Gastrointestinální stromální tumory (GIST) jako nejčastější mezenchymální tumory trávicího traktu vycházejí z Cajalových intersticiálních buněk. Ke konfirmaci morfologické diagnózy jsme použili panel imunohistochemických protilátek CD117, desmin, S100 a DOG1. Při hodnocení biologických vlastností jsme vycházeli z Miettinenovy klasifikace pro GIST, rozdělující tumory do tří skupin podle rizika agresivního chování. Klasifikace tumoru sleduje tři parametry: lokalizaci, velikost a mitotický index. Mitotický index je definován jako počet mitóz v nádorové tkáni na celkové ploše 5 mm2. Provedli jsme srovnání metod odběru tenkojehlové, navigované aspirační cytologie – EUS FNA a klešťové endoskopické biopsie či resekátu nádorové tkáně z pohledu patologa se zaměřením na stanovení všech potřebných morfologických a prognostických parametrů této neoplázie. Naše závěry a zkušenosti ukazují, že výsledky cytologického vyšetření jsou pro diagnostiku GIST srovnatelné s bioptickým vyšetřením. Hlavní limitací cytologické metody je nemožnost v 75 % případů stanovit mitotický index z důvodů malého množství nádorové tkáně. Tento nedostatek lze nahradit vyšetřením nukleárního proteinu asociovaného s buněčnou proliferací Ki-67, který se dle literárních zdrojů ukazuje jako významný prognostický parametr progrese onemocnění., Gastrointestinal stromal tumors (GIST) belong to the most common mesenchymal tumors of digestive tract deriving from Cajal interstitial cells. A panel of immunohistochemical antibodies CD117, desmin, S100 and DOG1 is used to confirm the morphological diagnosis. When evaluating the biological characteristics we used a Miettinen classification for GIST which divides tumors into three groups according to a risk of aggressive behaviour. This classification observes three parameters – tumor localisation, its size and mitotic index. Mitotic index is defined as a number of mitoses in the tumor tissue on the overall area of 5 mm2. We performed a comparison of sampling methods – biopsy forceps, resection versus endoscopic ultrasound-guided fine-needle aspiration cytology (EUS FNA) from the point of view of a pathologist, focusing on the assessment of all necessary morphological and prognostic parameters of neoplasia. The study conclusions and experience show that when diagnosing GISTs, results of cytological examination of a sample using EUS FNA are comparable to those using forceps endoscopic biopsy. The main limitation of EUS FNA method is that in 75 % of cases it is impossible to state the mitotic index due to insufficient amount of tumor tissue. Z 99 378 nových karcinomů kolorekta (KRK) u mužů a 78 977 u žen, evidovaných v registru nádorů ČR v letech 1976–2005, bylo 14 860 (15 %) KRK u mužů a 11 142 (14,1%) u žen spojeno s výskytem jiných zhoubných novotvarů (ZN). Po 6 504 primárních KRK u mužů následovalo 7 406 ZN, po 4 342 KRK u žen 4 798 následných ZN. Naopak 8 356 následným KRK u mužů předcházelo 10 152 primárních ZN, 6 800 KRK u žen předcházelo 7 829 ZN. Počet vícečetných KRK se zvýšil ze 141 v roce 1976 na 1 534 v roce 2005. V letech 1996–2005 dosáhly primární KRK 57 % časných, 30 % pokročilých a 13 % neznámých stadií, následné KRK 43,5 %, 34 % a 22,5 %. Vícečetné KRK byly u mužů nejpočetnější ve věku 50–69 let, u žen 70–79 let. Průměrná doba vzniku následného novotvaru po primárním KRK u mužů byla 5,1 roku, u žen 5,4 roku. Po primárních KRK byly nejpočetnější ZN GIT, kůže, pohlavních orgánů a močového systému. Ze synchronních ZN byla téměř polovina nádorů trávicích cest. Z 12 204 následných ZN bylo 19 % zjištěno v pokročilých stadiích z toho asi polovina ZN trávicích cest. Víc než třetinu primárních nádorů před 17 981 následnými KRK tvořily karcinomy kůže. U 1 715 (26,4 %) mužů a 1 206 (27,8 %) žen, přežívajících v roce 2007 s primárním KRK, byl vyšší podíl časných stadií než s následným KRK; naopak u 6 646 (79,5 %) mužů a 5 291 (77,8 %) žen, zemřelých v roce 2007 s následným KRK, byl vyšší podíl pokročilých stadií než s primárním KRK. Z uvedených dat se odhaduje, že z 8 265 KRK hlášených v roce 2010 bylo téměř 1 700 (21 %) vícečetných KRK, z toho 260 primárních a 1440 následných případů. Každé páté onemocnění KRK bylo spojeno s výskytem dalšího ZN. I když je obtížné odhadnout počet vícečetných případů mezi 53 340 přežívajícími s KRK v roce 2010, jejich pokračující trend zvyšuje zátěž diagnostických a léčebných kapacit., Among the total of 99,378 new colorectal cancers (CRCs) in males and 78,977 in females, based on the Czech Cancer Registry in 1976–2005, there were 14,860 (15 %) CRCs in males and 11,142 (14.1 %) in females associated with the other malignant neoplasms (MNs). The primary 6,504 CRCs in males were followed by 7,406 subsequent MNs, the primary 4,342 CRCs in females by 4,798 subsequent MNs. On the contrary before the subsequent 8,356 CRCs in males were registered 10,152 primary MNs, 6,800 CRCs in females 7,829 MNs. The number of multiple CRCs increased from 141 in 1976 to 1,534 in 2005. In 1996–2005 the primary CRCs reached 57 % of early, 30 % advanced and 13 % unknown clinical stages, respectively the subsequent CRCs of 43.5 %, 34 % and 22.5 %. The occurrence of all multiple CRCs were highest in the age group of 50–69 years in males, and of 70–79 years in females. The average interval between primary CRCs and subsequent MNs was 5.1 years in males and 5.4 years in females. The most frequent MNs following primary CRCs were cancers of the gastrointestinal tract, skin, genital organs and urinary tract. Nearly half of synchronous MNs after primary CRCs were cancers of the gastrointestinal tract. There were 19 % cases diagnosed at advanced stages from the total 12,204 subsequent MNs, one half of those in patients with gastrointestinal cancers. More than one third of primary MNs preceded by 17,981 subsequent CRCs were cancers of the skin. Among 1,715 (26.4 %) males and 1,206 (27.8 %) females survived up to 2007 with primary CRCs, there were higher early stages than with subsequent CRSc; on the contrary among 6,646 (79.5 %) males and 5,291 (77.8 %) females, died up to 2007 with subsequent CRCs, were higher advanced stages than with primary CRCs. By the above mentioned data is estimated that among a total of 8,265 CRCs registered in 2010 were nearly 1,700 (21 %) multiple CRCs, i.e. 260 primary and 1,440 subsequent cases. Every fifth CRC was associated with the occurrence of the other MNs. Z 30 166 případů leukemií, evidovaných v českém registru nádorů v letech 1976–2005 bylo 3 937 vícečetných případů spojeno s výskytem jiných novotvarů. Z nich se 2 450 (62,2 %) vyskytlo u mužů a 1 487 (37,8 %) u žen, tj. 14,7% případů u mužů a 11 % u žen ze všech evidovaných leukemií. Po 1 715 primárních leukemiích bylo diagnostikováno 2 064 jiných novotvarů a před 2 222 následnými leukemiemi 2 642 jiných primárních nádorů. Ze 14 krajů se ve třech vyskytlo 35,5 % vícečetných leukemií (Jihomoravský a Severomoravský kraj, Praha). V průměru za 4,9 roku u mužů a 5,5 roku u žen následovalo 35,1 % nádorů kůže a melanom, 16,9 % nádorů trávicích, 13 % dýchacích a 8,4 % močových cest. Očekávaný nízký počet pokročilých klinických stadií u následných nádorů se nepotvrdil: u mužů zahrnoval 43,8 % nádorů dutiny ústní, 41,5 % dýchacích, 34,2% trávicích, 10,6 % močových cest a 15,7 % pohlavních orgánů; u žen 39,4 % dýchacích, 30,6 % močových, 30 % trávicích cest a 28,2 % pohlavních orgánů, 25 % prsů, 25 % dutiny ústní. Bohužel mnoho klinických stadií nebylo v registru nahlášeno. Před následnými leukemiemi bylo nejvíce primárních karcinomů kůže a melanomu u 43,6 % případů jako ukazatel maligního stavu. Do října 2007 přežívalo u mužů jen 12,4 % primárních a 10,3 % následných leukemií, respektive 19,7 % a 14 % u žen. Kolik dalších novotvarů se vyskytne u osmi tisíc přežívajících s leukemií, očekávaných v roce 2015? There were diagnosed 2,064 other neoplasms after 1,715 primary leukemias and 2,642 other primary neoplasms before 2,222 subsequent leukemias. A total of 14 Czech regions were distributed 35.5 % multiple leukemias in the three regions (Southern and Northern Moravia, Prague). The expected low representation of advanced clinical stages in subsequent cancers was not confirmed: in males 43.8 % cancer of oral cavity, 41.5 % respiratory, 34.2 % digestive, 10.6 % urinary and 15.7 % genital organs; in females 39.4 % respiratory, 30.6 % urinary, 30 % digestive and 28.2 % genital organs, 25 % breast, 25 % oral cavity. Unfortunatelly a lot of clinical stages were not registered. Before the subsequent leukemias the most frequent were 43.6 % primary cancers of skin and melanoma as a sign of malignant status. Up to October 2007 in males survived only 12.4 % of primary and 10.3 % subsequent leukemias, respective 19.7 % and 14 % in females. How many other neoplasms will be diagnosed among 8,000 surviving with leukemia, estimated in the Czech Republic in 2015?
