Brachyterapie je metoda léčby zářením, při které se radioaktivní zdroje zavádějí přímo do oblasti nádoru. Umožňuje aplikaci vysoké dávky záření v krátkém čase a dosažení vysokého stupně lokální kontroly nádoru. Největší klinická zkušenost byla získána s brachyterapií s kontinuálním nízkým dávkovým příkonem (0,4–2,0 Gy/hod), kdy se zářiče zaváděly manuálně. V současně době byla tato metoda nahrazena na většině pracovišť automatickými afterloadingovými přístroji využívajícími vysoký dávkový příkon (> 12 Gy/hod) a frakcionaci. U časných ORL nádorů lze užít brachyterapii jako samostatnou léčebnou metodu. Její výhodou v porovnání s chirurgickou léčbou je většinou lepší kosmetický a funkční efekt, v porovnání se zevním ozařováním lepší kontrola nádoru a eliminace postradiační xerostomie. U rozsáhlejších nádorů je brachyterapie indikována k navýšení dávky po zevní radioterapii. Lze ji použít i pro léčbu recidiv v ozářeném terénu. Klasickými indikacemi jsou nádory dutiny ústní, orofaryngu a nasofaryngu. Přes současný technologický pokrok metod zevní radioterapie zůstává brachyterapie důležitou léčebnou modalitou u nádorů ORL., Brachytherapy is a method of radiation treatment, when the radioactive sources are directly applicated into the tumor region. Brachytherapy allows to deliver high doses to the tumor in a short time with excellent local tumor control. The largest experience was achieved with continual low dose rate brachytherapy (0.4–2Gy/hr) and manual insertion of sources. Recently this method was mostly replaced by automatic afterloading devices and high dose rate (> 12Gy/hr) fractionated treatment. Brachytherapy can be used in early stages of head and neck cancer as a sole treatment method. The advantage in comparison with surgery is a better functional and cosmetic outcome and in comparison with external beam radiotherapy a better tumor control and absence of xerostomia. In locally advanced tumors brachytherapy can be used for dose escalation of external beam radiotherapy. Brachytherapy is also useful for salvage treatment of recurrences after previous radiotherapy. Classical indications are oral cancer, oropharyngeal and nasopharyngeal tumors. Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed.
