Cieľ: Zámerom štúdie bolo zistiť, či dĺžka praxe ovplyvňuje mieru syndrómu vyhorenia v nami sledovanom súbore sestier a pôrodných asistentiek. Metódy: Zvolili sme štandardne využívaný dotazník na stanovenie miery syndrómu vyhorenia Maslach Burnout Inventory, ktorý on-line formou mohli vypĺňať sestry a pôrodné asistentky zaregistrované v Slovenskej komore sestier a pôrodných asistentiek. Vzorku tvorilo 844 sestier a pôrodných asistentiek. Výsledky: Hlbšou analýzou sa potvrdil štatisticky významný vzťah medzi mierou syndrómu vyhorenia a dĺžkou praxe. Sestry a pôrodné asistentky pracujúce v období od 1–3 rokov a od 5 rokov majú signifi kantne vyšší výskyt syndrómu vyhorenia. Závery: Zistené výsledky môžu uplatniť najmä zamestnávatelia v oblasti prevencie syndrómu vyhorenia., Aim: The aim of this study was to explore whether the length of employment aff ects the rate of burnout syndrome in nurses and midwives. Methods: In our research, we employed the standardized Maslach Burnout Inventory questionnaire, which was completed on-line by nurses and midwives registered in the Slovakian Guild of Nurses and Midwives. The sample consisted of 844 nurses and midwives registered by the Slovakian Guild of Nurses and Midwives. Results: Our results suggest signifi cant relation between the employment length and burnout syndrome. Nurses and midwives working from 1 to 3 years and more than 5 years had signifi cantly higher prevalence of burnout syndrome. Verejné zdravotné poistenie je založené na princípe solidarity.Autori v článku analyzujú desaťročné obdobie využívania indexov rizika nákladov v prerozdeľovacom mechanizme zdravotného poistenia v Slovenskej republike. Z výsledkov analýzy je zrejmý postupný nárast indexu rizika nákladov oproti roku 1999 v najvyšších vekových skupinách, v ktorých platiteľom poistného je štát a teda i nárast finančných prostriedkov, vynakladaných na poskytnutú zdravotnú starostlivosť v týchto vekových skupinách.Táto skutočnosť má význam aj z dôvodu prognóz vývoja stavu obyvateľstva, nakoľko trend vývoja populácie ukazuje starnutie obyvateľstva. Jeden z najprepracovanejších systémov prerozdeľovania poistného je zavedený v Spolkovej republike Nemecko a okrem veku a pohlavia zohľadňuje aj ďalšie parametre. V Českej republike bol systém nákladových indexov podľa pohlavia a 18 vekových skupín, členených po 5 rokoch zavedený od roku 2005. V porovnaní so Slovenskou republikou využíva aj prerozdeľovanie podľa výskytu poistencov, za ktorých bola zdravotnými poisťovňami uhradená zvlášť nákladná zdravotná starostlivosť. Finančne spravodlivejší solidárny systém v Slovenskej republike je teda možné dosiahnuť zavedením ďalšieho parametra do systému prerozdeľovania poistného., Public health care system is based on the solidarity. The authors of the article analyzed a decade during which the cost risk indices were applied to redistribution mechanism of the health care insurance in Slovak Republic. Conclusions of the analysis yield significant gradual increase in cost risk index as compared to the year 1999 in the group of elderly, whose insurance fees are covered by the state, as is the financial package supplied to the system for the provided health care in this group of patients. This finding has a prognostic significance given the fact that the general population is getting old. One of the most complexes of redistribution mechanism known is the system used in Germany. The German system adjusts for additional markers besides the age and gender. In the Czech Republic, the cost index system, being used since 2005, reflects the gender and 18 age groups - each age group being five years apart. Unlike Slovak Republic, the Czech system redistributes the finance flow depending on the distribution of health care participants and the most cost demanding health care covered by the insurance companies. Cílem práce je kritická reflexe probíhajících reforem péče ve visegrádských zemích (V4) v kontextu globalizace a ve vztahu k mezinárodnímu diskurzu těchto procesů. Výzkumné otázky se vztahují k základnímu pojetí a příčinám těchto reforem, k jejich zdravotním a sociálně-ekonomickým důsledkům. Práce využívá metody analýzy veřejných politik. Teoretická východiska: jsou zaměřena na hodnocení zdravotnických systémů a na kritickou reflexi společenských důsledků ekonomické globalizace. Zjištění: země V4 patří ke skupině transitivních ekonomik s horším zdravotním stavem (health gap) v porovnání s vyspělejšími státy OECD. Tento problém nové reformy neřeší. Tlak ekonomicky orientované globalizace (preference ekonomického růstu nad sociálním rozvojem) na nízkou cenu práce a omezení aktivit sociálního státu tuto situaci zhoršuje. Zdravotnické systémy V4 jsou dlouhodobě podfinancovány a nedosahují průměru vyspělých zemí OECD nebo EU. Zatímco cíle a výsledky reforem po roce 1990 vedly k rozvoji demokratických svobod a občanské participace, lze současné reformy po roce 2000 charakterizovat jako určitou negaci takového vývoje a omezení občanských svobod a lidských práv. Příčiny reforem po roce 2000 jsou spatřovány v rostoucím vlivu nad národních korporací a ekonomické globalizace. Zaměření reforem v transitivních ekonomikách se soustřeďuje téměř výhradně na ekonomické cíle, zatímco v rámci mezinárodního diskursu jsou rozvíjeny širší koncepty dlouhodobé udržitelnosti s důrazem na celkovou efektivitu a kvalitu produkce služeb a také jejich soulad s lidskými právy se vztahem ke zdraví., The aim of this article is a critical reflection of current health care reforms in Visegrad countries in the context of globalization and in relation to international discourse of these processes. The research questions are oriented at essential concept, causes and health and socioeconomic effect of these reforms. The main methods used in this work are methods of public policy analysis. Theoretical concepts target the evaluation of health systems and the critical reflection of social effects of economic globalization. Finding: Countries V4 are transitive economics with poorer heath status (health gap) in comparison with more development countries OECD. This problem is not solving by these reforms. The situation is going worse by pressures of economic globalization on low labour price and by reduction of welfare states activities.Health care systems of V4 countries are long-term sub financed and they don't reach level of development OECD or EU countries. While objectives and results of reforms after 1990 were directed at development of democratic freedoms and civic participation, we can designate current reforms (after 2000) as negation of this development and as a restriction of civic and human rights. V predkladanej štúdii autori ponúkajú prehľad problematiky zdravia rómskej populácie na Slovensku spolu s faktormi ktoré ho ovplyvňujú. Rozoberajú demografický vývoj Rómov v spojitosti s reprodukčným a populačným zdravím. Autori predkladajú analýzu zdravia Rómov založenú na výsledkoch parciálnych prieskumov a samohodnotení zdravotného stavu samotnými Rómami. Článok stručne mapuje oblasti najvýraznejších problémov zdravotného stavu rómskeho etnika na Slovensku. Informuje čitateľa o známych príčinách tohto stavu. Zlepšenie situácie v oblasti zdravia vidia autori v transkultúrnom prístupe k Rómom zo strany zdravotníckych pracovníkov a majoritnej spoločnosti a v realizácii efektívnej edukácie zameranej na podporu a ochranu zdravia., In the present study, the authors provide an overview of the health problems of the Roma population in Slovakia together with influencing factors and analyse factors. The demographic development of the Roma in conjunction with reproductive population health. The authors also present an analysis of the health of the Roma based on the partial results of surveys and self-assessed health status. The article briefly charts the area that is most outstanding in health problems of the Roma ethnic group in Slovakia. It informs the reader about the known causes of this condition.
