Objectives. The aim of this study was to explore possible relationships between the HEXACO personality traits (i.e., Extraversion, Emotionality, Honesty-Humility, Agreeableness, Conscientiousness, Openness to experience) and negative effects of psychotherapy. Sample and settings. A total of 148 respondents answered an online survey. Descriptive and multivariate regression analyses were performed. Hypotheses. Clients’ personality traits were expected to predict negative effects of psychotherapy. Results. Extraversion was the strongest negative predictor of negative effects, serving as a protective factor against worsening of symptoms, hopelessness, and stigma. Furthermore, Emotionality was related to lower perceived quality of psychotherapy, Conscientiousness was related to hopelessness, and Openness to experience served as a protector against dependency. Limitations. The findings are based on a onetime retrospective measurement and a convenience sample. The results must be interpreted with caution because the assumptions of linear regression were not fulfilled. and Cíle.Cílem této studie bylo prozkoumat možné vztahy mezi osobnostními rysy HEXACO (tj. Extraverze, Emocionalita, Poctivost-Pokora, Přívětivost, Svědomitost, Otevřenost vůči zku-šenosti) a negativními účinky psychoterapie.Výzkumný soubor a metody.V online průzkumu odpovídalo celkem 148 respondentů. Ve výzku-mu byly provedeny deskriptivní a vícerozměrné regresní analýzy. Hypotézy. Od osobnostních rysů klientů autoři očekávali, že predikují negativní účinky psy-choterapie. Výsledky. Extraverze byla nejsilnějším nega-tivním prediktorem negativních účinků psy-choterapie, sloužila jako ochranný faktor před zhoršením symptomů, beznadějí a stigmatizací. Faktor Emocionalita souvisel s nižší vnímanou kvalitou psychoterapie, Svědomitost souvisela s beznadějí a Otevřenost vůči zkušenosti slouži-la jako ochrana před závislostí na psychoterapii. Limity.Nálezy jsou založeny na jednorázovém retrospektivním měření a na limitujícím vzorku. Výsledky je nutné interpretovat opatrně, protože předpoklady lineární regrese nebyly zcela spl-něny.Závěr.Tato pilotní studie poskytla nové důkazy o roli faktorů na straně klienta při vysvětlování negativních účinků v psychoterapii. V datech vysvětlovaly osobnostní rysy HEXACO od 9 % do 23 % rozptylu u negativních účinků psycho-terapie. I když to znamená, že tyto osobnostní rysy hrají důležitou roli při vzniku negativních jevů, procenta vysvětlené variance (spolu se zkreslenými rezidui na provedených regres-ních analýzách) jasně ukázala, že existují další podstatné faktory, které je třeba pro vysvětlení těchto účinků objevit. Budoucí studie se mohou pokusit zahrnout kontextové, předterapeutické a terapeutické faktory, terapeutické procesy a důvody ukončení (Curran et al., 2019) spolu s osobnostními rysy klientů, aby určily jejich relativní vlivy na negativní účinky psychoterapie.
Objective. The Cooper-Norcross Inventory of Preferences (C-NIP) is a new and promising tool for measuring clients’ preferences regard-ing psychotherapy. However, the psychometric evaluation of this measure is scarce in general and completely missing for the Czech adapta-tion of the measure. This study aimed to test the Czech version of the C-NIP factor structure, test its measurement invariance, and establish cut points. Methods.N = 772 adults answered the C-NIP in an online survey. Confirmatory and explora-tory factor analyses were used to test the fac-tor structure and assess the C-NIP measurement invariance between men and women and across several levels of psychotherapy experience.Results. The original four-factor model was not supported. Instead, a five-factor model was sug-gested that fit the data adequately and was strict-ly invariant with respect to gender and levels of experience with psychotherapy. Conclusions. The Czech C-NIP can be con-sidered a valid and reliable measure of clients’ preferences regarding psychotherapy. The repli-cation of the new factor model is needed. and Cíl. Cooper-Norcross Inventory of Preferences (C-NIP) představuje nejnovější a slibný nástroj určený k měření preferencí klientů ve vztahu k psychoterapii. Psychometrické zhodnocení to-hoto nástroje je však zatím sporé a u české verze zcela chybí. Cílem této studie bylo ověřit fakto-rovu strukturu české verze C-NIP, ověřit invari-anci měření a stanovit hraniční skóry. Metody.N = 772 dospělých respondentů vypl-nilo C-NIP v online průzkumu. K ověření fak-torové struktury a invariance měření mezi po-hlavími a napříč různými úrovněmi zkušenosti s psychoterapií byla použita konfirmační fakto-rový analýza. Výsledky. Původní faktorová struktura nebyla podpořena. Namísto toho byl navržen pětifak-torový model, který vykazoval adekvátní shodu s daty a byl striktně invariantní ve vztahu k po-hlaví i úrovni zkušenosti s psychoterapií. Závěr. Českou verzi C-NIP lze považovat za validní a reliabilní nástroj na měření preferencí klientů ve vztahu k psychoterapii. Je zapotřebí replikovat navrženou faktorovou strukturu.
Objective. The Chronic Pain Acceptance Questionnaire – Symptoms (CPAQ-S, 20 items) measures patients’ acceptance of their symptoms. The questionnaire was created by reframing the Chronic Pain Acceptance Questionnaire-20 (CPAQ-20). This study describes the Czech validation of the full and short CPAQ-S forms.Sample and settings. The final sample consisted of 368 patients (71% female) recruited at seven clinical sites in the Czech Republic. Hypotheses. A hypothesized two-dimensional factor structure (Activity Engagement and Symptom Willingness) was tested together with other theoretically relevant factor solutions.Statistical analyses. An ordinal confirmatory and exploratory factor analysis were employed. Results. None of the theory-driven factor structures were confirmed in the CPAQ-S-20, and exploratory factor analysis did not yield any satisfactory factor solution. However, an eightitem version (CPAQ-S-8) was derived based on the factor analysis that was characterized by good psychometric properties even when retaining important facets of the expected two-factor structure (i.e., Activity Engagement and Symptom Willingness).Limitations. While the sample heterogeneity was conceived as a strength of the study, an underlying noninvariance across different types of complaints could have caused unsatisfactory functioning of the scale. and Cíl. Škála Chronic Pain Acceptance Questionnaire (20položková verze) měřící přijetí chronické bolesti byla adaptována k měření přijetí obecných chronických obtíží pacientů klinických zařízení: Chronic Pain Acceptance Questionnaire – Symptoms. Tato studie popisuje českou adaptaci plné a zkrácené (CPAQ-S) verze škály. Vzorek. Finální vzorek sestával z 368 pacientů (71 % žen) ze sedmi klinických zařízení v České republice. Hypotézy. Předpokládaná dvoufaktorová struktura (Zapojení do aktivit navzdory bolesti a Ochota snášet symptomy) byla testována společně s dalšími teoreticky relevantními modely. Statistické analýzy. Byla použita ordinální konfirmační a následná explorační faktorová analýza. Výsledky. Žádný z testovaných faktorových modelů CPAQ-S-20 nebyl podpořen daty. Využití explorační faktorové analýzy nepřineslo žádnou přidanou informační hodnotu. Empiricky odvozená osmi-položková verze škály (CPAQ-S-8) byla charakterizována poměrně dobrými psychometrickými vlastnostmi i po zachování očekávané dvoufaktorové struktury tj., Zapojení do aktivit navzdory bolesti a Ochota snášet symptomy. Limity. Zatímco heterogenitu vzorku je možné vnímat jako silnou stránku studie, heterogenita různých typů obtíží, jimiž pacienti ve vzorku trpěli, mohla způsobit neuspokojivé fungování škály.
Objective.The paper introduced the concept of narrative sensitivity. The study aimed to identify the facets of narrative sensitivity (NS) in medi-cal students and assess the changes in NS during undergraduate medical education.Methods. A mixed method study was designed to explore NS. Data was collected from a group of students in their second year (N=50) and then in their fifth year (N=42) of their undergraduate education. The data was analysed using thematic analysis in accordance with consensual qualita-tive research. A coding system was developed to identify and refine the facets of NS. The theme frequencies were counted for each year sepa-rately and Fisher’s exact test was conducted to assess the statistical significance of NS change between both years.Results. Nine themes related to NS were identi-fied in the data. The frequency of themes indi-cated a trend towards a decline in NS between the second and fifth years of undergraduate edu-cation. However, the exploratory nature of this study and the small sample size did not allow any definitive conclusions regarding changes over time.Conclusions. The findings raised doubts about the outcome of undergraduate education based on a narrow biomedical perspective. The study supported the arguments for giving emphasis on training of communication skills including nar-rative sensitivity during medical education. and Cíl.Narativní koncept obohatil kromě psycholo-gie a psychoterapie také medicínu. V zahraničí došlo ke vzniku vzdělávacích programů v nara-tivní medicíně zaměřených na rozvoj narativních dovedností u studentů medicíny a začínajících lékařů. V této souvislosti text představuje pojem narativní citlivosti. Cílem explorativní studie bylo identifikování fazet narativní citlivosti u studentů medicíny a posouzení změn v narativní citlivosti u těchto studentů v průběhu pregraduálního studia. Soubor a metody.V rámci smíšené explorativ-ní studie byla od dospělých účastníků výzkumu získána data v podobě písemných výpově-dí v reakci na podnětovou fotografii nejprve ve druhém ročníku pregraduálního studia medi-cíny (N = 50) a posléze v pátém ročníku (N = 42). Data byla analyzovaná metodou tematické analýzy s prvky konsensuálního kvalitativního výzkumu. Byl vytvořen kódovací systém s cí-lem identifikování fazet narativní citlivosti. Čet-nost témat byla kvantifikována v každém roční-ku zvlášť a k posouzení statistické významnosti změny v narativní citlivosti u studentů během tří let studia byl použit Fisherův test. Výsledky.Během textové analýzy bylo identi-fikováno devět témat relevantních ke koncep-tu narativní citlivosti. Četnost témat naznačila pokles narativní citlivosti u studentů medicíny mezi druhým a pátým ročníkem pregraduální-ho studia. Nicméně explorativní design studie a malý soubor neumožnily učinit definitivní závěr týkající se posouzení změny narativní citlivosti v čase.Závěr. Navzdory limitům studie výsledky vyvo-lávají pochybnosti o výsledcích pregraduálního studia medicíny založeném na biomedicínském modelu. Studie podporuje argumenty pro vět-ší důraz na rozvoj komunikačních dovedností včetně narativní citlivosti u budoucích lékařů během studia medicíny.
Objectives. This study aimed to assess psychometric properties, such as reliability, construct validity, and cut-off scores, for the Czech version of the Clinical Outcome in Routine Evaluation – General Population (GP-CORE) questionnaire, a tool usable for repeated measurement of psychological distress within routine clinical settings. Participants and setting. Two general populations and one clinical sample were used with N values of 420, 394, and 345, respectively. Hypotheses. One of the competing theoretical factor solutions will demonstrate the best fit. Statistical analysis. To examine the factor structure of the GP-CORE, a confirmatory multidimensional item response theory analysis (graded response model) was employed. Results. The best fitting model was a bifactor solution representing one content domain of overall distress and two item wording domains (positively and negatively worded items). Clinical cut-off scores were determined to be 1.85 (men) and 1.90 (women). Study limitations. The GP-CORE can be used as an unidimensional measure of overall distress, but users have to be aware of the influence of positive vs. negative item wording on the responses. and Cíle. Tato studie si kladla za cíl zhodnotit psychometrické vlastnosti české verze škály Clinical Outcome in Routine Evaluation - General Population (GP-CORE), konkrétně poskytnout informace o reliabilitě, konstruktové validitě a klinickém cut-off skóru. GP-CORE je nástroj využitelný pro opakované měření psychologického stresu. Vzorek a design. Byly využity tři vzorky respondentů, z nichž dva pocházely z obecné (N = 420 a 394) a jeden z klinické populace (N = 345). Hypotézy. Jedno z faktorových řešení bude mít superiorní fit oproti ostatním. Statistické analýzy. K ověření faktorové struktury GP-CORE bylo využito konfirmační multidimenzionální teorie odpovědi na položku (graded response model). Výsledky. Jako finální model byl zvolen bi-faktorový model reprezentující obecný obsahový faktor distresu a dva metodické faktory způsobené pozitivní a negativní formulací položek. Kromě toho, že vykazoval nejlepší fit, byl také dobře teoreticky interpretovatelný. Klinický cut-off skór pro průměrnou hodnotu napříč položkami byl ustanoven na 1,85 pro muže a 1,90 pro ženy. Limity. GP-CORE lze využít jako jednodimenzionální nástroj pro měření obecného psychologického stresu, ale uživatelé by si měli být vědomi také možného vlivu pozitivní či negativní formulace položek na odpovědi respondentů.
Objectives. The study aimed to explore which barriers adult people perceive when deciding whether to enter psychotherapy and how these barriers influence their decision to start psycho-therapy.Sample and setting. The sample consisted of 392 adult respondents who considered entering individual psychotherapy during the last five years. They completed a questionnaire on per-ceived barriers to entering psychotherapy, the phase of decision-making, and the severity of psychological symptoms.Hypotheses. This was an exploratory study with the following questions: a) What is the per-ceived importance of specific barriers to enter-ing psychotherapy? b) What groups (principal components) of barriers to entering psychother-apy can be identified? c) What is the relationship between the perceived importance of barriers and the decision to enter psychotherapy?Statistical analysis. The number of barriers was reduced using PCA. The relationship of the decision-making phase to other variables was explored via multinomial regression analysis.Results. The most severe barrier to entering psychotherapy were price, reluctance to express emotions in front of others and the assumption that one’s problems were not as severe as the problems of people in psychotherapy. Respond-ents who decided not to enter psychotherapy were older, expressed more distrust of psycho-therapy, and perceived it as more stigmatizing than those who contacted a therapist. Respond-ents who decided to enter psychotherapy (but who had not contacted a therapist) had less se-vere symptoms and perceived psychotherapy as more stigmatizing than those who contacted a therapist. Study limitations. Limits of the study include a non-representative sample and the time window used for the retrospective rating of symptoms and barriers. and Cíl. Cílem studie bylo prozkoumat, jaké překáž-ky dospělí lidé vnímají při rozhodování o vstu-pu do psychoterapie a jak tyto překážky souvisí s rozhodnutím psychoterapii zahájit.Soubor a metoda. Výzkumný soubor tvořilo 392 dospělých respondentů, kteří během posledních pěti let zvažovali zahájení individuální psycho-terapie. Respondenti vyplnili dotazník zaměře-ný na vnímané překážky vstupu do psychotera-pie, fázi rozhodování vstoupit do psychoterapie a intenzitu potíží. Hypotézy. Studie měla explorační charakter, za-měřila se na následující otázky: a) Jak vnímají lidé závažnost jednotlivých překážek vstupu do psychoterapie? b) Jaké skupiny (hlavní kom-ponenty) překážek pro vstup do psychoterapie můžeme identifikovat? c) Jak souvisí závažnost vnímaných překážek s rozhodnutím vstoupit do psychoterapie?Statistická analýza. Množství překážek autoři nejprve redukovali pomocí analýzy hlavních komponent. Souvislost fáze rozhodování o vstu-pu do psychoterapie s dalšími proměnnými byla ověřována pomocí multinomiální regresní ana-lýzy. Výsledky. Jako nejzávažnější překážka byla vní-mána cena psychoterapie, následovaly zábrany v odhalování emocí před ostatními a pocit, že vlastní potíže nejsou tak závažné jako potíže lidí navštěvujících psychoterapii. Respondenti, kteří zavrhli vstup do psychoterapie, byli starší, vyja-dřovali větší nedůvěru k psychoterapii a vnímali ji jako více stigmatizující než ti, kdo kontakto-vali terapeuta. Účastníci, kteří vstup do terapie zvažovali (ale terapeuta nekontaktovali), vyka-zovali nižší míru potíží a vnímali psychoterapii jako více stigmatizující než ti, kdo kontaktovali terapeuta.Omezení studie. Limity studie jsou nereprezen-tativnost výzkumného souboru a využití delšího časového rozmezí pro retrospektivní hodnocení úrovně potíží a významnosti překážek.
