Úvod: Karcinom prsu je nejrozšířenějším zhoubným nádorem u žen. Nejčastěji se toto onemocnění vyskytuje u žen mezi 60. a 70. rokem života, nicméně v posledních letech dochází k nárůstu počtu pacientek i v mladších věkových skupinách. Incidence karcinomu prsu má celosvětově vzestupný charakter, zejména ve Spojených státech a západní Evropě. Jeho mortalita má však zejména díky screeningovému programu a pokročilým léčebným metodám spíše mírně sestupnou tendenci. Metody: Do souboru byly zařazeny pacientky s karcinomem prsu, které podstoupily operaci pro tuto diagnózu na I. chirurgické klinice FN Olomouc v letech 2008– 2012. U každé pacientky byl vypočítán věk v době stanovení diagnózy, respektive v době operace. Pacientky byly v každém roce rozděleny do věkových skupin po 10 letech. Současně byla v každém roce vytvořena podskupina pacientek s karcinomem prsu mladších 45 let. Byl porovnán počet pacientek v odpovídajících skupinách, resp. podskupinách, v jednotlivých letech. V každém roce byl rovněž stanoven průměrný věk a medián pacientek. Ke zhodnocení přínosu mamárního screeningu bylo porovnáno zastoupení stadií operovaných tumorů v jednotlivých sledovaných letech. Výsledky byly statisticky zpracovány a vyhodnoceny. Výsledky: V letech 2008–2012 bylo operováno na I. chirurgické klinice FN Olomouc celkem 980 pacientek ve věku od 20 do 88 let, z toho 101 pacientek mladších 45 let. Průměrný věk pacientek byl 59 let a medián 60 let. V jednotlivých letech se celkový počet pacientek zvýšil ze 153 v roce 2008 až na 240 v roce 2012. V podskupině pacientek do 45 let nedošlo ve sledovaných letech k signifikantnímu nárůstu počtu nemocných. V roce 2012 bylo diagnostikováno více pacientek ve stadiu II a méně pacientek ve stadiu III v porovnání s rokem 2010. Závěr: Analýzou našeho souboru byla potvrzena vzestupná tendence ve výskytu karcinomu prsu. Nebyl prokázán statisticky významný nárůst počtu pacientek v tzv. předscreeningovém věku, tedy mladších 45 let. Zároveň byl zjištěn častější záchyt časnějších stadií onemocnění, zejména v posledních třech sledovaných letech., Introduction: Breast cancer is the most common malignancy in women. It affects mostly women between the ages of 60 and 70; however, in the past years, the number of younger female patients has been increasing. The incidence of breast cancer has been rising worldwide, especially in the United States and Western Europe. Breast carcinoma mortality, on the other hand, has shown a slight decrease due to early screening programmes and advanced treatment methods. Methods: We included patients who had undergone surgery for breast carcinoma in the 1st Department of Surgery at Teaching Hospital in Olomouc between 1 January 2008 and 31 December 2012. In each patient, her age at the time of diagnosis/surgery was calculated. All patients were divided into 10-year age groups. At the same time, a sub-group of breast cancer patients younger than 45 years was created. We compared the numbers of patients in the respective groups and sub-groups in every year. The mean age and the median of age were also calculated. To evaluate the benefit of mammary screening, we compared the staging of operated tumours in the different years studied. The results were statistically processed and evaluated. Results: The total number of 980 patients underwent surgery for breast carcinoma at our department between 2008 and 2012 with age ranging from 20 to 88 years. 101 of them were younger than 45 years. The mean age of the patients was 59 years, the median was 60 years. The total number of patients increased from 153 in 2008 to 240 in 2012. There was no significant increase in the number of patients younger than 45 years. There were more patients diagnosed with stage II carcinoma and fewer patients with stage III carcinoma in 2012 than in 2010. Conclusion: The analysis of our group of patients confirmed the increasing tendencies of breast cancer incidence in total. We did not prove a statistically significant increase in the number of patients in pre-screening age (i.e., younger than 45 years). Úvod: Cílem práce je identifikovat pacienty operované pro vývodový karcinom pankreatu, kteří přežili více než 5 let a analyzovat jejich charakteristiky a faktory s vlivem na přežití. Zkušenosti a výsledky terciárního centra. Metoda: Prospektivně vedená databáze 155 pankreatických resekcí v období leden 2006 až červen 2010 byla prozkoumána se záměrem identifikace pacientů operovaných kurativně pro vývodový karcinom pankreatu. Klinická a patologická data byla analyzována retrospektivně. Kaplan-Meierova analýza byla použita pro hodnocení přežívání, Log-rank test a Coxova regresní analýza pro analýzu faktorů s vlivem na přežití. Testy byly dělány na hladině signifikance 0,05. Jako nezávisle proměnné v Coxově regresní analýze byly vybrány faktory, u nichž byl pomocí Kaplan-Meierovy analýzy prokázán signifikantní vliv na přežívání. Výsledky: V sestavě 155 resekcí pankreatu s kurativním záměrem bylo identifikováno 73 nemocných operovaných pro vývodový karcinom pankreatu, u nichž bylo možné hodnotit 5leté přežití. Více než 5 let přežilo 15 nemocných (20,5 %), z nichž 13 dosud žije. Průměrná doba přežití ve skupině přeživších 5 let je 77,1 měsíce (60–110) a medián 74 měsíce, průměrná doba přežití bez nemoci (relaps free survival) je 63,3 měsíce (14−110), medián 62 měsíce. Ze sledovaných faktorů statisticky významný vliv na přežití měly: nepřítomnost infiltrace lymfatických uzlin (p=0,031), nekomplikovaný pooperační průběh (p=0,025), nepřítomnost vaskulární invaze v tumoru (p=0,017), léčba bez nutnosti podání transfuze krve (p=0,015) a pooperační onkologické léčba – převážně chemoterapie (p=0,009). Významnými nezávislými prediktory přežívání jsou: přítomnost angioinvaze HR=2,239 (95%CI: 1,093−4,590; p=0,028), pooperační chemoterapie HR=2,587 (95%CI: 1,301−5,145; p=0,007) a nutnost podání transfuze HR=2,080 (95%CI: 1,027−4,212; p=0,042). Přítomnost angioinvaze zvyšuje riziko úmrtí pacienta 2,2x, podání transfuze zvyšuje riziko úmrtí 2,1x a nepodání chemoterapie zvyšuje riziko úmrtí 2,6x. Závěr: Vliv na dlouhodobé přežití u nemocných operovaných pro karcinom hlavy pankreatu byl prokázán u těchto faktorů: nepřítomnost infiltrace lymfatických uzlin, nekomplikovaný pooperační průběh, nepřítomnost vaskulární invaze v tumoru, léčba bez nutnosti podání transfuze krve a pooperační onkologická léčba – chemoterapie. Významnými nezávislými prediktory přežívání jsou: vaskulární invaze, chemoterapie a nutnost podání transfuze., Introduction: The purpose was to identify 5-year survivors among a group of radically resected patients with pancreatic cancer and analyse the characteristics and factors associated with their 5-year survival. Single tertiary centre experience. Method: A prospectively maintained database of 155 pancreatic resections from January 2006 to June 2010 was scanned to identify patients after curative radical resections for pancreatic ductal adenocarcinoma. The clinical and pathological data was analysed retrospectively. The outcomes of the PDAC group were evaluated using Kaplan-Meier analysis (survival) with the Log-rank test and Cox regression analysis (evaluation of prognostic factors). Characteristics of the survivors were discussed. Significance level of 0.05 was used. Those factors were used as independent variables for Cox regression analysis whose significant effect on survival was shown based on Kaplan-Meier analysis. Results: Among 155 patients undergoing a curative pancreatic resection, 73 had a pancreatic ductal adenocarcinoma. Fifteen patients (20.5%) after radical surgery survived over 5 years, 13 of whom are still alive. In the group of the survivors, the mean overall survival was 77.1 months (60−110) and the median survival was 74 months. The mean relapse-free interval in the group of the survivors was 63.3 months (14−110) with the median of 65 months. Factors associated with a longer survival included the absence of lymph node infiltration (p=0.031), uncomplicated postoperative course (p=0.025), absence of vascular invasion (p=0.017), no blood transfusions (p=0.015) and the use of postoperative therapy – predominantly chemotherapy (p=0.009). Significant independent predictors of survival included vascular invasion HR=2.239 (95%CI: 1.093−4.590; p=0.028), postoperative chemotherapy HR=2.587 (95%CI: 1.301−5.145; p=0.007) and blood transfusion HR=2.080 (95%CI: 1.027−4.212; p=0.042). The risk of death was increased 2.2 times in patients with vascular invasion, 2.1 times in patients with transfusions, and finally 2.6 times in those with no chemotherapy. Conclusion: Factors associated with an improved overall survival included: the absence of lymph node infiltration, an uncomplicated postoperative course, absence of vascular invasion, no need of blood transfusions, and finally the use of postoperative chemotherapy. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases.
