Úvod: Tumory thalamu jsou relativně vzácné léze rostoucí ve vysoce funkčně důležité části mozku. Jsou častější v pediatrické populaci. Jejich chirurgické odstranění je náročné s rizikem značné morbidity. Relativně benigní charakter mnoha těchto lézí vybízí k pokusu o jejich radikální odstranění. Chirurgický přístup musí být velice pečlivě plánován, obvykle s použitím moderních diagnostických metod, jako je DTI (Diffusion Tensor Imaging). Lokalizace a projekce tumoru v thalamu hraje při výběru přístupu důležitou roli. Materiál a metodika: Od roku 2005 do roku 2014 bylo na Neurochirurgické klinice FN Brno operováno celkem 13 pacientů s tumorem thalamu, průměrný věk 20,6 roku (1–64 let). Jednalo se o 10 pacientů mužského a tři ženského pohlaví. Další 15letý pacient s pilocytárním astrocytomem je observován. Transkortikální přístup byl použit 7×, transkalózní přístup 4×, transsylvijský přístup 2× a supracerebellární infratentoriální 1×. Výsledky: Totální resekce (GTR) byla provedena v šesti případech, subtotální resekce ve čtyřech případech a parciální resekce ve třech případech. Jednalo se o osm pilocytárních astrocytomů, jeden subependymální obrovskobuněčný astrocytom, jeden difúzní astrocytom GII a tři glioblastomy. Dva starší pacienti s glioblastomem zemřeli šest a devět měsíců po operaci. Ostatní pacienti přežívají se střední délkou sledování 5 let. Neurologický stav po operaci byl dobrý kromě jednoho staršího pacienta s glioblastomem, u kterého se rozvinula těžká hemiparéza a fatická porucha. Závěr: Pokud je zvolen správný chirurgický přístup, thalamické tumory mohou být celkem bezpečně radikálně odstraněny. Volba přístupu je dána projekcí tumoru. Menší tumory, které nedosahují povrchu thalamu, mohou být observovány nebo bioptovány, pokud máme podezření na maligní povahu léze. Následná onkologická léčba je vyhrazena pro vysokostupňové gliomy., Introduction: Thalamic tumours are relatively rare lesions growing in a highly functional part of the brain. They are more frequent in paediatric population. Their surgery is challenging and associated with a risk of significant morbidity. Relatively benign nature of many of these tumours suggests that radical resection should be attempted. The approach has to be very carefully planned, sometimes with help of modern diagnostic methods such as DTI. Location and projection of the tumour in the thalamus plays an important role in selecting an approach. Material and methods: During 2005–2014 we operated on 13 patients with thalamic tumours, mean age 20.6 years (1–64 years). These were 10 males and three females. Another 15-year-old male patient is being observed. Transcortical approach was used 7×, transcallosal 4×, transsylvian 2× and supracerebellar infratentorial 1×. Results: Gross total resection was achieved in six cases, subtotal in four and partial in three. There were eight pilocytic astrocytomas, one subependymal giant cell astrocytoma, one diffuse astrocytoma G II and three glioblastomas. Two older patients with glioblastomas died six and nine month after operation. All other patiens are still alive with the mean follow-up 5 years. Neurological status after the operation was good except for one older patient with glioblastoma who developed severe hemiparesis and aphasia. Conclusion: Thalamic tumours might be safely radically resected if correct approach is used. The choice of approach is based on the projection of the tumour. Smaller tumours that are not reach thalamic surface might be followed up or biopsied if there is a likelihood of their malignant nature. Oncological treatment should be reserved to malignant tumours. Cíl: Na kohortě pacientů s asymptomatickou stenózou vnitřní karotidy podstupujících karotickou endarterektomii otestovat protokol vyšetření hemodynamiky mozku pomocí magnetické rezonance a zjišťovat rozdíl v kognitivní kondici pomocí neuropsychologické testové baterie RBANS (Repeatable Battery for the Assessment of Neuropsychological Status). Metody: Standardní karotická endarterektomie pro asymptomatickou stenózu vnitřní karotidy v intravenózní celkové anestezii se selektivním užitím shuntu. Měření průtoku velkými cévami Willisova okruhu pomocí magnetickorezonační angiografie s fázovým kontrastem před operací a tři měsíce po operaci, a to paralelně s psychologickým vyšetřením testovou baterií RBANS. Výsledky: U pěti žen a 12 mužů s nekomplikovanou karotickou endarterektomií pro asymptomatickou stenózu vnitřní karotidy bylo zaznamenáno signifikantní zlepšení v oblasti kognitivního výkonu v celkovém skóru neuropsychologické baterie (p = 0,05) a indexech týkajících se bezprostřední paměti (p = 0,04) a řeči (p = 0,01). Následné korelace lokální změny hemodynamiky a změn v kognitivním výkonu z magnetickorezonančních dat získaných od 12 pacientů byly signifikantní pro přední mozkovou tepnu a index bezprostřední paměti (p = 0,01), a pro střední mozkovou tepnu a indexy řeči (p = 0,02) a vizuoprostorové orientace (p = 0,02). Závěr: V pilotní studii byl na malé skupině pacientů s klinicky i radiologicky asymptomatickou stenózou vnitřní karotidy poprvé v naší literatuře otestován protokol vyšetření průtoku krve (ml/min) na všech velkých cévách Willisova okruhu a srovnány změny hemodynamiky před karotickou endarterektomií a tři měsíce po ní. V úrovni kognitivního výkonu bylo prokázáno zlepšení, u kterého byl v dílčích testech neuropsychologické baterie RBANS identifikován vztah s hodnotami průtoků v povodí přední nebo střední mozkové tepny. Větší studie s delším sledováním výsledků je proveditelná., Aim: To test a brain haemodynamics examination protocol using magnetic resonance imaging and to investigate the differences in cognitive performance with a neuropsychology test battery RBANS (Repeatable Battery for the Assessment of Neuropsychological Status) on a cohort of patients with an asymptomatic carotid stenosis undergoing carotid endarterectomy. Methods: Standard carotid endarterectomy in asymptomatic carotid stenosis under general intravenous anaestehesia with selective use of a shunt. Flow measurement in major vessels of the Willis circle using magnetic resonance angiography with phase contrast before and three months after the operation and concurrently with RBANS psychological examination testing battery. Results: Significant cognitive performance improvement in neuropsychological test battery total score and in short-term memory (p = 0.04) and speech (p = 0.01) indexes was seen in five women and 12 men undergoing uncomplicated carotid endarterectomy. Subsequent correlations of local haemodynamic change and cognitive performance change in 12 patients were significant for the anterior cerebral artery and immediate memory index (p = 0.01), and for the medial cerebral artery and speech indeces (p = 0.02) and visuospatial orientation (p = 0.02). Conclusion: In a pilot study, a flow (ml/min) examination protocol of all major Willis circle arteries was tested and haemodynamic changes before the operation and in three months follow-up after carotid endarterectomy were compared on a small group of patients with radiologically and clinically asymptomatic carotid stenosis for the first time in Czech literature. Improvement in cognitive performance was demonstrated and an association with anterior or medial cerebral artery flow values was identified in individual RBANS tests. A larger study with a longer follow-up is feasible. Cíl: Od roku 2002 lze v ČR využít evokované potenciály (EP) k potvrzení smrti mozku (SM). V článku informujeme o užitečnosti kombinace BAEP (sluchových kmenových evokovaných potenciálů) a SEP (somatosenzorických evokovaných potenciálů) jakož i opakovaného vyšetření, jež zvyšují senzitivitu průkazu SM. Metodika: Na Neurochirurgickém oddělení FN Plzeň bylo v letech 2002–2012 u 41 pacientů provedeno potvrzení SM pomocí EP. Všichni pacienti splňovali klinická kritéria SM a podstoupili potvrzení SM pomocí BAEP a SEP n. medianus. Výsledky: Senzitivita prvního vyšetření k průkazu SM byla u BAEP 37 %. Opakováním vyšetření BAEP pak došlo k potvrzení SM se senzitivitou 100 %. Kombinace vyšetření BAEP a SEP zvýšila senzitivitu průkazu SM na 75 %. Senzitivita SEP pro průkaz SM byla při prvním vyšetření 70 %, senzitivita druhého vyšetření dosáhla 100 %. Závěr: Kombinace BAEP a SEP a opakování vyšetření zvyšuje senzitivitu průkazu SM u pacientů s klinicky prokázanou SM., Aim: Since 2002, evoked potentials (EP) can be used to confirm brain death (BD) in Czech Republic. The aim of this study was to show an improvement in reliability of the diagnosis of brain death (BD) using serial recordings and a combination of auditory brain stem potentials (BAEPs) and median nerve somatosensory evoked potentials (SEPs). Methods: From 2002 to 2012, 41 patients had BAEP and SEP performed to confirm BD at the Department of Neurosurgery, University Hospital Pilsen. Results: There was a sensitivity of 37% at the first BEAPs registration to confirm BD. Serial BAEP recordings increased BD confirmation to 100%. BAEPs and SEPs combination increased sensitivity of BD confirmation to 75%. Sensitivity was 70% for the first recording SEP and 100% for serial recordings to confirm BD. Conclusion: Combined use of BAEPs and SEPs and serial recordings can improve BD confirmation sensitivity in all clinically BD patients.
