We will deal with a new geometrical interpretation of the classical Legendre and Jacobi conditions: they are represented by the rate and the magnitude of rotation of certain linear subspaces of the tangent space around the tangents to the extremals. (The linear subspaces can be replaced by conical subsets of the tangent space.) This interpretation can be carried over to nondegenerate Lagrange problems but applies also to the degenerate variational integrals mentioned in the preceding Part II.
The criteria of extremality for classical variational integrals depending on several functions of one independent variable and their derivatives of arbitrary orders for constrained, isoperimetrical, degenerate, degenerate constrained, and so on, cases are investigated by means of adapted Poincare-Cartan forms. Without ambitions on a noble generalizing theory, the main part of the article consists of simple illustrative examples within a somewhat naive point of view in order to obtain results resembling the common Euler-Lagrange, Legendre, Jacobi, and Hilbert-Weierstrass conditions whenever possible and to discuss some modifications necessary in the degenerate case. The inverse and the realization problems are mentioned, too.
Variational integrals containing several functions of one independent variable subjected moreover to an underdetermined system of ordinary differential equations (the Lagrange problem) are investigated within a survey of examples. More systematical discussion of two crucial examples from Part I with help of the methods of Parts II and III is performed not excluding certain instructive subcases to manifest the significant role of generalized Poincaré-Cartan forms without undetermined multipliers. The classical Weierstrass-Hilbert theory is simulated to obtain sufficient extremality conditions. Unlike the previous parts, this article is adapted to the category of continuous objects and mappings without any substantial references to the general principles, which makes the exposition self-contained.

Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J. Cíl: Podle literatury dosahuje četnost komplikací kranioplastiky po dekompresivní kraniektomii i více než 30 %. Cílem studie je hodnocení četnosti, příčin a léčby komplikací kranioplastik po dekompresivní kraniektomii pro expanzivně se chovající supratentoriální ischemii nebo kraniocerebrální poranění a analýza důvodů neprovedení kranioplastiky u části nemocných po dekompresivní kraniektomii. Soubor a metodika: V retrospektivní studii nemocných po dekompresivní kraniektomii (2006–2014) byla kranioplastika provedena u 25 z 35 nemocných s expanzivní malacií (71,4 %) a u 33 z 68 zraněných s poúrazovou nitrolební hypertenzí (48,5 %). Výsledky: Nejčastější příčinou neprovedení kranioplastiky bylo úmrtí. U 20 % nemocných s mozkovou ischemií a 61,3 % zraněných byla kranioplastika provedena časně (do 10 týdnů). Četnost neurochirurgických komplikací byla u kranioplastik u nemocných s expanzivní malacií 12 % (explantace plastiky u 8 %) a u kranioplastik po kraniocerebrálním poranění dosahovala 9,1 % (explantace plastiky u 3 %). Hydrocefalus a subdurální kolekce byly před kranioplastikou přítomny u 6 a 9,1 % zraněných. Četnost hydrocefalu po kranioplastice byla u kraniotraumat 6 % a u mozkové ischemie 4 %. Závěry: Četnost komplikací kranioplastik, byť nižší než uvádí literatura, převyšuje hodnoty očekávané u elektivních operací. Subdurální kolekce a hydrocefalus nelze jednoznačně hodnotit jako komplikaci kranioplastiky, protože mohou být přítomny již před kranioplastikou v důsledku základního onemocnění., Aim: Reported incidence of cranioplasty complications after decompressive craniectomy may exceed 30%. The aim of the study was to analyse the incidence, causes and treatment options for neurosurgical complications of cranioplasty after decompressive craniectomy for expansive supratentorial ischaemic stroke and craniocerebral injury and to analyse the reasons for not performing cranioplasty in a subgroup of patients after decompressive craniectomy. Patients and methods: During a retrospective study (2006–2014) cranioplasty was performed in 25 of 35 patients after decompressive craniectomy for expansive supratentorial ischaemia (71.4%) and in 33 of 68 (48.5%) patients after decompressive craniectomy for posttraumatic intracranial hypertension. Results: Death was the most frequent reason for not performing cranioplasty after decompressive craniectomy. The frequency of early cranioplasties (< 10 weeks after decompressive craniectomy) was 20% in patients with ischaemic stroke and 61.3% in head-injured patients. The incidence of neurosurgical complications was 12% in patients after expansive brain ischaemia (cranioplasty explantation rate 8%). The incidence of cranioplasty complications in patients after craniocerebral injury was 9.1% (cranioplasty explantation rate 3%). Subdural collections and hydrocephalus were observed in 9.1% and 6% of patients after craniocerebral injury before cranioplasty, resp. Hydrocephalus requiring neurosurgical treatment was found in 6% of patients after craniocerebral injury and 4% of patients with brain ischaemia during the postcranioplasty period. Conclusions: Although the incidence of cranioplasty complications after decompressive craniectomy is lower than that reported in literature, it exceeds the rates expected for elective surgeries. Subdural collections and hydrocephalus cannot be unequivocally considered a complication of cranioplasty, because they are frequently observed before cranioplasty and may be caused by the disease requiring decompressive craniectomy. Cíl: Výzkum se zaměřuje na zjištění míry syndromu vyhoření, motivačních faktorů dle Herzbergovy dvoufaktorové teorie a jejich vzájemného vztahu u všeobecných sester pracujících ve Fakultní nemocnici Olomouc. Cílem výzkumu bylo poznání osobních priorit faktorů pracovního prostředí všeobecných sester a stupně jejich saturace zaměstnavatelem. Vzájemnou komparací zjištěného pak přehledně ukázat největší divergence mezi přáními a realitou všeobecných sester v pracovním prostředí a jejich souvislostí se syndromem vyhoření. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byly použity dva dotazníky: standardizovaný dotazník MBI (Maslach Burnout Inventory) a nestandardizovaný dotazník Motivačních faktorů vytvořený na principech Herzbergovy dvoufaktorové motivační teorie a po diskuzích s manažery nemocnic. Výsledky: Z výsledků výzkumu vyplynulo, že existuje statisticky významný vztah mezi mírou syndromu vyhoření a mírou neuspokojení faktorů pracovního prostředí dle Herzbergovy teorie. Nepotvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou syndromu vyhoření a spokojeností s fi nanční odměnou. Na žebříčku preferencí mají nejvyšší pracovní hodnotu faktory mzda, péče o pacienty a jistota pracovního místa. Mezi nejvíce divergující faktory patřila mzda a jistota pracovního místa. Výsledky potvrzují protektivní vliv saturace faktorů pracovního prostředí dle Herzbergovy teorie na výskyt syndromu vyhoření. Závěr: Výzkum prokázal význam saturace faktorů pracovního prostředí jako protektivního faktoru ve vztahu k syndromu vyhoření. Potvrdilo se, že se zvyšující se mírou saturace se snižuje míra vyhoření u všeobecných sester pracujících ve Fakultní nemocnici Olomouc., Aim: Th e research deals with the rate of burnout syndrome, motivation factors of Herzberg’s two-factor motivation theory and their mutual relation in general nurses working in the University Hospital Olomouc. Th e aim was to identify personal priorities among hygiene factors with general nurses and the degree of their saturation by the employer. Comparison of the fi ndings clearly shows the biggest divergence between wishes and reality of the working environment of general nurses and the relation to burnout syndrome. Methods: A quantitative approach was chosen for the research. To collect data, two questionnaires were used: a standardised MBI (Maslach Burnout inventory) and a non-standardised Motivation factors questionnaire, which was on the principles of Herzberg’s motivation theory aft er several discussions with hospital management. Results: Th e result of the survey research revealed an existing, statistically signifi cant relation between the burnout syndrome rate and the degree of dissatisfaction with hygiene factors according to Herzberg’s theory. Th e assumption about the existence of a signifi cant negative relation between burnout syndrome rate and salary was not confi rmed. Factors of salary care for the patient and job security occupy the top levels on the preference list. Th e most diverging factors include salary and job security. Th e results confi rm the protective eff ect the saturation of Herzberg’s hygiene factors has on the burnout syndrome rate. Conclusion: Th e survey research confi rmed the importance of saturating hygiene factors as a protective factor against burnout syndrome. Poruchy kortikálního vývoje a benigní tumory mozku jsou častou příčinou dlouhotrvající farmakorezistentní epilepsie u dětských i dospělých nemocných. Tito nemocní jsou operačně řešeni a následně sledováni v epileptochirurgických centrech. Progrese gradingu mozkových tumorů u nemocných s farmakorezistentní epilepsií je vzácným problémem a rozvoj maligního tumoru mozku u nemocných s malformacemi kortikálního vývoje mozku je publikován zcela výjimečně. Ve sdělení prezentujeme dvě kazuistiky nemocných s farmakorezistentní epilepsií s neočekávaným rozvojem maligního tumoru mozku. U jedné nemocné s chronickou epilepsií došlo k velmi časné progresi gradingu opakovaně histologicky verifikovaného nízkostupňového gliomu s finálním nálezem glioblastomu. U druhého nemocného byl popsán výjimečný rozvoj glioblastomu v terénu hemimegalencefalie., Malformations of cortical development and benign brain tumors are frequent causes of long lasting pharmacoresistant epilepsy in paediatric and adult patients. Such patients are surgically treated and subsequently followed in epileptosurgical centers. Tumor upgrading in patients with pharmacoresistant epilepsy is only rarely encountered and the development of malignant brain tumor in a patient with malformation of cortical development is an exception. The aim of the paper is to present two cases of patients suffering from intractable epilepsy with unexpected development of malignant brain tumor. There was upgrading of low grade glioma (verified by repeated biopsy sampling) in one patient with chronic epilepsy with final diagnosis glioblastoma multiforme. Histologická verifikace CT nebo MRI suspektního tumoru mozku je v současnosti nutnou podmínkou pro indikaci komplexní onkologické terapie. Strukturální patologie postihující zadní jámu lební jsou méně častým cílem stereotaktické biopsie pro koncentraci vitálních struktur v omezeném prostoru a komplikovanější přístup. Volba přístupu je dána především lokalizací léze ve vztahu k jednotlivým strukturám mozkového kmene. Je možné využít přístup transfrontální v dlouhé ose mozkového kmene (léze postihující mezencefalon, horní pons), transtentoriální přístup (léze mozečkové hemisféry a vermis mozečku) a přímý transcerebellární přístup (léze hemisféry mozečku a pontu, zvláště laterálního). Cílem sdělení je prezentace kazuistik 2 nemocných s diagnosticky nejasnými patologickými procesy postihujícími struktury zadní jámy lební, u kterých výsledek stereotaktické biopsie vedl k stanovení diagnózy, která zásadním způsobem ovlivnila další léčebný postup ve smyslu indikace komplexní onkologické terapie., Histological verification of CT or MRI suspected brain tumor is at prezent time necessary requirement for the indication of complex oncological treatment. Lesions affecting posterior fossa structures are less frequent target for stereotactic biopsy because of the concentration of vital structures in the limited space and more demanding approach. The choice of the approach is determined mainly by the lesion relationship to brainstem components. It is possible to use transfrontal approach along the long axis of the brainstem (lesions affecting mezencephalic and upper pontine region), transtentorial (lesions affecting cerebellar hemisphere and vermis) and direct transcerebellar (lesions of cerebellar hemisphere and pons, especially lateral). The paper analyses the use of the modified transcerebellar approach in 2 patients with lesions of unclear etiology affecting posterior fossa structures. Cíl: Práce se zaměřuje na zjištění vlivu sociální opory jako protektivního faktoru zdraví na míru deprese, úzkosti a životní spokojenosti u pacientů léčených pro lékařskou diagnózu F10.2 Závislost na alkoholu a F32 Depresivní epizoda na odděleních psychiatrie. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byla použita dotazníková baterie. Baterii tvořily čtyři standardizované dotazníky: PSSS (Perceived Social Support Scale) J. A. Blumenthala, BDI-II (Beck Depression Inventory) Becka a kol., SAS (Self Rating Anxiety Scale) Zungova sebeposuzovací stupnice úzkosti a DŽS – Dotazník životní spokojenosti Fahrenberga et al. Výsledky: Z výsledků výzkumného šetření vyplynulo, že existuje statisticky významný rozdíl v míře sociální opory mezi skupinou kontrolní a zkoumanou (pacienti s lékařskou diagnózou deprese a závislost na alkoholu). Potvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou úzkosti a sociální opory u pacientů s lékařskou diagnózou závislost na alkoholu. Výsledek potvrzuje protektivní vliv sociální opory ve vztahu ke zdraví. Nebyla prokázána existence signifi kantního vztahu mezi mírou deprese a mírou sociální opory. Výsledky výzkumu rovněž potvrzují, že existuje statisticky významný rozdíl v míře životní spokojenosti mezi skupinou kontrolní a zkoumanou (osoby se závislostí na alkoholu). Na základě výsledků výzkumu lze konstatovat, že existuje signifi kantní pozitivní vztah mezí mírou sociální opory a mírou životní spokojenosti u osob se závislostí na alkoholu. Závislost na alkoholu vede ke snížení životní spokojenosti. Závěr: Výzkum prokázal význam sociální opory jako protektivního faktoru zdraví. Potvrdilo se, že se zvyšující se mírou sociální opory se snižuje míra úzkosti a naopak se zvyšuje míra celkové životní spokojenosti u osob se závislostí na alkoholu., Aim: The article focuses on the effect of social support as a health protective factor against depression, anxiety and life satisfaction in patients treated for the diagnosis F10.2 Alcohol dependence and F32 Depression on psychiatric wards. Methods: A quantitative approach was chosen for the research. To collect data, a questionnaire battery comprising four standardised questionnaires was used: PSSS (Perceived Social Support Scale) J. A. Blumenthal, BDI-II (Beck Depression Inventory) Beck et al., SAS (Zung’s Self Rating Anxiety Scale) and Questionnaire of Life Satisfaction by Fahrenberg et al. Results: The results of the research show that there is a statistically signifi cant diff erence in the degree of social support between the control and target group (patients with medical diagnosis of depression and alcohol dependence). It confi rmed the assumption about signifi cant negative relation between the degree of anxiety and social support in patients with medical diagnosis of alcohol dependence. The existence of a signifi cant relationship between the level of depression and level of social support was not confi rmed. The research results also confi rm the existence of a statistically signifi cant diff erence in the degree of life satisfaction between the control and target group (people with alcohol dependence). Based on the results, we can claim there is a signifi cant positive relation between the degree of social support and the degree of life satisfaction in people with alcohol dependence. Alcohol dependence leads to lower life satisfaction.
