Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Limited financial resources and persistent risks of lifestyle do not allow to slow down the increase of primary and subsequent neoplasms in the near future among cancer survivors not only in gynaecology., Edvard Geryk, Radim Štampach, Viera Bajčiová, Teodor Horváth, and Literatura
Z 99 378 nových karcinomů kolorekta (KRK) u mužů a 78 977 u žen, evidovaných v registru nádorů ČR v letech 1976–2005, bylo 14 860 (15 %) KRK u mužů a 11 142 (14,1%) u žen spojeno s výskytem jiných zhoubných novotvarů (ZN). Po 6 504 primárních KRK u mužů následovalo 7 406 ZN, po 4 342 KRK u žen 4 798 následných ZN. Naopak 8 356 následným KRK u mužů předcházelo 10 152 primárních ZN, 6 800 KRK u žen předcházelo 7 829 ZN. Počet vícečetných KRK se zvýšil ze 141 v roce 1976 na 1 534 v roce 2005. V letech 1996–2005 dosáhly primární KRK 57 % časných, 30 % pokročilých a 13 % neznámých stadií, následné KRK 43,5 %, 34 % a 22,5 %. Among the total of 99,378 new colorectal cancers (CRCs) in males and 78,977 in females, based on the Czech Cancer Registry in 1976–2005, there were 14,860 (15 %) CRCs in males and 11,142 (14.1 %) in females associated with the other malignant neoplasms (MNs). The primary 6,504 CRCs in males were followed by 7,406 subsequent MNs, the primary 4,342 CRCs in females by 4,798 subsequent MNs. On the contrary before the subsequent 8,356 CRCs in males were registered 10,152 primary MNs, 6,800 CRCs in females 7,829 MNs. The number of multiple CRCs increased from 141 in 1976 to 1,534 in 2005. In 1996–2005 the primary CRCs reached 57 % of early, 30 % advanced and 13 % unknown clinical stages, respectively the subsequent CRCs of 43.5 %, 34 % and 22.5 %. The occurrence of all multiple CRCs were highest in the age group of 50–69 years in males, and of 70–79 years in females. The average interval between primary CRCs and subsequent MNs was 5.1 years in males and 5.4 years in females. The most frequent MNs following primary CRCs were cancers of the gastrointestinal tract, skin, genital organs and urinary tract. Nearly half of synchronous MNs after primary CRCs were cancers of the gastrointestinal tract. There were 19 % cases diagnosed at advanced stages from the total 12,204 subsequent MNs, one half of those in patients with gastrointestinal cancers. More than one third of primary MNs preceded by 17,981 subsequent CRCs were cancers of the skin. Among 1,715 (26.4 %) males and 1,206 (27.8 %) females survived up to 2007 with primary CRCs, there were higher early stages than with subsequent CRSc; on the contrary among 6,646 (79.5 %) males and 5,291 (77.8 %) females, died up to 2007 with subsequent CRCs, were higher advanced stages than with primary CRCs. By the above mentioned data is estimated that among a total of 8,265 CRCs registered in 2010 were nearly 1,700 (21 %) multiple CRCs, i.e. 260 primary and 1,440 subsequent cases. Every fifth CRC was associated with the occurrence of the other MNs. Although it is difficult to estimate the number of multiple cases among 53,340 survivors with CRCs in 2010, their ongoing trend will increase the burden of diagnostic and therapeutic capabilities., and Edvard Geryk, Petr Dítě, Radim Štampach, Teodor Horváth, Jiří Kozel
