The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Cíl studie: Stanovit koncentraci N-terminálního fragmentu natriuretického peptidu typu B u pacientů s neobstruktivním typem hypertrofické kardiomyopatie (HKM) a zjistit vztah k morfologickým a funkčním parametrům zjištěným echokardiograficky. Typ studie: observační. Název a sídlo pracoviště: I. interní kardioangiologická klinika LF UK a FN, Sokolská 585, Hradec Králové. Metody: Do studie bylo zařazeno 21 pacientů s HKM, u kterých bylo před zařazením do studie provedeno echokardiografické vyšetření. Byly analyzovány plazmatické hladiny N-terminálního fragmentu natriuretického peptidu typu B (NT-proBNP) a jejich vztah k ukazatelům morfologie srdečních oddílů a funkce levé komory srdeční. Výsledky: Hodnoty NT-proBNP byly v souboru pacientů významně vyšší v porovnání s kontrolní skupinou (377,7 ± 317,5 ng/l vs. 91,3 ± 76,6 ng/l, p 0,005). Analýza echokardiografických dat ukázala významnou asociaci NT-proBNP a ejekční frakce levé komory (r = -0,48, p = 0,03), frakčního zkrácení levé komory (r = -0,44, p = 0,05), a indexu masy levé komory (r = 0,52, p = 0,02). Závěry: Hypertrofická kardiomyopatie je provázena vzestupem hladiny NT-proBNP. Tento vzestup koreluje s masou levé komory a její systolickou funkcí. Stanovení těchto parametrů může mít klinický význam pro diagnostiku a stratifikaci rizika těchto pacientů., Objective: To assess plasma N-terminal pro brain natriuretic peptide (NT-proBNP) concentration in patients with nonobstructive cardiomyopathy (HCM) and try to find an association with morphological and functional echocardiographic parameters. Design of the study: observational study. Settings: 1st Department of Internal Medicine - Cardioangiology, University Hospital Hradec Králové, Czech Republic. Methods: 21 patients with HCM was enrolled into the study. Echo and catheterization were performed before enrolment into the study. We analysed plasma NT-proBNP concentrations and their association with morphological and functional parameters. Results: Compared to control group, plasma NT-proBNP was significantly increased (377.7 ± 317.5 ng/L vs. 91.3 ± 76.6 ng/L, p = 0.005). We observed significant association of NT-proBNP and left ventricular ejection fraction (r = -0.48, p = 0.03), left ventricular fractional shortening (r = -0.44, p = 0.05) and left ventricle mass index (r = 0.52, p = 0.02). Conclusions: Hypertrophic cardiomyopathy is associated with increased plasma NT-proBNP concentration. The increase of NT-proBNP is determined by left ventricle mass and systolic function. Objective: A prospective study was performed to determine the relation between plasma N-terminal pro brain natriuretic peptide (NT-proBNP) levels and short- and long-term mortality in patients with acute heart failure (AHF). Settings: 1st Department of Medicine, Institute of Clinical Biochemistry and Diagnostics, Institute of Clinical Immunology and Allergology, Charles University Prague, Medical Faculty and Faculty Hospital Hradec Králové. Methods: NT-proBNP levels were measured at time of admission in 92 consecutive patients with AHF. Results: During one-year follow-up, 32 patients died. Mean levels of NT-proBNP were signifi cantly lower among the survivors (NT-proBNP: 7 855.4 ± 9 919.9 ng/l, vs.15 470.6 ± 11 273.1, p < 0.001). Hazard ratio (HR) for death from any cause for the patients with NT-proBNP levels above median as compared with those with NT-proBNP below median was 2.91 (0.84–10.10) for 7-day, 3.58 (1.17–11.1) for 28-day, and 3.76 (1.49–9.55): for 1-year mortality. Conclusions: NT-proBNP levels are elevated in acute heart failure, NT-proBNP is also the marker of short- and long-term mortality in acute decompensated heart failure patients, and provides prognostic information above and beyond that provided by conventional cardiovascular risk factors., Cíl studie: Posoudit vztah mezi plazmatickou hladinou N-terminálního mozkového natriuretického peptidu (NT-proBNP) a parametry krátko- i dlouhodobé mortality pacientů s akutním srdečním selháním. Materiál a metody: V souboru 92 pacientů s akutním srdečním selháním byla změřena hladina NT-proBNP ze vzorku krve odebraného v době přijetí. Parametry krátko- i dlouhodobé mortality byly stanoveny z dat z následného jednoročního sledování vyšetřovaného souboru. Výsledky: V průběhu sledování klinického stavu do konce prvního roku 32 pacientů zemřelo. Průměrná hladina NT-proBNP byla u přeživších pacientů významně nižší (NT-proBNP: 7 855,4 ± 9 919,9 ng/l, vs 15 470,6 ± 11 273,1, p < 0,001). Hazard ratio (HR) pro úmrtí z jakékoliv příčiny u pacientů nad medián bylo 2,91 (0,84–10,10) pro 7denní, 3,58 (1,17–11,1) pro 28denní a 3,76 (1,49–9,55) pro jednoroční mortalitu. We report the case of a 22-year old male, a self-confessed recreational drug user who developed cardiogenic shock because of severe destruction of the aortic valve by rapidly progressive aortic valve endocarditis. The disease progression was acute; in a matter of days, the clinical manifestations were life-threatening necessitating urgent aortic valve replacement surgery. Cultivation revealed Streptococcus viridans as the microbial agent. Subsequent recovery with antibiotic treatment was without complication. This case report shows that immediately performed transoesophageal echocardiography and early consultation with a cardiac surgeon has fundamental importance in diagnosis and management of acute infective endocarditis in haemodynamically instable patients., Malkia S. Cíl studie: Tato práce sleduje pacienty s akutním koronárním syndromem a k diagnostice akutního infarktu myokardu (AIM) využívá zcela nové technologie proteinových biočipů. Snahou je nejenom ohodnotit tuto novou možnost laboratorní diagnostiky AIM, ale také srovnání méně známých kardiomarkerů se standardními markery a posouzení jejich vztahu ke klinickým parametrům. Materiál a metody: Do studie bylo celkem zařazeno 44 pacientů s diagnózou AIM a krevní odběry byly prováděny v den přijetí, za 24 hodin a poté čtvrtý nebo pátý den hospitalizace. Z kardiomarkerů byl testován panel Cardiac Array (cTnI, CK MB, myoglobin, CAIII, FABP a GPBB) na biočipovém analyzátoru The Evidence InvestigatorTM od firmy Randox (Randox Laboratories Ltd., Velká Británie). Výsledky: Naše práce potvrdila těsný vztah FABP a GPBB k AIM a na základě jejich dynamiky je můžeme považovat za časné kardiomarkery. Zároveň jsme zjistili, že FABP má, stejně jako standardní marker CKMB nebo TnI, také prognostický význam - vyšší hodnoty jsou spojené se systolickou dysfunkcí. Také jsme poukázali na možnost využití biočipového analyzátoru při diagnostice AIM, kdy za poměrně krátký časový úsek získáme výsledky několika analytů najednou, které nám tak podají širší informace o probíhajícím AIM. Závěr: Metoda proteinových biočipů představuje v diagnostice AIM nový efektivní přístup, který ale jistě ke svému rozšíření vyžaduje řadu dalších studií., Objective: This study observes patients with acute coronary syndromes. To the diagnosis of acute myocardial infarction (AIM) we use the new proteins biochip technologies. The aim of this study is to evaluate the new strategy of laboratory diagnostic method and to confront the less known’s cardio markers with the standard markers and with the clinical parameters. Material and methods: This study contains 44 patients with AIM and the blood samples were taken in the time of the admission, 24 hours later and then the fourth or fifth day of hospitalization. We tested the cardiac panel Cardiac Array (cTnI, CK MB, myoglobin, CAIII, FABP and GPBB) on the biochip analyser The Evidence InvestigatorTM from Randox (Randox Laboratories Ltd., United Kingdom). Results: Our study confirms the close relationship between FABP and GPBB to AIM and we can consider these markers as early markers in the diagnosis of AIM. FABP has, as the standard markers CKMB or TnI, the prognostic value too (the higher values are associated with the systolic dysfunction). This study employs a new strategy of the biochip analyser in the diagnosis of AIM (in the same time we have the concentrations of more parameters and complex information about AIM). Conclusion: This method presents new interesting multianalytes approach to the diagnosis of the AIM.
