Cíl: Cílem studie bylo vyhodnotit vztah mezi hodnotami indexu IgG a počtem oligoklonálních IgG pásů (OCB) v mozkomíšním moku (CSF) pacientů s roztroušenou sklerózou (SM). Materiál a metodika: Soubor 150 pacientů sestával ze 41 mužů a 109 žen (ve věku od 18 do 68 let, průměrný věk byl 36,6 ±10,1 let). U mozkomíšního moku odebraněho lumbální punkcí byla posouzena intratekální syntéza s využitím indexu IgG a stanovením OCB. Počet alkalických OCB v CSF byl stanoven metodou izoelektrické fokusace. K vyhodnocení statistické významnosti výsledků byla rovněž použita Pearsonova korelační analýza, x2 test homogenity, Mannův-Whitneyův test, párový t-test (parametrický) a jednovýběrový Wilcoxonův test (neparametrický). Výsledky: Pozitivní korelace mezi IgG indexem a počtem OCB nebyla nalezena. Ani Mannův-Whitneyův test neprokázal podstatný rozdíl v hodnotách indexu IgG u pacientů s OCB > 2 a < 2. Závěr Studie nepotvrdila korelaci mezi hodnotami IgG indexu a počtem OCB v CSF u pacientů s roztroušenou sklerózou., Objective: The aim of this study was to assess the correlation between IgG index values and the number of the oligoclonal IgG bands (OCB) in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS). Material and Methods: The set of 150 patients consisted of 41 males and 109 females (aged 18–68, mean 36.6 ± 10.1 years). The CSF collected by a lumbar puncture was examined evaluating intrathecal synthesis using the IgG index and determining OCB. The number of alkaline OCB in the CSF was assessed using the method of isoelectric focusation. Pearson's correlation analysis, and homogeneity χ2 test , Mann-Whitney test, paired-sample t-test (parametric) and Wilcoxon signed-ranks test (nonparametric) were used to evaluate the statistical significance of the results. Results: No positive correlation between the IgG index and the number of OCB was found. Mann-Whitney test also did not demonstrate any significant difference of the IgG index values in patients both with the OCB number ≥ 2 and < 2. Úvod: D-dimery jsou jedním se základních laboratorních ukazatelů aktivity fibrinolytického systému. Cílem naší prospektivní studie bylo zjistit, zda v akutní fázi ischemické CMP lze detekovat zvýšení hladiny D-dimerů a jejich závislost na typu, etiopatogenezi, místě okluze a rizikových faktorech. Materiál a metodika: Během 12 měsíců bylo do studie konsekutivně zařazeno 165 pacientů s akutní centrální neurologickou symptomatikou, kteří byli přijati na neurologickou kliniku do 6 hodin od vzniku příznaků. U 143 pacientů byla diagnostikována cévní mozková příhoda (CMP). U všech pacientů byla ihned při přijetí provedena vyšetření neurologické, CT mozku a laboratorní (krevní obraz, biochemie a koagulační vyšetření, včetně hladiny D-dimerů). U všech pacientů bylo provedeno neurosonologické vyšetření, eventuálně CT angiografické vyšetření při příjmu pacienta, u pacientů s ischemickou CMP také za 6, 24 a 72 hodin od začátku příznaků. Výsledky: Vstupní hodnota D-dimerů byla signifikantně vyšší u pacientů s okluzí kmene tepny Willisova okruhu v porovnání s pacienty s okluzí drobných větví mozkových tepen, okluzí krční tepny či pacienty bez detekovatelné okluze tepny, u pacientů s ischemickou chorobou srdeční, fibrilací síní a mozkových infarktů kardioembolické etiologie (p < 0,05). Hladina D-dimerů a fibrinogenu se nelišila signifikantně mezi pacienty s aterotrombotickými a lakunárními infarkty, TIA a hemoragickou CMP ve srovnání s pacienty s jinou etiologií akutního neurologického deficitu (p > 0,05). Vstupní hladina D-dimerů a fibrinogenu nebyly v korelaci s časem rekanalizace mozkové tepny. Závěr: U pacientů s ischemickou CMP kardioembolické etiologie lze detekovat signifikantně vyšší hladinu D-dimerů, ale pro velkou interindividuální variabilitu má jednorázové vyšetření v klinické praxi velmi omezený význam., Introduction: D-dimers belong among the basic laboratory indicators of the activity of the fibrinolytic system. The objective of the prospective study was to find out whether an increase in the level of D-dimers in the acute phase of an ischaemic stroke and their dependence on the type, ethiopathogenesis, place of occlusion and risk factors can be detected. Material and methods: 165 patients with acute central neurological symptomatology were consecutively included in the study in the course of 12 months, all of them having been admitted to the clinic within 6 hours after the occurrence of symptoms. 143 patients were diagnosed with cerebrovascular accident (CVA). A neurological examination, a brain CT and laboratory analyses (blood count, biochemical and coagulation examination including D-dimer levels) were performed in all patients upon admission. All patients underwent a neurosonological examination or a CT angiography upon admission, which were also performed within 6, 24 and 72 hours from the occurrence of symptoms in the patients who had suffered an ischaemic cerebrovascular event. Results: The input value of D-dimers was significantly higher in patients with trunk occlusion of an artery of the circle of Willis than in patients with occlusion of minor branches of cerebral arteries, occlusion of the carotid artery or in patients without detectable arterial occlusion, and in patients with ischaemic heart disease, chamber fibrillation and brain infarctions of cardioembolic etiology (p < 0.05). There was no significant difference in terms of levels of D-dimers and fibrinogen between patients with a atherothrombotic and lacunar infarction, a TIA or a haemorrhagic CVA and those with a different etiology of an acute neurological deficit (p > 0.05). The input levels of D-dimers and fibrinogen were not in correlation with the time of recanalisation of the cerebral artery. Conclusion: Significantly higher levels of D-dimers can be detected in patients with an ischaemic CVE of cardioembolic etiology, but single examination is of limited value in clinical practice due to large interindividual variablilty., David Školoudík, M. Úvod: Kardiochirurgické výkony jsou zatíženy signifikantním rizikem vzniku ischemické cévní mozkové příhody (CMP), které se dle jednotlivých studií pohybuje mezi 1,3 a 3,6 %. Asymptomatické ischemické infarkty lze detekovat po kardiochirurgické operaci dokonce až u 32 % pacientů. Cílem studie je prokázat efekt sonolýzy (kontinuálního transkraniálního dopplerovského (TCD) monitoringu a. cerebri media (ACM)) u pacientů podstupujících kardiochirurgický výkon na snížení rizika vzniku nových ischemických lézí detekovaných pomocí magnetické rezonance (MR). Soubor a metodika: Do studie jsou zařazováni pacienti indikovaní k izolovanému aortokoronárnímu bypassu nebo k izolovanému výkonu na jedné srdeční chlopni. Pacienti jsou randomizováni do skupiny se sonolýzou (monitorování pomocí TCD) a kontrolní skupiny bez sonolýzy. U všech pacientů byla provedena MR mozku před výkonem a 24 hod po něm a byla hodnocena četnost výskytu, lokalizace a velikost nových ischemických změn po výkonu. Výsledky: Během 16 měsíců bylo zařazeno 78 pacientů (48 mužů, průměrný věk 63,7 ? 15,8 let), 35 pacientů bylo randomizováno do skupiny se sonolýzou, 43 pacientů do kontrolní skupiny. Ve skupině sonolýzy byly detekovány nové ischemické změny u osmi pacientů (23 %) – u pěti (14 %) v povodí pravé (stimulované) ACM, ale jen u dvou (6 %) větší než 0,5 cm3. V kontrolní skupině byly detekovány nové ischemické změny u 10 pacientů (23 %) – u osmi (19 %) v povodí pravé ACM, u pěti (12 %) větší než 0,5 cm3 (p > 0,05 ve všech případech). Závěr: Pilotní data studie prokázala trend k redukci počtu i objemu ischemických lézí mozku u pacientů se sonolýzou v průběhu kardiochirurgické operace., Background: Heart surgery is burdened with a significant risk of ischemic stroke. Asymptomatic cerebral infarctions can be detected in as many as 32% of patients after heart surgery. The aim of this study is to prove the efficacy of sonolysis (continual transcranial Doppler (TCD) monitoring) during open-heart surgery to decrease the risk of new brain infarctions detected by magnetic resonance imaging (MRI). Material and methods: Patients indicated to isolated coronary artery bypass or isolated one heart valve surgery were included in the study. Patients were randomized to a sonolysis group (TCD monitoring), and to a control group without sonolysis. All patients underwent brain MRI before and 24 hours after the surgery and the presence of new ischemic lesions was evaluated. Results: During 16 months, 78 patients (48 males, mean age 63.7 ? 15.8 years) were enrolled to the study. Thirty five patients were randomized to the sonolysis group and 43 to the control group. In the sonolysis group, new brain infarctions were found in eight (23%) patients – in the right (TCD-monitored) MCA territory in five patients (14%) but > 0.5 cm3 in two (6%) patients only. In the control group, new infarctions were found in 10 patients (23%) – in the right (TCD-monitored) MCA in eight (19%) patients and > 0.5 cm3 in five (12 %) (p > 0.05 in all cases). Conclusion: Pilot study results showed a trend towards reduction in a number and volume of new brain ischemic lesions in patients treated with sonolysis during heart surgery.
