Legionářská nemoc je akutní bakteriální plicní onemocnění se závažným průběhem a vysokou mortalitou. Z epidemiologického hlediska má původ v komunitním, cestovním, nemocničním nebo pracovním prostředí. Nejvýznamnější rezervoáry legionel pro expozici osob představují chladicí věže a odpařovací kondenzátory, dále centrálně řešené vytápění a chlazení moderních budov. Autoři prezentují výsledky mikrobiologických vyšetření vody na přítomnost legionel v technologiích průmyslu plastikářského, sklářského, z chladicích věží. Ve vazbě na tyto výsledky vyhodnocují možné zdravotní dopady na exponované pracovníky. Ve všech vyšetřovaných provozech byly legionely nalezeny, a to kmeny patogenní i saprofytické. Největší potenciál možného zdravotního rizika má Legionella pneumophila séroskupina 1, podskupina A – Pontiac. Riziko expozice pracovníků je relativně nízké při běžném provozu, významně narůstá při operacích údržby a oprav. Se zjištěnou kolonizací prostředí legionelami korespondují v České republice sporadické případy hlášených profesionálních onemocnění legionelózou. Posuzování profesionality tohoto onemocnění musí vycházet z kritérií epidemiologické surveillance s využitím moderních diagnostických a identifikačních metod., Legionnaire's disease is an acute bacterial disease affecting the lungs, with severe course and high mortality rates. In epidemiological terms, the disease originates in communal, travel, hospital, or occupational environments. The most important potential reservoirs of Legionella causing human exposure are cooling towers and evaporative condensers, as well as central heating and cooling systems in modern buildings. The authors present the results of microbiological tests of water for the presence of Legionella in industrial technologies (plastic injection molding, glass and cooling towers) and their potential health effects on exposed workers. Pathogenic and saprophytic Legionella strains were found in all investigated plants. The highest potential health risks are connected to Legionella pneumophila serogroup 1, subgroup A – Pontiac. During normal operation the risk of occupational exposure is relatively low, but it significantly increases during repairs and maintenance operations. The environmental Legionella colonization found in the Czech Republic corresponds to sporadic cases of reported occupational cases of legionellosis. V databázi Globocan 2008 (IARC) bylo za rok 2008 z celosvětového počtu 12,663 milionů nových nádorů odhadováno 2,445 milionů v EU, ze 7,565 milionů zemřelých téměř 1,235 milionů v EU a z pětileté prevalence 28,8 milionů asi 6,617 milionů v EU. Početnější prevalence se týkala ve světě 18 % nádorů prsů, 11 % kolorekta, 11 % prostaty, 6 % plic, 5,5 % žaludku, 5,5 % cervixu, 4 % měchýře a 4 % dělohy, v EU 20 % ZN prsů, 14 % kolorekta, 18 % prostaty, 5 % plic, 4 % melanomu, 3 % ledvin, 3 % dělohy a 3 % NH lymfomu. V roce 2030 mohou počty nových nádorů dosáhnout ve světě 21 milionů případů, v EU 3 miliony, v ČR 93 tisíc (dg. C00-D09). Z předběžných celkových výdajů 288 572 mil. Kč na zdravotnictví za rok 2011 uhradily zdravotní pojišťovny 217 653 mil., z toho 19 217 mil. na onkologii. V letech 1990–2011 vzrostly celkové náklady na zdravotnictví o 257 534 mil. Kč s podílem 7,58 % na hrubém domácím produktu. Průměrné výdaje VZP na léčbu jednoho nádoru v roce 2010 dosáhly 19 030 Kč s nejvyššími náklady u novotvarů mízní a krvetvorné tkáně, mozku a CNS, trávicího ústrojí, úst a hltanu, dýchacích cest. Výkonnost globální ekonomiky nedosahuje potřebných prostředků na zdravotnictví. Český veřejný dluh překračuje 1 680 miliard Kč v době dokončení tohoto článku. I když celkové výdaje obyvatel na zdravotní péči rostou, zdá se, že jeho pokračující zadlužování zvýší také dluhy nemocnic a zdravotních pojišťoven. Here preliminary total health care expenditures in 2011 were 288,572 million CZK; of these, health insurance companies covered 217,653 million, with 19,217 million per one cancer treatment. Between 1990 and 2011, the total health expenditures increased by 257,534 million CZK, accounting for 7.58 % of the gross domestic product. In 2010 the General Health Insurance Company paid on average 19,030 CZK for the treatment of one cancer case, with the highest expenditures for neoplasms of the lymphoid and haemotopoietic tissues, brain and central nervous system, digestive organs, oral cavity and pharynx, and respiratory organs. The global economic performance cannot provide enough resources for public health care. The Czech public debt exceeded 1,680 billion CZK at the time this article was written. Although out-of-pocket money paid by cancer patients for treatment is growing, the continuing public indebtedness is likely to increase debts of hospitals and health insurance companies. Z 99 378 nových karcinomů kolorekta (KRK) u mužů a 78 977 u žen, evidovaných v registru nádorů ČR v letech 1976–2005, bylo 14 860 (15 %) KRK u mužů a 11 142 (14,1%) u žen spojeno s výskytem jiných zhoubných novotvarů (ZN). Po 6 504 primárních KRK u mužů následovalo 7 406 ZN, po 4 342 KRK u žen 4 798 následných ZN. Naopak 8 356 následným KRK u mužů předcházelo 10 152 primárních ZN, 6 800 KRK u žen předcházelo 7 829 ZN. Počet vícečetných KRK se zvýšil ze 141 v roce 1976 na 1 534 v roce 2005. V letech 1996–2005 dosáhly primární KRK 57 % časných, 30 % pokročilých a 13 % neznámých stadií, následné KRK 43,5 %, 34 % a 22,5 %. Vícečetné KRK byly u mužů nejpočetnější ve věku 50–69 let, u žen 70–79 let. Průměrná doba vzniku následného novotvaru po primárním KRK u mužů byla 5,1 roku, u žen 5,4 roku. Po primárních KRK byly nejpočetnější ZN GIT, kůže, pohlavních orgánů a močového systému. Ze synchronních ZN byla téměř polovina nádorů trávicích cest. Z 12 204 následných ZN bylo 19 % zjištěno v pokročilých stadiích z toho asi polovina ZN trávicích cest. Víc než třetinu primárních nádorů před 17 981 následnými KRK tvořily karcinomy kůže. U 1 715 (26,4 %) mužů a 1 206 (27,8 %) žen, přežívajících v roce 2007 s primárním KRK, byl vyšší podíl časných stadií než s následným KRK; naopak u 6 646 (79,5 %) mužů a 5 291 (77,8 %) žen, zemřelých v roce 2007 s následným KRK, byl vyšší podíl pokročilých stadií než s primárním KRK. Z uvedených dat se odhaduje, že z 8 265 KRK hlášených v roce 2010 bylo téměř 1 700 (21 %) vícečetných KRK, z toho 260 primárních a 1440 následných případů. Každé páté onemocnění KRK bylo spojeno s výskytem dalšího ZN. I když je obtížné odhadnout počet vícečetných případů mezi 53 340 přežívajícími s KRK v roce 2010, jejich pokračující trend zvyšuje zátěž diagnostických a léčebných kapacit., Among the total of 99,378 new colorectal cancers (CRCs) in males and 78,977 in females, based on the Czech Cancer Registry in 1976–2005, there were 14,860 (15 %) CRCs in males and 11,142 (14.1 %) in females associated with the other malignant neoplasms (MNs). The primary 6,504 CRCs in males were followed by 7,406 subsequent MNs, the primary 4,342 CRCs in females by 4,798 subsequent MNs. On the contrary before the subsequent 8,356 CRCs in males were registered 10,152 primary MNs, 6,800 CRCs in females 7,829 MNs. The number of multiple CRCs increased from 141 in 1976 to 1,534 in 2005. In 1996–2005 the primary CRCs reached 57 % of early, 30 % advanced and 13 % unknown clinical stages, respectively the subsequent CRCs of 43.5 %, 34 % and 22.5 %. The occurrence of all multiple CRCs were highest in the age group of 50–69 years in males, and of 70–79 years in females. The average interval between primary CRCs and subsequent MNs was 5.1 years in males and 5.4 years in females. The most frequent MNs following primary CRCs were cancers of the gastrointestinal tract, skin, genital organs and urinary tract. Nearly half of synchronous MNs after primary CRCs were cancers of the gastrointestinal tract. There were 19 % cases diagnosed at advanced stages from the total 12,204 subsequent MNs, one half of those in patients with gastrointestinal cancers. More than one third of primary MNs preceded by 17,981 subsequent CRCs were cancers of the skin. Among 1,715 (26.4 %) males and 1,206 (27.8 %) females survived up to 2007 with primary CRCs, there were higher early stages than with subsequent CRSc; on the contrary among 6,646 (79.5 %) males and 5,291 (77.8 %) females, died up to 2007 with subsequent CRCs, were higher advanced stages than with primary CRCs. By the above mentioned data is estimated that among a total of 8,265 CRCs registered in 2010 were nearly 1,700 (21 %) multiple CRCs, i.e. 260 primary and 1,440 subsequent cases. Every fifth CRC was associated with the occurrence of the other MNs. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Kolorektální karcinom je druhým nejčastějším maligním onemocněním ve vyspělých zemích. Vzhledem k tomu, že proces kancerogeneze sporadického karcinomu probíhá relativně pomalu, je možné tomuto onemocnění úspěšně předcházet. Národní screening kolorektálního karcinomu byl v České republice zahájen v roce 2000 a od této doby prochází kontinuálním vývojem s cílem zvýšení efektivního pokrytí cílové populace. V současnosti je k dispozici dvouetapový screeningový program (test na okultní krvácení do stolice a v případě pozitivity kolonoskopie), nebo jednoetapový program (primární screeningová kolonoskopie). Kvalita programu je hodnocena pomocí indikátorů kvality – zejména záchytu adenomů (adenoma detection rate, ADR) a podílu totálních kolonoskopií (cecal intubation rate, CIR). Díky screeningovému programu dochází v ČR k dlouhodobé stabilizaci incidence a mírnému poklesu mortality na kolorektální karcinom. K dalšímu snižování incidence a mortality onemocnění je nutná vyšší účast cílové populace za pomocí adresného zvaní cílové populace a mediální kampaně., Colorectal cancer is the second most common malignant disease in developed countries. Given that the process of sporadic cancer carcinogenesis is relatively slow, it is possible to successfully prevent this disease. Czech National Colorectal Cancer Screening Program was launched in 2000 and since that time is in continual development in order to increase the effective coverage of the target population. It is currently available as two-step screening program (stool test for occult blood and colonoscopy) or one-step program (primary screening colonoscopy). Quality of the program is evaluated using performance indicators – namely adenoma detection rate (ADR) and cecal intubation rate (CIR). The screening program in the Czech Republic enables long-term stabilization of incidence and a slight decrease in mortality from colorectal cancer. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L.
