Typ studie: retrospektivní. Cíl: Cílem studie bylo zjistit úspěšnost porodů u žen s císařským řezem v anamnéze na našem pracovišti. Rovněž bylo cílem určit rizikové faktory pro úspěšnost porodu po předchozím císařském řezu a míru rizika, kterou tyto pacientky podstupují. Metodika: Výzkumný soubor tvořilo 1 060 porodů, které na našem pracovišti proběhly ve sledovaném období. Z toho 103 žen mělo císařský řez v anamnéze a 70 z nich se pokusilo porodit vaginálně. Výsledky: Z 1 060 porodů bylo 19,2 % sekcí, ze 70 TOLAC (trial of labour after caesarean – pokus o spontánní porod po císařském řezu) bylo 54,3 % úspěšných (porodily vaginálně), ze 13 indukcí po sekci bylo úspěšných 23,1 %. Jako prediktivní faktor pro úspěšný TOLAC byl prokázán vaginální porod v anamnéze (p = 0,00), BMI < 35 (p = 0,04) a trend naznačil věk rodičky < 25 let (p = 0,05). Byl prokázán signifikantní rozdíl v krevní ztrátě (měřené odhadem) mezi pokusem o vaginální porod po sekci a bez sekce v anamnéze (p = 0,000), pokusem o vaginální porod po sekci a primární sekcí po sekci (p = 0,000) a mezi úspěšným a neúspěšným pokusem o vaginální porod po sekci (p = 0,000). Byl rovněž prokázán signifikantní rozdíl Apgar(ové) skóre v 1. a 5. minutě u dětí z úspěšných a neúspěšných pokusů o vaginální porod po sekci. Tyto výsledky byly dány do kontextu s doporučenými postupy z USA, Kanady, Spojeného království, Německa, Rakouska a Austrálie a Nového Zélandu. Závěr: Studie poskytla informace, se kterými je možné seznámit pacientku plánující pokus o vaginální porod po sekci na našem pracovišti. We also wanted to determine the predictive factors for successful vaginal birth and the amount of risk which the vaginal birth after caesarean brings to the patient. Metodics: There were 1,060 births in our study, that took place in our clinic at the specified time. Out of this set there were 103 women, who had caesarean delivery in medical history and 70 of those tried to deliver vaginally. Results: Out of 1,060 births there were 19.2 % caesarean sections, from 70 patients who tried to give birth vaginally after caesaren (TOLAC – trial of labour after caesarean) 54.3 % succeeded (the rest gave birth per sectionem caesaream), out of 13 induced labours after caesarean there were only 23.1 % successful. We determined following predictive factors for successful TOLAC: at least one vaginal birth in medical history (p = 0.00), BMI under 35 (p = 0.04) at the time of birth and age under 25 (p = 0.05). We found significant difference in the blood loss between following groups: TOLAC vs deliveries without caesarean in medical history (p = 0.000), TOLAC vs. elective repeat sections (p = 0.000) and successful vs. unsuccessful TOLAC (p = 0.000). We also found significant difference in Apgar score in first and fifth minute between the group of successful TOLAC and unsucessful TOLAC. These results were compared with the current guidelines in UK, USA, Canada, Australia and New Zealand, Germany and Austria. Conclusion: This study has brought us data, that should be provided to the patients who think about giving birth in our clinic and who have already caesarean section in medical history. Hormonálně dependentní karcinom prsu je heterogenní onemocnění z pohledu molekulárního i klinického. Riziko relapsu časného karcinomu prsu léčeného adjuvantní hormonální terapií kolísá. Prediktivní markery pro volbu chemoterapie (CHT) u obecně příznivého nádoru s pozitivitou estrogenových receptorů (ER) a bez exprese receptoru pro epidermální růstový faktor HER2 (ER+/HER2-) jsou nedostatečné a vedou k indikaci CHT tam, kde nemůže mít efekt. To dokládá i molekulární klasifikace, která ukazuje, že hormonálně dependentní karcinom prsu není uniformní entitou, ale skupina nádorů s odlišnými vlastnostmi, které vyžadují individuální přístup v indikaci adjuvantní terapie. Oncotype DX je multigenová esej (MGA) s prognostickým i prediktivním významem. Na základě studií s více než pěti tisíci pacientkami s ER+/HER2- časným karcinomem prsu s průkaznou úrovní evidence, dosáhlo jeho použití klinické praxe. Test je v ČR (onkologických centrech) dostupný od ledna 2014 pro indikovanou skupinu nemocných. První zkušenosti s tímto vyšetřením jsou velmi dobré. Na onkologické klinice VFN a 1.LF UK v Praze bylo dosud vyšetřeno přes 20 pacientek., Hormonal dependent breast cancer is a heterogeneous disease from a molecular and clinical perspective. The relapse risk of early breast cancer patients treated with adjuvant hormonal therapy varies. Validated predictive markers concerning adjuvant cytotoxic treatment are still lacking in ER+/HER2- breast cancer, which has a good prognosis in general. This can lead to the inefficient chemotherapy indication. Molecular classification of breast cancer reports evidence about the heterogeneity of hormonal dependent breast cancer and its stratification to different groups with different characteristics. Oncotype DX is a multigene assay with a prognostic and predictive impact. On behalf of evidence with more than 5000 patients the test obtain clinical practice. For indicated group of patients is the test available in oncology centres from January 2014 in the Czech Republic. First experiences with this examination are very good. Program pro screening nádorů prsu České republiky byl zahájen v září 2002, kdy byla mimo jiné určena síť akreditovaných screeningových center (v současnosti 68). Screening karcinomu prsu prošel úspěšnou transformací od oportunní prevence k organizovanému programu. Incidence karcinomu prsu nepřetržitě roste, nicméně při rostoucím podílu časných stadií. Úmrtnost podstatně klesá, důvodem je screening a lepší léčba. Pokrytí ve věkové skupině 45-69 let dosáhlo v roce 2013 56,6 % a účast postupně roste - tento rok očekáváme více než 60 % z důvodu adresného zvaní. Okolo 73 % zjištěných karcinomů je relativně malých s dobrou prognózou (CIS nebo T1)., The Czech Breast Cancer Screening Programme was launched in September 2002 by establishing a network of accredited screening centres (currently 69 centres). Breast cancer screening underwent succesful transformation from opportunic to an organised programme. Breast cancer incidence is steadily increasing, however with a growing proportion of early stages. The mortality rate is significantly decreasing due to screening and better treatment. The coverage in the age group 45-69 reached 56,6% in 2013 and the attendance is slowly rising - we expect this year more then 60%. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. In this period the number of primary cases increased twice, of subsequent ones nearly 30 times during five periods. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract.
