Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Paliativní péče klade důraz na kvalitu života nemocného a saturaci jeho bio-psycho-sociálních a spirituálních potřeb. V zahraničí se používají nástroje pro hodnocení potřeb pacientů v onkologické či paliativní péči, které pomáhají zdravotnickým pracovníkům porozumět specifickým potřebám pacientů, které by mohly být předmětem profesionální individualizované péče. Cílem tohoto přehledového článku je analyzovat měřící nástroje pro hodnocení potřeb pacientů v paliativní péči, porovnat je s doporučenými postupy pro tvorbu měřících nástrojů v paliativní péči a s českou verzí dotazníku PNAP. V elektronických databázích (Medline, Scopus, Proquest Public Health a Web of Knowledge) bylo nalezeno a dále podrobněji analyzováno 12 měřících nástrojů. Český dotazník PNAP pokrývá velkou část nejčastěji měřených potřeb i doporučení plynoucích ze směrnic pro měření potřeb v paliativní péči, které byly vytvořeny na základě vědeckých důkazů., Palliative care emphasizes quality of life of the ill and the saturation of his bio-psycho-social and spiritual needs. Abroad are used tools for assessing needs of patients in oncological or palliative care, which helps to health workers to understand the specific needs of patients that could be subject to professional individualized care. The aim of this review is to analyse measuring tools for assessing needs of patients in palliative care, to compare them with recommended procedures for creation of measurement tools in palliative care and with Czech version of the questionnaire PNAP. In the electronic databases (Medline, Scopus, Proquest Public Health and Web of Knowledge) was recovered and further analysed in detail 12 measuring tools. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. Cíl: Cílem studie bylo zjistit psychometrické vlastnosti české verze škály QUALID pro hodnocení kvality života pacientů s těžkou demencí a posoudit vhodnost použití v ČR. Dále pak zjistit, zda dochází ke změně v hodnocení kvality života během hospitalizace. Soubor a metodika: Výzkumný soubor tvořilo 184 pacientů s těžkým stupněm demence, kteří byli hospitalizováni ve FN Ostrava z důvodu pokročilé fáze nemoci. Škála QUALID byla přeložena do češtiny podle standardní metodiky. Pro hodnocení psychometrických vlastností dotazníku byla hodnocena reliabilita, a to vnitřní konzistence škály (Cronbachův koeficient alfa) a inter‑<span class=""></span>rater reliabilita (shoda mezi dvěma posuzovateli) pomocí Cohenova Kappa indexu a hodnoty ICC (Intraclass Correlation). Konstruktová validita byla ověřena Spearmanovým korelačním koeficientem mezi celkovým skórem a doménami QUALID a položkami dotazníku SM‑<span class=""></span>EOLD. Výsledky: Pomocí Cronbachova koeficientu alfa byla prokázána dobrá vnitřní konzistence škály QUALID (<span class="">α</span> = 0,820). Inter‑<span class=""></span>rater reliabilita (ICC) byla zjištěna vysoká (ICC = 0,8317; p < 0,01). Předpokládaná střední závislost mezi QUALID a SM‑<span class=""></span>EOLD (r > 0,3) byla zjištěna u domény diskomfort a v celkovém skóre QUALID. Pomocí vícerozměrné regresní analýzy byla prokázána souvislost mezi nižší kvalitou života a bolestí, poruchou kůže, obavami, strachem a znepokojením. Během hospitalizace došlo k signifikantnímu zlepšení kvality života pacientů s těžkou demencí (p < 0,01). Závěry: U škály QUALID v české verzi byla zjištěna dobrá reliabilita i vyhovující konstrukční validita. Škála je vhodná pro použití v ČR. Byla prokázána souvislost mezi zatěžujícími symptomy a nižší kvalitou života., Aim: The aim of this study was to examine psychometric properties of the Czech version of the QUALID scale for evaluating the QoL in patients with severe dementia, to assess suitability of its use in the Czech Republic and to establish whether there is a change in the QoL during hospitalization. Sample and methods: The sample consisted of 184 patients with severe dementia who were hospitalized at the University Hospital Ostrava. The QUALID was translated into Czech using standard methodology. To evaluate psychometric properties of the questionnaire, reliability, namely internal consistency (Cronbach´s alpha) and inter-rater reliability (Cohen´s kappa, ICC), was assessed. Construct validity was verified using the Spearman correlation coefficient for the total QUALID score and its domains and for SM-EOLD items. Results: Good internal consistency of the QUALID (<span class="">α</span> = 0.820) was proved using Cronbach´s alpha. Inter-rater reliability was found to be high (ICC = 0.8317; p < 0.01). Assumed mean dependency between QUALID and SM-EOLD (r > 0.3) was found for the discomfort domain and for the QUALID total score. Multivariate regression analysis found a correlation between lower QoL and pain, skin disorder, anxiety, fear and concern. During hospitalization, QoL was significantly improved in patients with severe dementia (p < 0.01). Conclusions: the Czech version of the QUALID was found to have good reliability as well as satisfactory construct validity. The scale is suitable for the use in the Czech Republic. A correlation was found between burdensome symptoms and lower QoL.
