Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). A total of 711 patients (17.0%) receiving LCZ696 and 835 patients (19.8%) receiving enalapril died (hazard ratio for death from any cause, 0.84; 95% CI, 0.76 to 0.93; P < 0.001); of these patients, 558 (13.3%) and 693 (16.5%), respectively, died from cardiovascular causes (hazard ratio, 0.80; 95% CI, 0.71 to 0.89; P < 0.001). As compared with enalapril, LCZ696 also reduced the risk of hospitalisation for heart failure by 21% (P < 0.001) and decreased the symptoms and physical limitations of heart failure (P = 0.001). The LCZ696 group showed higher proportions of patients with hypotension and non‑serious angioedema but lower proportions of patients with renal impairment, hyperkalemia, and cough when compared to the enalapril group. Conclusions: LCZ696 was more effective than enalapril in reducing the risk of death and of hospitalisation for heart failure. Předpoklad: Studie IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) hodnotila potenciální prospěch na snížení velkých kardiovaskulárních (KV) příhod při přidání ezetimibu vs placebo k léčbě 40 mg simvastatinu u nemocných s akutním koronárním syndromem a nízkou hladinou LDL-cholesterolu ≤ 125 mg/dl. Metodika: Randomizovaná dvojitě slepá studie u nemocných po akutním koronárním syndromu s nízkou hladinou cholesterolu. Ve větvi se simvastatinem v monoterapii byl cílový LDL-cholesterol < 70 mg/dl, druhá větev byla kombinace simavastatin + ezetimib. Předpokladem bylo, že ezetimib sníží hladinu LDL-cholesterolu o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (MI), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo non-ST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října roku 2005 do července roku 2010. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %; p = 0,016) léčených kombinací. Pacienti léčení kombinací simvastatin + ezetimib vs pacienti léčení simvastatinem + placebo měli o 6,4 % nižší kombinovaný cíl kardiovaskulární úmrtí, nefatální infarkt myokardu, nefatální cévní mozková příhoda, hospitalizace pro nestabilní anginu pectoris a revaskularizace po 30 dni. Srdeční infarkty byly sníženy o 13 %, nefatální CMP o 20 %. Úmrtí z kardiovaskulární příčiny bylo v obou skupinách stejné. Průměrná doba sledování byla 6 let, nejdelší 8,5 roku. Průměrně 2 pacienti ze 100 předešli kardiovaskulární příhodě za 7 let (Number Needed to Treat – NNT) 50/7 let. Závěr: Studie prokázala jasný prospěch z kombinační léčby simvastatinem a ezetimbem u nemocných po akutním koronárním syndromu a nízkým LDL-cholesterolem., Background: The IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) is evaluating the potential benefit for reduction in major cardiovascular (CV) events from the addition of ezetimibe versus placebo to 40 mg/d of simvastatin therapy in patients who present with acute coronary syndromes and have low-density lipoprotein cholesterol (LDL-C) ≤ 125 mg/dl. Methods: Randomized double blind clinical trial in patients with acute coronary syndrome and low cholesterol level. The simvastatin monotherapy arm’s LDL-C target was < 70 mg/dl, the comparison arm was simvastatin + ezetimibe. Ezetimibe was assumed to further lower LDL-C by 15 mg/dl and produce an estimated ~ 8 % to 9 % treatment effect. The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, rehospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postrandomization). The targeted number of events was 5,250. Results: 18,144 patients were enroled with either ST segment elevation MI (STEMI, n = 5,192) or UA/non-ST segment elevation MI (UA/NSTEMI, n = 12,952) from October 2005 to July 2010. Primary endpoint occured in 2 742 patients (34.7 %) treated with simvastatin in monotherapy and in 2 572 patients (32.7 %) (p = 0.016) treated with combination. Compared to patients with coronary heart disease given the drug simvastatin plus a placebo, those given both simvastatin and the non-statin drug, ezetimibe, had a 6.4 % lower combined risk of subsequent heart attack, stroke, cardiovascular death, rehospitalization for unstable angina and procedures to restore blood flow to the heart. Heart attacks alone were reduced by 13 %, and non-fatal stroke by 20 %. Deaths from cardiovascular disease were statistically the same in both groups. Patients were followed an average of approximately six years, and some as long as 8.5 years. Approximately 2 patients out of every 100 patients treated for 7 years avoided a heart attack or stroke [Number Needed to Treat (NNT) = 50/7years]. Předpoklad: Tikagrelor je inhibitor receptoru P2Y12 a má prokázaný účinek na snížení ischemických příhod 1 rok po akutním infarktu myokardu. Jeho efekt v delším časovém období nebyl znám. Metodika: Zařazeno bylo 21 162 nemocných s anamnézou infarktu myokardu 1–3 roky před randomizací, která byla v poměru 1 : 1 : 1 na skupiny: tikagrelor 2krát 90 mg : tikagrelor 2krát 60 mg : placebo. Všichni nemocní museli být léčeni malou dávkou kyseliny acetylsalicylové. Pacienti byli sledováni 33 měsíců. Primární cíle byly: kardiovaskulární úmrtí, infarkt myokardu nebo cévní mozková příhoda. Primárním bezpečnostním cílem bylo krvácení. Výsledky: Obě dávky tikagreloru signifikantně snížily primární kombinovaný cíl za 3 roky – 7,85 % tikagrelor 2krát 90 mg, 7,77 % tikagrelor 2krát 60 mg a 9,04 % placebo (HR pro tikagrelor 2krát 90 mg vs placebo 0,85; 95% CI 0,75–0,96; p = 0,0080; HR pro tikagrelor 2krát 60 mg vs placebo 0,84; 95% CI 0,74–0,95; p = 0,0043). The efficacy and safety of long-term ticagrelor therapy beyond 1 year after a myocardial infarction is unknown. Methods: We randomized 21,162 patients with a history of myocardial infarction within the prior 1–3 years in a double-blind 1 : 1 : 1 fashion to ticagrelor 90 mg twice daily, ticagrelor 60 mg twice daily, or placebo, all with low-dose aspirin, and followed them for a median of 33 months. The primary efficacy endpoint was the composite of cardiovascular death, myocardial infarction, or stroke. The primary safety endpoint was TIMI major bleeding. Results: Both doses of ticagrelor significantly reduced the primary combined efficacy endpoint compared to placebo with Kaplan-Meier rates at 3 years of 7.85 % with ticagrelor 90 mg, 7.77 % with ticagrelor 60 mg, and 9.04 % with placebo (HR for ticagrelor 90 mg vs placebo 0.85, 95% CI 0.75–0.96, p = 0.0080; HR for ticagrelor 60 mg vs placebo 0.84, 95% CI 0.74–0.95, p = 0.0043). Úvod: Prognostický význam laboratorních parametrů asociovaných s postižením jater byl prokázán v mnoha studiích, nicméně jen málo prací sleduje prediktivní vliv jaterních enzymů u akutních koronárních syndromů (AKS). Cílem naší práce bylo zjistit prognostický vliv abnormalit funkčních jaterních testů u pacientů hospitalizovaných pro AKS s manifestním srdečním selháním. Metodika a výsledky: Z celkového souboru 8 818 pacientů v registru AHEAD (Acute Heart Failure Database) bylo vybráno 359 pacientů s diagnózou AKS a dostupnými hodnotami aspartátaminotransferázy (AST), alaninaminotransferázy (ALT), alkalické fosfatázy (ALP) a gamaglutamyltransferázy (GGT). Výskyt abnormálních hodnot zkoumaných laboratorních parametrů v našem souboru byl následující – AST u 71,3 %, ALT u 55,2 %, ALP u 11,7 % a GGT u 31,8 % pacientů. S krátkodobou (30denní) celkovou mortalitou byly významně asociovány abnormální hodnoty AST a ALT, zatímco dlouhodobou celkovou mortalitu signifikantně ovlivňovaly abnormality AST, ALP a GGT. Závěr: I přes vývoj moderních biomarkerů myokardiální ischemie zůstává role funkčních jaterních testů díky jejich snadné proveditelnosti a cenové dostupnosti v diagnostice infarktu myokardu nezastupitelná. Jak však naznačují výsledky naší studie, neméně důležitý je i jejich prediktivní význam., Introduction: The prognostic significance of the laboratory parameters associated with hepatic impairment has been proven in many studies. However, only a few of them focused on the predictive value of liver enzymes in acute coronary syndromes (ACS). The aim of our study was to evaluate the prognostic significance of abnormal liver function tests in patients hospitalised with ACS and concomitant acute heart failure. Methods and results: Of 8,818 patients included in the AHEAD (Acute Heart Failure Database) registry, 359 patients had ACS on admission and complete records on aspartate aminotransferase (AST), alanine aminotransferase (ALT), g‑glutamyltransferase (GGT), and alkaline phosphatase (ALP) serum levels. The incidence of abnormalities was as follows – AST in 71.3%, ALT in 55.2%, ALP in 11.7% and GGT in 31.8%. A significant association with short‑term (30-day) mortality was found in AST and ALT. A significant influence of AST, ALP and GGT was found in the case of long‑term mortality. Conclusion: Despite the development of modern myocardial ischaemia biomarkers, the role of liver function tests due to their high accessibility and affordability remains irreplaceable.
