Serum or plasma creatinine level determination has been serving for decades as the most commonly used marker of renal function. However, creatinine levels are affected by a number of other factors not related to renal function, particularly age, gender, muscle mass, starvation, wasting diseases, post surgical states, and activity or exertion. Alternatives for the assessment of renal function or glomerular fi ltration rate (GFR), through measurement of the clearance of exogenous substances like, inulin, 51Cr-ethylendiaminotetraacetate (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetic acid or iohexol are more accurate, but too complex, laborious, and not suitable for routine clinical use. Therefore, serum creatinine measurement yet remains the most common method to estimate renal function in routine clinical practice. In the presented case study, nearly 50% decline in serum creatinine level as measured by enzymatic colorimetry method according to the manufacture’s recommendation is described. Relatively rapid serum creatinine concentration decline from 67 µmol/L baseline value to 30 µmol/L within 4 days in a CF patient post bilateral lung transplantation, who, was on courses of essential immunosuppressive, antimycotic, and antibacterial drugs including an aminoglycoside (amikacin) was observed. The change in serum creatinine level within four consecutive days after bilateral lung transplantation (mean difference, -49.25 % µmol/L; with 95% confi dence interval, -29 to -37 µmol/L) was signifi cant. Herein, the event is not essentially refl ecting better renal function, since amikacin trough level was unacceptably high (20 mg/L) at the same time, rather explaining probably low production of creatinine as a result of underlying pathology in cystic fi brosis. Patient related explanations and existing methodological challenges have also been discussed., Hladina sérového nebo plazmatického kreatininu slouží již desítky let jako nejčastější používaný ukazatel renální funkce. Přesto je koncentrace sérového kreatininu ovlivněna jinými, s funkcí ledvin nesouvisejícími faktory (věk, pohlaví, svalová hmota, stav výživy, nemoci poškozující organismus, pooperační stavy, tělesná aktivita nebo vyčerpávající fyzický výkon). Alternativy pro zjištění renální funkce/gromerulární fi ltrace (GFR) pomocí podání exogenních látek (inulin, 51Cr-ethylendiaminotetraacetát (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetát nebo iohexol) jsou přesnější, ale zároveň velmi složité, pracné a nehodí se pro rutinní praxi. Měření sérového kreatininu proto stále zůstává běžnou metodou k odhadu renální funkce v rutinní klinické praxi. V tomto sdělení popisujeme cca 50% pokles sérového kreatininu měřeného enzymatickou metodou s kolorimetrickou detekcí podle návodu výrobce. Relativně rychlý pokles sérového kreatinu z 67 µmol/l na 30 µmol/l v průběhu 4 dnů byl pozorován u pacientky s cystickou fi brózou po oboustranné transplantaci plic, s náležitou imunosupresivní, antimykotickou i antibakteriální léčbou včetně aminoglykosidů (amikacin). Změny v hladině sérového kreatininu během 4 dnů (průměrný rozdíl 49, 25 %, při 95% intervalu spolehlivosti, -29 až -37 µmol/l) jsou významné. Tento jev přesto nemusel být odrazem lepší clearance vzhledem k nepřijatelně vysoké „trough“ hladině amikacinu a nejspíše poukazuje na možné snížení produkce kreatininu v souvislosti s cystickou fi brózou. Cílem studie bylo zavést HPLC metodu pro stanovení albuminurie, potvrdit či vyvrátit tvrzení, že HPLC poskytuje falešně vyšší koncentrace albuminu v moči díky interferenci s dalšími bílkovinami, porovnat koncentrace albuminu v moči stanovené HPLC a imunoturbidimetricky. Materiál a metody: Metodu HPLC jsme zavedli za následujících podmínek: složení mobilní fáze: složka A) voda, složka B) acetonitril, složka C) 100 mM NaH2PO4 a složka D) 1 % trifluoroctová kyselina; kontinuální gradient mobilní fáze; průtok mobilní fáze 2 ml/min; teplota analýzy 22 ºC; UV detekce při 280 nm; kolona Zorbax 300 SB-C3, kapalinový chromatograf Agilent 1200 (Agilent Technologies, USA). Analyzovali jsme 2 směsné moči. Do směsi 1 připravené ze vzorků 30 imunoturbidimetricky negativních močí byl přidán standard albuminu. Směs 2 byla připravena z 30 imunoturbidimetricky „mikroalbuminurických“ vzorků. Soubor tvořilo 301 pacientů (161 diabetiků a 140 nediabetiků, 182 mužů a 119 žen, věk 44,1 ± 25,3 let). Výsledky: Za uvedených analytických podmínek transferin, α-1-kyselý glykoprotein, α-1-antitrypsin, α-1-antichymotrypsin a hemopexin neinterferují s albuminem, zatímco eluční křivka prealbuminu se štěpí na několik píků, z nichž některé z nich s albuminem koeluují. Vzhledem k nízkým sérovým i močovým koncentracím prealbuminu lze usuzovat, že je tato interference klinicky nevýznamná. Ve směsi 1 jsme nezaznamenali významný rozdíl v hodnotách albuminurie mezi metodami (79,1 mg/l imunoturbidimetricky vs. 82,5 mg/l HPLC), oproti tomu, ve směsi 2 byla HPLC zjištěná hodnota o 26 % vyšší (79,5 mg/l imunoturbidimetricky vs. 99,9 mg/l HPLC). Tyto výsledky nasvědčují na existenci imunonereaktivního albuminu. Zjistili jsme statisticky významný rozdíl mezi metodami v případě pacientských vzorků moči (soubor všech pacientů [medián ± SEM]: 21,1 ± 38,2 mg/l imunoturbidimetricky vs. 30,4 ± 42,2 mg/l HPLC, p < 0,0001, Mann-Whitneyův test), nejmarkantnější u imunoturbidimetricky „normoalbuminurických“ diabetiků. Závěr: Naše výsledky svědčí pro vyšší senzitivitu HPLC metody pro zachycení „mikroalbuminurie“ (ve srovnání s imunoturbidimetrií). Uvedená HPLC metoda je dostatečně specifická pro stanovení albuminu v moči, nedochází k významným interferencím s dalšími bílkovinami., Objective: The aim of our study was the implementation of HPLC method for assessing albuminuria, confirming or refusing the hypothesis about the existence of co-eluting proteins, and comparison of urine albumin concentrations assessed using HPLC and immunoturbidimetric methods in patient samples. Material and Methods: We developed the HPLC method under these chromatographic conditions: multilinear gradient of mobile phase consisting of water (solvent A), acetonitrile (solvent B), 100 mM NaH2PO4 (solvent C), 1 % trifluoroacetic acid (solvent D), flow rate 2 mL/min, temperature 22 ˚C, UV detection in the wavelength 280 nm, chromatographic column Zorbax 300 SB-C3, liquid chromatograph Agilent 1200 (Agilent Technologies, USA). We analyzed two mixtures of urine. The first one was prepared from 30 patient urine samples with immunoturbidimetrically physiological albuminuria, to which we added albumin standard. The second mixture was prepared from 30 patient urine samples with mild albuminuria. We analyzed and compared albuminuria in a group of 301 patients (161 diabetics and 140 nondiabetics, 182 males and 119 females, age 44.1 ± 25.3 years). Results: Transferrin, α-1-acid glycoprotein, α-1-antitrypsin, α-1-antichymotrypsin and hemopexin do not interfere with albumin in the presented HPLC method, whereas the elution curve of prealbumin splits into several peaks, of which a few interfere with albumin. With regard to very low prealbumin concentrations in serum and in urine, we can assume that this interference is not of clinical importance. In mixture 1 we did not find a significant difference between the albuminuria assessed using both methods (79.1 mg/L immunoturbidimetrically vs. 82.5 mg/L HPLC), while in mixture 2 we measured over 26 % greater albuminuria using HPLC (79.5 mg/L immunoturbidimetrically vs. 99.9 mg/L HPLC). These results suggest that immunounreactive albumin really exists. We found a statistically significant difference between the methods in patient urine samples (entire patients´ group: [median ± SEM]: 21.1 ± 38.2 mg/L immunoturbidimetrically vs. 30.4 ± 42.2 mg/L HPLC, p < 0.0001, Mann-Whitney test), which was most remarkable in the group of diabetic patients with immunoturbidimetrically physiological albuminuria. Conclusion: Our results prove that the HPLC method for albumin detection is more sensitive than immunoturbidimetry and sufficiently specific and there are no significant interferences with other proteins., and Fořtová M., Klapková E., Průša R.
