Onkologická léčba je spojená s četnými nežádoucími účinky. Pokud je kurativní léčba pouze v rukou chirurga, je její použití limitováno pouze kontraindikacemi celkové narkózy a operačního výkonu. Pokud se zvažuje dlouhodobá medikamentózni nebo radiační léčba, je nutno vždy zvážit nežádoucí účinky a prinos. Faktory ovlivňující úspěch či neúspěch neoperačni léčby (histologie, klinické stadium, grade, věk, performance status, stav výživy) jsou analyzovány v textu. Incidence maligních chorob se zvyšuje s věkem, a proto lékaři právě u starších lidí stojí často před hamletovskou otázkou: dát či nedát chemoterapii. V závěru textu navrhujeme doporučení pro řešení této otázky., Current therapy of malignant diseases has various adverse effects. In all cases with planned chemotherapy or radiotherapy the potential benefit should overweigh the risks of toxicity. The following prognostic factors are discussed in this paper: histology, clinical stage, pathologic grade, age, performance status, nutrition and individual will of informed person. The incidence of cancer is high in older people and therefore many doctors must solve the Hamlet's question to give or not to give chemotherapy in higher age. K tomu, aby každý mohl bez problémů vykonávat svoji práci, nebyl unavený, podráždený, vyčerpaný a byl fyzicky i psychicky v rovnováze, potřebuje zdravý spánek, který by měl trvat 6-9 hodin. Poruchy spánku, kterými trpí 35-50 % obyvatel v ČR, mohou být primární nebo sekundární, nicméně stanovit příčinu, potvrdit diagnózu a navrhnout správnou terapii je často velmi problematické. K objasnění příčiny poruchy spánku a bdění je nutné kompletní vyšetření neurologické, internistické, psychiatrické a biochemické se zaměřením na vyvolávající příčiny. Své místo má i tzv. „objektivizace poruchy", kdy základní informace o poruše spánku získáváme od druhé osoby, která je v kontaktu s postiženým. Získané informace a závěry jednotlivých vyšetření nám pomáhají adekvátně zahájit terapii nejen farmakologickou (hypnotika, antidepresiva, anxiolytika), ale i psychoterapii, která má význam hlavně u geriatrických pacientů., Everyone needs healthy sleeping of 6-9 hours a day enabling to perform work without problems, to avoid feeling of exhausting and irritation as well as to feel physically and psychically balanced. In Czech Repubhc 35-50 % of population suffer from sleeping defects which could be primary or secondary, anyhow identification of source of problems, confirmation of diagnosis and starting of correct therapy are often very complicated. Identification of causes requires complex neuralgic, internal and psychiatric examination as well as biochemical focused on the eventual initiating causes. It means objective identification of defect where basic information about sleeping defects is acquired from the person who is in close contact with the patient. Acquired information and results of examination enable to start therapy, not only pharmacological (hypnotics, antidepresives, anxiolitics) but also psychotherapy (very important especially at geriatric patients)., Lenka Hronovská, and Lit. 6
Fyziologické změny propulzní aktivity trávicího ústrojí provázející stárnutí jsou klinicky nevýznamné, u zdravých dobrovolníků seniorů prokazujeme pouze mírně zpomalený tranzitní čas potravy tlustým střevem. Většina klinických potíží je způsobena organickým postižením trávicího traktu, jiným orgánovým nebo systémovým onemocněním (viscerální neuropatie při diabetů nebo Parkinsonově chorobě, chronická systémová zánědivá reakce, sklerodermie, hypotyreóza apod). Méně se ve vyšším věku setkáváme s funkčními gastrointestinálními porúchami. Symptomatická léčba zahrnuje především prokinetika, která stimulují propulzní aktivitu a upravují porušenou koordinaci gastrointestinálni motility. Je uveden přehled indikací prokinetik a diskutován jejich účinek u nejčastějších patologických stavů ve stáří., Physiological changes of the propulsive activity of gastrointestinal system accompanying aging are clinically non signihcant; only mildly delayed colonic transit time was reported in healthy elderly volunteers. The majority of chnical symptoms is caused by organic disorders of the gastrointestinal system and/or by other organ pathology or systemic diseases (e.g. (visceral neuropathy in diabetes or Parkinson's disease, chronic inflammatory reaction, sclerodermia, hypothyroidism). Functional gastrointestinal disorders are less common in older age. Symptomatic treatment includes prokinetic drugs, which stimulate propulsive activity and improve impaired co-ordination of gastrointestinal motility.
