Úvod: Incidence kolorektálního karcinomu narůstá. Nejlepší léčebné výsledky má multimodální léčba, jejíž základní součástí je snaha o R0 resekci všech nádorových ložisek. Materiál a metodika: Byla provedena literární rešerše v souvislosti z vlastními zkušenostmi. Výsledky: Spektrum chirurgických zákroků zahrnuje endoskopickou submukózní resekci, transanální endoskopickou mikrochirurgii, laparoskopické resekce, otevřené resekce a cytoredukční chirurgii s peritonektomií a hypertermickou intraperitoneální chemoterapií. Nově se kromě konceptu totální mezorektální excize začíná uplatňovat také koncept totální mezokolické excize, který vede k nižšímu počtu lokálních recidiv a zvýšenému přežívání. Závěr: Stejně jako protinádorová farmakoterapie se významně stále vyvíjí i chirurgická léčba. K určení správné pozice jednotlivých chirurgických postupů v multimodálním algoritmu léčby je nutné sledování pacientů ve vhodných databázích, případně je léčit v rámci studií., Background: The incidence of colorectal carcinoma has been increasing. Only a multimodal treatment provides the best results. Surgical R0 resection of all tumour sites is the goal of all treatment regimes. Material and methods: A survey on the literature and our experiences has been conducted. Results: Surgical treatment comprises endoscopic submucosal dissection, transanal endoscopic microsurgery, laparoscopy, open surgery and cytoreduction surgery with the hyperthermic intraperitoneal chemotherapy. The news is the total mesocolic excision that similarly to the total mesorectal excision provides fewer local relapses and higher long-term survival. Chirurgická léčba Crohnovy choroby je významnou součástí celkového léčebného algoritmu. Postavení chirurgie se mění s rozvojem konzervativních postupů. V posledních letech je návrat k časné léčbě pacientů s Crohnovou chorobou. Vzhledem k charakteru onemocnění a střevního postižení je vyžadován specifický přístup k těmto pacientům zejména ve správné volbě operačního přístupu. Článek je zaměřen na luminální postižení tenkého a tlustého střeva včetně komplikací choroby. Popisujeme jednotlivé indikace operačního řešení včetně volby anastomózy či vícedobého operačního řešení., Surgery of Crohn’s disease is an important part of the general treatment algorithm. The role of surgery is changing with the development of conservative procedures. The recent years have seen the return to early treatment of patients with Crohn’s disease. Given the character of the disease and its intestinal symptoms, a specific approach to these patients is necessary, especially regarding the correct choice of surgery. The paper focuses on the luminal damage of the small and large intestine including complications of the disease. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet.
