Cíl studie: Cílem studie bylo analyzovat životní styl studentů lékařské fakulty v dimenzi I. (rizikové chování – výživa, spánek, tělesná hmotnost, kouření, stres), dimenzi II. (volný čas, studium) a v dimenzi III. (sport a pohybové aktivity) a srovnat jednotlivé parametry u studentů prvního a pátého ročníku. Metodika: Výzkumu se zúčastnilo 90 studentů 1. ročníku a 90 studentů 5. ročníku, průměrný věk respondentů byl 19 a 23 let. K získání důležitých anamnestických dat byl používán předem sestavený dotazník. Výsledky: V 1. ročníku bylo signifikantně více studentů s BMI≤18,5 kg.m-2 (14,4 % vs. 2,2 % v 5. ročníku, p=0,003). V 5. ročníku bylo signifikantně více studentů s BMI mezi 25–30 kg.m-2 (11,1 % vs. 3,4 % v 1. ročníku, p=0,003). U studentek 5. ročníku byla zjištěna signifikantně vyšší hodnota BMI než u studentek 1. ročníku (medián 21,2 kg.m-2 vs. 20,1 kg.m-2, p=0,001), stejně jako u mužů studentů 5. ročníku (medián 23,7 kg.m-2 vs. 22,4 kg.m-2, p=0,029). V souvislosti se studiem na LF došlo ke zvýšení tělesné hmotnosti u 52,2 % studentů 5. ročníku a 23,9 % studentů 1. ročníku, p=0,002. Studenti 1. ročníku se cítí více ve stresu (78,9 % vs. 38,4 %, p<0,001), spí denně pouze 4–6 hodin (31,1 % vs. 17,8 %, p<0,001), méně studentů (17,8 %) užívá pravidelně léky (vs. 55,6 % 5. ročník, p<0,001). Studenti prvního ročníku mají méně volného času než studenti 5. ročníku (medián 14 hodin/týden vs. 20 hodin/týden, p<0,001) a významně méně cvičí pravidelně každý týden (41,1 % vs. 58,9 %, p=0,025). Signifikantně více studentů 5. ročníku pije kávu (81,1 % vs. 63,3 %, p=0,012), studenti častěji zažili v průběhu studia na LF záchvat úzkosti související se školou (81,1 % vs. 61,1 %, p=0,005), spí častěji pravidelně po návratu ze školy (43,3 % vs. 22,2 %, p=0,004). Více studentů 5. ročníku má vlastní zkušenost s užíváním antidepresiv (10,0 % vs. 1,1 %, tj. 1 student 1. ročníku, p=0,018). Závěr: Studenti lékařské fakulty mají vysoký výskyt některých ovlivnitelných rizikových faktorů kardiovaskulárních onemocnění., Aim: The study aimed at analyzing the lifestyle of medical students in dimensions I (risk behaviors – nutrition, sleep, body weight, smoking, stress), II (leisure time, studies) and III (sports and physical activities), and comparing individual parameters in first- and fifth-year students. Methods: 90 first-year students and 90 fifth-year students participated in the research, the average age of the respondents was 19 and 23 years. A questionnaire was used to obtain important anamnestic data. Results: There were significantly more students with BMI≤18.5 kg.m-2 in the first-year (14.4% vs. 2.2% in year 5, p=0.003). In the fifth-year, significantly more students had BMI between 25 and 30 kg.m-2 (11.1% vs. 3.4% in year 1, p=0.003). Both female and male fifth-year students had significantly higher BMI than their female and male first-year counterparts (median 21.2 kg.m-2 vs. 20.1 kg.m-2, p=0.001 and 23.7 kg.m-2 vs. 22.4 kg.m-2, p=0.029, respectively). A total of 52.2% of fifth-year students and 23.9% of first-year students (p=0.002) increased their body weight in association with medical studies. First-year students feel stressed more often (78.9% vs. 38.4%, p<0.001) and sleep for only 4–6 hours (31.1% vs. 17.8%, p<0.001) but take regular medicines less often (17.8% vs. 55.6%, p<0.001). They also have less leisure time (median 14 hours/week vs. 20 hours/week, p<0.001) and significantly less regular weekly physical activity (41.1% vs. 58.9%, p=0.025) than students in the fifth-year. Significantly more fifth-year students drink coffee (81.1% vs. 63.3%, p=0.012), have experienced an anxiety attack associated with their medical studies (81.1% vs. 61.1%, p=0.005), and sleep regularly after returning from school (43.3% vs. 22.2%, p=0.004). More fifth-year students have their own experience with the use of antidepressants (10.0% vs. 1.1%, i.e. 1 student in year 1, p=0.018). Cíl: Charakterizovat změny struktur kolenního kloubu, které se podílejí na poruše funkce kloubu po plastice předního zkříženého vazu. Metodika: V souboru 44 pacientů po operaci předního zkříženého vazu odeslaných k vyšetření magnetickou rezonancí pro potíže v operovaném kolenním kloubu jsme hodnotili celistvost náhrady LCA a výskyt dalších strukturálních změn kloubu. V souboru bylo 36 mužů (81,8 %) a osm žen (18,2 %). Medián věku pacientů byl 31, 0 let, minimální věk 18 let a maximální věk 52 let. Pacienty jsme vyšetřili na přístroji Signa HDxT 1,5T (GE Healthcare, Milwaukee, USA) s použitím cívky HD 1,5T knee coil. Výsledky: V souboru 44 vyšetřených pacientů po plastice LCA jsme u tří pacientů (6,8 %) diagnostikovali selhání graftplastiky LCA. U dalších pacientů s příznivým průběhem graftu LCA, ale s klinickými obtížemi v kolenním kloubu jsme prokázali následné leze, které se podílely na pooperačním dyskomfortu pacientů. Šlo o leze menisků v 88,6 %, akcentovanou nitrokloubní tekutinu v 75 %, kostní edém v 45,5 %, prosáknutí Hoffova tělesa v 36,4%, náplň Bakerovy pseudocysty v 15,9%, lézi laterálního kolaterálního vazu v 11,4 %, lézi zadního zkříženého vazu v 6,8 %. U dvou pacientů (4,5 %) jsme vyšetřili z důvodu předchozího selhání reoperovaný, nyní pevný graft LCA. Závěr: Mezi nejčastější příčiny dysfunkce kolenního kloubu po plastice LCA mimo selhání graftu LCA patří leze menisků, zmnožení nitrokloubní tekutiny, kostní edém, prosáknutí Hoffova tělesa, náplň Bakerovy pseudocysty, leze laterálního kolaterálního vazu, leze zadního zkříženého vazu. Diagnostika a přesné určení typu leze kolenního kloubu je pro ošetřujícího lékaře důležitá z důvodu stanovení dalšího konzervativního nebo chirurgického terapeutického postupu. Klíčová slova: graftplastika předního zkříženého vazu, kolenní kloub, magnetická rezonance, ruptura náhrady předního kříženého vazu, Aim: To characterize changes in the structures of the knee joint which contribute to the failure of the joint to function after the reconstruction of the anterior cruciate ligament. Methods: In a group of 44 patients after surgery of the anterior cruciate ligament sent for MRI examination for problems in the knee joint that had been operated on, we evaluated the integrity of the ACL replacement and the occurrence of other structural changes in the joint. The group comprised 36 men (81.8%) and 8 women (18.2%). The median age of the patients was 31.0 years, the minimum age was 18 years and the maximum age was 52 years. The patients were examined with the HDxT Signa 1.5 T (GE Healthcare, Milwaukee, USA) using the HD 1.5T knee coil. Results: In the group of 44 patients examined after ACL reconstruction, we diagnosed a failure of the ACL graft in 3 patients (6.8%). For other patients with a favorable course of the ACL graft, but with clinical problems in the knee joint, we demonstrated subsequent lesions that caused postoperative discomfort in the patients. These lesions include a me-niscal lesion in 88.6%, increased synovial fluid in 75%, a bone edema in 45.5%, Hoffas fat pad syndrome in 36.4%, filling of the Baker pseudocyst in 15.9%, a lesion of the lateral collateral ligament in 11.4% and a lesion of the posterior cruciate ligament in 6.8%. In 2 patients (4.5%) we examined a currently stable ACL graft that had been reoperated because of the failure of a previous graft. Conclusion: The most common causes of dysfunction of the knee joint after ACL reconstruction, besides the failure of the ACL graft, include a meniscal lesion, an increase in the amount of synovial fluid, a bone edema, Hoffas fat pad syndrome, filling of the Baker pseudocyst, a lesion of the lateral collateral ligament and a lesion of the posterior cruciate ligament. Diagnosis and accurate determination of the type of lesion of the knee joint are important for an orthopedic surgeon because of the determination of further conservative or surgical therapeutic procedures.
