Perioperační péče o diabetika je specifická nejen nezbytností adekvátní korekce glykemie, ale také výskytem komplikací, které se u nediabetiků nevyskytují. Dosavadní antidiabetickou léčbu je proto většinou nutno přizpůsobit perioperačnímu období, u malých a krátkých chirurgických výkonů to však není většinou nutné. Perioperační péči je nezbytné u diabetiků pečlivě zorganizovat a naplánovat. Victoza (liraglutid) je moderní antidiabetikum ze skupiny GLP1(glucagon like peptid) analog; v České republice je dostupné od roku 2009. Je indikována v terapii diabetiků 2. typu se zachovalou endogenní produkcí inzulinu, nejčastěji v kombinaci s perorálními antidiabetiky či bazálním inzulinem. Aplikuje se v 1 denní dávce podkožně. Kromě základního antidiabetického efektu s protekcí B buněk je výhodný i podpůrný účinek na redukci hmotnosti. Pozitivní efekt na kardiovaskulární systém je očekávaný, výsledky klinických studií ovšem aktuálně nejsou k dispozici. Z nežádoucích účinků se v běžné klinické praxi nejčastěji setkáme s mírnými projevy dyspepsie horního typu při zahájení terapie, které odeznívají v řádu dnů. Celkově jde o velmi dobře tolerovaný preparát s potenciálem redukce glykovaného hemoglobinu většinou do 15 mmol/l a zároveň minimálním rizikem vzniku hypoglykemie., Victoza (liraglutide) is a modern antidiabetic drug of GLP1 (glucagone like peptide) analogue group; since 2009 registered in Czech republic.
