Úvod: Karcinom prsu je nejrozšířenějším zhoubným nádorem u žen. Nejčastěji se toto onemocnění vyskytuje u žen mezi 60. a 70. rokem života, nicméně v posledních letech dochází k nárůstu počtu pacientek i v mladších věkových skupinách. Incidence karcinomu prsu má celosvětově vzestupný charakter, zejména ve Spojených státech a západní Evropě. Jeho mortalita má však zejména díky screeningovému programu a pokročilým léčebným metodám spíše mírně sestupnou tendenci. Metody: Do souboru byly zařazeny pacientky s karcinomem prsu, které podstoupily operaci pro tuto diagnózu na I. chirurgické klinice FN Olomouc v letech 2008– 2012. U každé pacientky byl vypočítán věk v době stanovení diagnózy, respektive v době operace. Pacientky byly v každém roce rozděleny do věkových skupin po 10 letech. Současně byla v každém roce vytvořena podskupina pacientek s karcinomem prsu mladších 45 let. Byl porovnán počet pacientek v odpovídajících skupinách, resp. podskupinách, v jednotlivých letech. V každém roce byl rovněž stanoven průměrný věk a medián pacientek. Ke zhodnocení přínosu mamárního screeningu bylo porovnáno zastoupení stadií operovaných tumorů v jednotlivých sledovaných letech. Výsledky byly statisticky zpracovány a vyhodnoceny. Výsledky: V letech 2008–2012 bylo operováno na I. chirurgické klinice FN Olomouc celkem 980 pacientek ve věku od 20 do 88 let, z toho 101 pacientek mladších 45 let. Průměrný věk pacientek byl 59 let a medián 60 let. V jednotlivých letech se celkový počet pacientek zvýšil ze 153 v roce 2008 až na 240 v roce 2012. V podskupině pacientek do 45 let nedošlo ve sledovaných letech k signifikantnímu nárůstu počtu nemocných. V roce 2012 bylo diagnostikováno více pacientek ve stadiu II a méně pacientek ve stadiu III v porovnání s rokem 2010. Závěr: Analýzou našeho souboru byla potvrzena vzestupná tendence ve výskytu karcinomu prsu. Nebyl prokázán statisticky významný nárůst počtu pacientek v tzv. předscreeningovém věku, tedy mladších 45 let. Zároveň byl zjištěn častější záchyt časnějších stadií onemocnění, zejména v posledních třech sledovaných letech., Introduction: Breast cancer is the most common malignancy in women. It affects mostly women between the ages of 60 and 70; however, in the past years, the number of younger female patients has been increasing. The incidence of breast cancer has been rising worldwide, especially in the United States and Western Europe. Breast carcinoma mortality, on the other hand, has shown a slight decrease due to early screening programmes and advanced treatment methods. Methods: We included patients who had undergone surgery for breast carcinoma in the 1st Department of Surgery at Teaching Hospital in Olomouc between 1 January 2008 and 31 December 2012. In each patient, her age at the time of diagnosis/surgery was calculated. All patients were divided into 10-year age groups. At the same time, a sub-group of breast cancer patients younger than 45 years was created. We compared the numbers of patients in the respective groups and sub-groups in every year. The mean age and the median of age were also calculated. To evaluate the benefit of mammary screening, we compared the staging of operated tumours in the different years studied. The results were statistically processed and evaluated. Results: The total number of 980 patients underwent surgery for breast carcinoma at our department between 2008 and 2012 with age ranging from 20 to 88 years. 101 of them were younger than 45 years. The mean age of the patients was 59 years, the median was 60 years. The total number of patients increased from 153 in 2008 to 240 in 2012. There was no significant increase in the number of patients younger than 45 years. There were more patients diagnosed with stage II carcinoma and fewer patients with stage III carcinoma in 2012 than in 2010. Conclusion: The analysis of our group of patients confirmed the increasing tendencies of breast cancer incidence in total. We did not prove a statistically significant increase in the number of patients in pre-screening age (i.e., younger than 45 years). Úvod: Adenokarcinom pankreatu (PDAC) je jedním z nejagresivnějších maligních onemocnění. Jeho špatná prognóza je dána kombinací různých faktorů, mezi něž patří agresivní biologie onemocnění, nespecifické či chybějící příznaky v časných stadiích, jejich nedocenění, protrahovaná diagnostika a zpoždění v zahájení léčby. Většina pacientů je diagnostikována již v pokročilém stadiu onemocnění. Medián přežití u těchto pacientů se pohybuje mezi 2 a 11 měsíci. Nejčastějšími důsledky lokálně pokročilého onemocnění, které si vynutí intervenci, jsou obstrukce žlučových cest a duodena. Cílem práce byla analýza přežívání pacientů s radikálně inoperabilním PDAC, kteří podstoupili paliativní chirurgický výkon či exploraci s biopsií, stanovení vlivu faktorů pacienta, nádoru a léčebné modality na délku přežití. Metody: Jedná se o retrospektivní analýzu souboru pacientů s inoperabilním PDAC, kteří podstoupili neradikální chirurgickou intervenci v letech 2006–2014. Předmětem analýzy byly věk, histopatologické nálezy, chirurgická a onkologická léčba a doba přežití. Výsledky byly statisticky zpracovány a vyhodnoceny pomocí softwaru IBM SPSS Statistics verze 22 (USA). Výsledky: Soubor tvořilo celkem 184 pacientů (105 mužů, 79 žen) s radikálně inoperabilním PDAC. Průměrný věk byl 64 let a převažovali pacienti s onemocněním ve stadiu IV. Průměrná délka přežití byla 7,04 měsíce, medián 4,7 měsíce. Závěr: Byl zjištěn statisticky signifikantní vliv pohlaví, stadia onemocnění, přítomnosti vzdálených metastáz v době operace a podání onkologické léčby na přežívání těchto pacientů. Průměrná délka i medián přežití pacientů s radikálně inoperabilním tumorem v našem souboru koresponduje s výsledky podobných studií., Introduction: Pancreatic cancer (PDAC) is one of the most aggressive malignancies. Its poor prognosis is due to a combination of various factors, mainly aggressive biology of the tumour, non-specific symptoms in early stages, their underestimation, prolonged time to diagnosis and late onset of treatment. The majority of patients are diagnosed in an advanced stage of the disease. Median survival of these patients ranges from 2−11 months. The most common consequences of locally advanced disease that require intervention include obstruction of the duodenum and biliary obstruction. The purpose of our study was to analyze the survival of patients with radically inoperable PDAC undergoing palliative surgery or exploration with biopsy, and to evaluate the influence of patient and tumour factors and treatment modalities on survival. Methods: In our retrospective study we included all patients with radically inoperable PDAC undergoing a non-radical surgical intervention between 01 January 2006 and 31 December 2014. Patient age, histopathological findings, surgical and oncological treatment and survival were included in the analysis. The results were statistically processed and evaluated using IBM SPSS Statistics software version 22 (USA). Results: 184 patients with radically inoperable PDAC, 105 males and 79 females, were included in our study. Mean age of the patients was 64 years and most patients presented with stage IV of the disease. Mean survival time was 7.04 months and median 4.7 months. Conclusion: We determined a statistically significant influence of the following factors on patient survival: sex, stage, presence of distant metastases at the time of surgery and oncological treatment administration. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases.
