However, the association was moderate. Education was also moderately related (p < 0.001) to the sum of K + P + S (r = 0.297) and animals (r = 0.357). However, we did not find a significant relationship between age and vegetables (r = 0.028; p = 0.523). Vegetables were also the only measure that showed highly significant sex differences (p < 0.001). We present normative Czech data for 60–<span class=""></span>75, 70–<span class=""></span>85 and 80–<span class=""></span>96 age groups. Conclusion: The results of our study confirm a significant moderate influence of age and education (with the exception of vegetables for the latter) on all VF measures. There were highly significant sex differences in the vegetable category. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L.
