Here we report the case of a 31-year-old man admitted to our hospital with echocardiografic and Cardiac Magnetic Resonance signs of myocarditis complicated by ventricular tachycardia, initially resolved with direct current shock. After the recurrence of ventricular tachycardia the patient was submitted to electrophysiological study revealing a re-entrant circuit at the level of the medium segment of interventricular septum, successfully treated with transcatheter ablation. This case highlights how the presence of recurrent ventricular arrhythmias at the onset of acute myocarditis, suspected or proven, could be associated with a pre-existing arrhythmogenic substrate, therefore these patients should be submitted to electrophysiological study in order to rule out the presence of arrhythmogenic focuses that can be treated with transcatheter ablation., Emanuele Cecchi, Serena Fatucchi, Elena Crudeli, Cristina Giglioli, and Literatura 15
Behçet's disease is a multisystem chronic inflammatory disease generally presenting as recurrent oral-genital ulcers and uveitis. Vascular involvement is a common presentation. However, cardiac involvement is rare in this disorder. In this case report, we present a large right ventricular thrombus detected both in computed tomography and echocardiography in a 32 year-old male patient with complaints of fever, haemoptysis and weight loss. V kazuistice je popsán průběh onemocnění 55letého nemocného s neuroendokrinním tumorem a postižením jater, pankreatu, mezenteria, tenkého střeva a mediastina. Léčba spočívala v podávání analog somatostatinu a při progresi onemocnění byla přidána léčba sunitinibem., Case history of 55 years old man with neuroendocrine tumor of liver, pankreas, mesentery, small intestine and mediastinum is decribed. Kazuistika pojednává o třicetileté pacientce, která byla opakovaně hospitalizována pro levostrannou ileofemorální trombózu. Pacientka byla indikována k lokální farmakologické trombolýze. V průběhu trombolýzy bylo však vyjádřeno podezření na možnost komprese cév zvenčí. Následovala další vyšetření, která odhalila měkkotkáňovou nebolestivou formaci na mediální straně stehna jako příčinu na léčbu rezistentní trombózy. Rezistence se nacházela v těsném kontaktu s nervově-cévním svazkem na stehně. Dle histologie se jednalo o bifázický synoviální sarkom. V době diagnózy byly již přítomny vzdálené plicní metastázy I přes intenzivní neoadjuvantní léčbu dochází k progresi zejména plicního postižení. Chirurgická léčba není toho času indikována. Pacientka umírá 6 měsíců od stanovení diagnózy., Case report deals with a 30-year-old female patient, who was repeatedly admitted to hospital with left sided ileofemoral thrombosis. The patient was scheduled to local farmacological thrombolysis. During the thrombolysis a suspicion of compression of the vessels from outside was expressed. A further examination revealed an extensive soft-tissue non-painful mass on the medial side of the thigh as a cause of the thrombosis, which was not responding to the treatment. The lesion was in close contact with the femoral neurovascular bundle. The histological examination proved biphasic synovial sarcoma. At the time of diagnosis, distant lung metastases were already present. Despite intensive neoadjuvant therapy the disease, especially the lung affection progressed. Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed. Význam péče o výživu je neoddiskutovatelný. Malnutrice provází vysoké procento onkologicky nemocných, může zásadně zhoršit výsledky onkologické léčby a ovlivňuje kvalitu života pacienta, proto její léčba i prevence musí být součástí podpůrné péče v onkologii. Stav výživy pacientů je nutné sledovat v průběhu celého onemocnění a jeho léčby. V článku je předvedena kazuistika padesátiletého pacienta s druhou recidivou karcinomu hrany jazyka, který díky podpůrné péči, jejíž součástí je i péče o výživu, absolvoval reiradiaci kombinovanou s cetuximabem a nyní je bez známek choroby. Kazuistika je také dokladem vývoje nutriční péče na našem ústavu v průběhu několika let., The significance of nutritional care is undeniable. Malnutrition is seen in a high proportion of cancer patients, may substantially deteriorate the outcome of oncological treatment, and affects the quality of life of the patient; therefore, its treatment and prevention must be a part of supportive care in oncology. The nutritional status of patients needs to be monitored throughout the disease course and treatment. The paper presents a case report of a fifty-year-old patient with a second recurrence of carcinoma of the margin of the tongue who owing to supportive care, a part of which is nutritional care, underwent reirradiation combined with cetuximab and is currently free of symptoms of the disease.
