Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Cíl: Významem naší prospektivní studie je možnost významné redukce celkového objemu kontrastní látky při CTA DK díky optimalizaci akvizičního času na základě individuálních cirkulačních parametrů. Metoda: Celkem u 68 osob s ischemickou chorobou dolních končetin bylo prospektivně provedeno CT angiografické vyšetření břišní aorty a dolních končetin (CTA DK) s použitím kontrastní látky o objemu 40 ml (31 osob) a 50 ml (37 osob) a vysokou rychlostí aplikace (6 ml/s). Akviziční čas byl individuálně vypočten na základě aorto-popliteálního (Ao-P) tranzitního času získaného stanovením denzitní bolusové křivky na dvou úrovních (aorta/popliteální tepna) po jednorázovém podání 10 ml kontrastní látky (tzv. dvojitý testovací bolus). Kvalita vyšetření vyšetření byla hodnocena subjektivním (skóre 1-3) a objektivním měřením denzity v pěti úrovních. Vyšetření byla označena jako nediagnostická, pokud byla alespoň jedna oblast při subjektivním hodnocení ohodnocena skórem 3. Ve statistickém hodnocení byla porovnána dosažená kvalita s hodnotami akvizičního času a zpoždění. Výsledky: Prokázali jsme statisticky významnou korelaci akvizičního času (21-43) a subjektivního (p = 0,0015) a objektivního (p = 0,0117) hodnocení kvality. Také v případě Ao-P tranzitního času byla prokázána statistická korelace s hodnocením kvality (0,0013, resp. 0,0191). Signifikantně vyšší akviziční čas byl použit u vyšetření s celkově subjektivně nediagnostickou kvalitou (p = 0, 073) a ve všech případech byl vyšší než 31 s. Naopak všechna vyšetření s akvizičními časy pod 30 s byla plně diagnostická. Při podání 50 ml kontrastní látky bylo dosaženo významně vyšší denzity v jednotlivých sledovaných úrovních (p = 0,076), hodnoty akvizičního času a Ao-P tranzitního času se významně mezi skupinami s různým množstvím kontrastní látky nelišily. Závěr: Naše práce prokázala možnost významné redukce objemu kontrastní látky při CTA DK za pomoci optimalizace akvizičního času na základě Ao-P tranzitního času, který byl získán po jednorázové aplikaci 10 ml kontrastní látky (dvojitý test bolus). Byla prokázána statisticky významná korelace akvizičního času a Ao-P tranzitního času na kvalitě provedených vyšetření., Aim: The goal of the prospective study is to determine feasibility of peripheral CT angiography (CTA) with adaptive scan time using low volume of contrast agent. Methods: We performed CTA in extent of abdominal aorta and lower limbs on dual-source CT in two groups of patients with different volumes of contrast agent (400 mgl/l). Volumes of 40ml (31 patients) and 50 ml (21 patients) with 6ml/s injection were used for examinations. Individual scan time was calculated from peak transit time established from double test bolus (single administration of 10 ml) measurements (proximal abdominal aorta and popliteal arteries). We analyzed subjective quality of CTA (1-3) and objective quality (intraluminal density and CNR) in 5 different levels. Examinations with subjective quality 3 (non-diagnostic) in one level were considered as overall non-diagnostic. Results: We found significant correlation between scan time (21-43 s) and subjective (p = 0.0015) and objective quality (p = 0.0117) in both groups. Significantly higher (p = 0.029) scan time was used in examinations with overall non-diagnostic quality (6 in 40 ml group and 3 in 50 ml group). The scan times in non-diagnostic examinations were completely over 31s (31.6-42.4) and all examinations with scan time below 30 s were with excellent quality. Significantly better objective quality was reached in 50 ml group (p = 0.076). Time parameters were not significantly different in both groups. Conclusion: Our results have proved feasibility of peripheral CTA with adaptive scan time and low volume of contrast agent.
