Osteochondritis dissecans(OCD) of the knee is identified with increasing frequency in the adolescent patient. Left untreated, OCD can cause significant impairment and restriction in physical activity and development of osteoarthritis at an early age. The diagnosis of lesions of OCD can be confirmed on plain radiographs. MRI has emerged as the gold standard to evaluate the stability of the lesion and the integrity of the overlying articular cartilage. Treatment of OCD lesions depend on the stability of the lesion. Stable lesions can be treated conservatively by physical activity modification and immobilization. Unstable lesions and stable lesions not responding to conservative measures should be treated surgically. Surgical options range from arthroscopic drilling, either transarticular or extra-articular drilling for stable lesions or salvage procedures such as autologous chondrocyte transplantation (ACT), mosaicplasty to restore joint and cartilage congruency. and H. V krátkém sdělení dokumentujeme u pacienta s gliomem postihujícím precentrální gyrus relativitu výsledku zobrazení pyramidové dráhy pomocí DW traktografie a diskutujeme o faktorech, které mohou ovlivnit zobrazení dráhy. Dle našeho názoru se v dostupné literatuře nevěnuje dostatečná pozornost volbě oblasti zájmu trasování, přičemž právě ta je klíčovým determinantem výsledku., In our short report, we show the relative nature of pyramidal tract representation using Diffusion Tensor Imaging in a patient with a precentral gyrus glioma and we discuss the factors that may affect tract reconstruction. We believe that, so far, inadequate attention has been paid in the published literature to the region of interest selection even though this is the key determinant of the imaging outcome. Předpoklad: Tikagrelor je inhibitor receptoru P2Y12 a má prokázaný účinek na snížení ischemických příhod 1 rok po akutním infarktu myokardu. Jeho efekt v delším časovém období nebyl znám. Metodika: Zařazeno bylo 21 162 nemocných s anamnézou infarktu myokardu 1–3 roky před randomizací, která byla v poměru 1 : 1 : 1 na skupiny: tikagrelor 2krát 90 mg : tikagrelor 2krát 60 mg : placebo. Všichni nemocní museli být léčeni malou dávkou kyseliny acetylsalicylové. Pacienti byli sledováni 33 měsíců. Primární cíle byly: kardiovaskulární úmrtí, infarkt myokardu nebo cévní mozková příhoda. Primárním bezpečnostním cílem bylo krvácení. Výsledky: Obě dávky tikagreloru signifikantně snížily primární kombinovaný cíl za 3 roky – 7,85 % tikagrelor 2krát 90 mg, 7,77 % tikagrelor 2krát 60 mg a 9,04 % placebo (HR pro tikagrelor 2krát 90 mg vs placebo 0,85; 95% CI 0,75–0,96; p = 0,0080; HR pro tikagrelor 2krát 60 mg vs placebo 0,84; 95% CI 0,74–0,95; p = 0,0043). TIMI velké krvácení bylo vyšší u tikagreloru (2,60 % pro 2krát 90 mg, 2,30 % pro 2krát 60 mg a 1,06 % pro placebo, p < 0,001 pro obě dávky). Smrtelné intrakraniální krvácení bylo 0,63 %, 0,71 % a 0,60 % (NS). Závěr: Léčba tikagrelorem více než 1 rok po infarktu myokardu snižuje riziko úmrtí infarktu myokardu, cévní mozkové příhody s mírným zvýšením rizika velkých krvácení., Background: Ticagrelor is a P2Y12 receptor antagonist that has been shown to reduce ischemic events for up to a year after an acute coronary syndrome. The efficacy and safety of long-term ticagrelor therapy beyond 1 year after a myocardial infarction is unknown. Methods: We randomized 21,162 patients with a history of myocardial infarction within the prior 1–3 years in a double-blind 1 : 1 : 1 fashion to ticagrelor 90 mg twice daily, ticagrelor 60 mg twice daily, or placebo, all with low-dose aspirin, and followed them for a median of 33 months. The primary efficacy endpoint was the composite of cardiovascular death, myocardial infarction, or stroke. The primary safety endpoint was TIMI major bleeding. Results: Both doses of ticagrelor significantly reduced the primary combined efficacy endpoint compared to placebo with Kaplan-Meier rates at 3 years of 7.85 % with ticagrelor 90 mg, 7.77 % with ticagrelor 60 mg, and 9.04 % with placebo (HR for ticagrelor 90 mg vs placebo 0.85, 95% CI 0.75–0.96, p = 0.0080; HR for ticagrelor 60 mg vs placebo 0.84, 95% CI 0.74–0.95, p = 0.0043). Rates of TIMI major bleeding were higher with ticagrelor (2.60 % for 90 mg, 2.30 % for 60 mg and 1.06 % for placebo, p < 0.001 for each dose against placebo); the rates of intracranial hemorrhage or fatal bleeding were 0.63 %, 0.71 % and 0.60 % in the 3 arms, respectively. Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. U dvou pacientů byl ejakulát úspěšně použit k fertilizaci., Objectives: Following spinal cord injury (SCI), ejaculatory dysfunctions are frequent. In men who wish to father children, assisted sperm retrieval is often necessary. Penile vibratory stimulation (PVS) is a non-invasive method for sperm retrieval in patients with a lesion above T10. The aim of our research was to evaluate the effectiveness and safety of PVS, semen quality and further utilization of the ejaculate. Methods: Between 2010 and 2014, we performed PVS in 20 patients. An injury of the cervical (C) and thoracic (T) spinal cord was present in 13 and seven patients, respectively. The mean age of patients was 30.8 years. The mean time from the injury to PVS was 64 months. Seven patients had the first PVS more than 3.5 years after the injury and 13 patients less than 3.5 years. FERTI CARE Multicept was used for PVS. Results: Ejaculation was achieved in 11 patients (55%) – in nine with C and in two with T SCI. Success rate of PVS in patients less than 3.5 years after the injury was 77%, compared to 14% in patients more than 3.5 years after the injury, p = 0.027. Total sperm count (p = 0.03), sperm concentration (p = 0.025) and the number of sperm with progressive motility (p = 0.027) increased in the semen from the second PVS. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients. Perigraft serom je poměrně vzácná komplikace, která může vzniknout po implantaci cévní protézy z dakronu nebo expandovaného polytetrafluoretylenu (ePTFE). Předkládáme kazuistiku 54letého pacienta s perigraft seromem okolo axillofemorálního bypassu (ePTFE protézou). Konečným řešením byla explantace této extraanatomické rekonstrukce a implantace aortobiilického bypassu s použitím cévní protézy z jiného materiálu. Článek diskutuje terapeutické možnosti této komplikace na základě publikovaných prací. Žádná guidelines ani jiná doporučení neexistují. Závěrem, přístup k léčbě perigraft seromu zůstává striktně individuální. Náhrada cévní protézy s použitím implantátu z jiného materiálu se zdá být nejlepší možností řešení pro recidivující perigraft seromy tam, kde jiné méně invazivní postupy selhaly., Perigraft seroma is quite a rare complication that may occur after implantation of Dacron or expanded polytetrafluoroethylene (ePTFE) vascular grafts. We report a case of a 54-year-old patient with perigraft seroma around an axillofemoral bypass (ePTFE graft). Definitive treatment involved the explantation of this extraanatomic bypass with perigraft seroma and the implantation of an aortobiiliac bypass using vascular prosthesis made of a different material. Based on published studies, therapeutic options for this complication are discussed. No guidelines or recommendations are available. In conclusion, the approach to perigraft seroma treatment remains strictly individual. Vascular graft replacement using grafts made of different material seems to be the best option in the case of recurring perigraft seroma, where less invasive procedures were not successful., and A. Úvod: Cílem práce je porovnat výsledky peroperační detekce sentinelové uzliny při operacích pro karcinom kolon a zjistit počet uzlin na resekátu ve srovnání se standardní resekcí. Metody: Do souboru byli zařazeni pacienti k elektivní resekci pro karcinom kolon. Resekát a sentinelová uzlina odeslány k histopatologickému zpracování. Srovnání s kontrolním souborem elektivních resekcí z roku 2011. Výsledky: Kontrolní soubor čítal 56 pacientů operovaných elektivně pro karcinom od céka po rektosigmoideum (dle MKN C18-C19). Průměrný počet uzlin na resekátu byl 12,73 (4−27). Do studované skupiny bylo do počátku listopadu 2014 zařazeno celkem 102 pacientů, celkem 29 pacientů bylo vyřazeno pro nedodržení nebo odchylku od grantového protokolu (neoznačení, neodebrání uzliny, pooperační jiná definitivní diagnóza než karcinom, přítomnost jaterních metastáz). Ze zbylých 73 pacientů (46 mužů a 27 žen) bylo po histopatologickém zpracování celkem 24 (17 mužů a 7 žen) pacientů označeno jako pN+, z nich 2 označeni jako pN1c. U zbývajících 22 pacientů byla sentinelová uzlina pozitivní v 8 případech, což odpovídá senzitivitě 36,36 %. Průměrný počet uzlin na resekátu po barvení Patentblau byl 15,97 (3−30). Závěr: Metoda detekce sentinelové uzliny peroparačně je snadná a snadno proveditelná. Hlavním přínosem se pro nízkou senzitivitu jeví zvýšení počtu nalezených uzlin na resekátu. Klíčová slova: sentinelová uzlina − karcinom kolon − peroperační detekce, Introduction: The aim of this paper is to evaluate the results of intraoperative sentinel node detection in colon cancer patients and to compare the number of nodes retrieved per specimen in comparison with standard resection. Methods: Patients undergoing elective colon cancer resection were included in the study. The specimen and the sentinel lymph node were sent for histopathological examination. A group of patients from 2011 who underwent elective resection served as the study control. Results: The control group comprised 56 patients. The average node count was 12.73 (4–27). The study group included 102 patients; 29 patients had to be excluded because of protocol deviation. Out of the remaining 73 (46 male and 27 female) patients, 24 were N-positive and 2 of them were pN1c. In the remaining 22 patients, the sentinel node was positive in 8 cases, corresponding to a sensitivity of 36.36%. The average lymph node count was 15.97(3–30) after patent blue dye injection. Conclusion: Intraoperative sentinel lymph node detection is an easy and feasible method. Despite the low sensitivity, the main positive effect of the method is the increased lymph node count per resection specimen. Úvod: Terapeutické a toxické účinky digoxinu jsou závislé na jeho sérové koncentraci. Současné poznatky prokázaly snížení mortality při chronickém srdečním selhání při koncentraci digoxinu v rozmezí 0,5–0,9 ng/ml. Cíl: Ukázat výhody terapeutického monitorování hladin léčiv při personalizované farmakoterapii digoxinem. Metoda: Údolní sérové koncentrace digoxinu byly stanoveny imunochemickou metodou MEIA (AxSym Abbott). Pro farmakokinetickou analýzu bylo použito Bayesianské analýzy za pomocí software MW-Pharm 3.30. Výsledky: U 6 pacientů ve věku 71–92 let jsou ukázány velké rozdíly v dávce digoxinu potřebné k dosažení terapeutického rozmezí. Tyto dávky se pohybují v rozmezí 0,03–0,25 mg za den. Závěr: Terapeutické monitorování hladin léčiv je velice užitečnou metodou k predikci koncentrace digoxinu bez nutnosti dosažení ustáleného stavu., Introduction: Therapeutic and toxic effects of digoxin are related to its serum concentrations, recent evidence suggests that a lower therapeutic range 0.5–0.9 ng/ml is associated with reduced mortality. Aim: To show the advantages of therapeutic drug monitoring for dosing of digoxin. Method: Trough serum levels of digoxin were estimated by immunoassay MEIA (AxSym Abbott). The long term serum concentration – time profile of digoxin has been predicted by the Bayesian analysis computer program MW-Pharm 3.30 MediWare. Results: Sex case reports of the patients between 71 to 92 years are presented to show the different doses to receive digoxin levels in the therapeutic range. The doses varies between 0.03–0.25 mg per day. Conclusion: Therapeutic drug monitoring is very usefull for prediction of serum level of digoxin. Úvod: Peritonitida patří mezi život ohrožující stavy zatížené vysokou mortalitou a morbiditou. Prognóza závisí na faktorech spojených s pacientem, charakterem onemocnění, použitými diagnostickými a terapeutickými metodami. Cílem práce bylo retrospektivně analyzovat soubor pacientů se sekundární peritonitidou, z tohoto souboru porovnat skupiny pacientů bez sterkorální peritonitidy a se sterkorální peritonitidou, stanovit hodnotu indexu prognostického systému MPI a zjistit, nakolik se shoduje s našimi výsledky. Metoda: Za období dvou let (2012–2013) byla analyzována data 124 pacientů operovaných s nálezem sekundární peritonitidy. Soubor byl rozdělen podle typu peritonitidy na skupinu A s jinou než sterkorální a skupinu B se sterkorální peritonitidou. Tyto skupiny byly dále porovnávány. Predikce mortality byla ověřena pomocí Mannheim Peritonitis Index (MPI). Výsledky: V celém souboru 124 operovaných pacientů bylo celkem 70 mužů a 54 žen. Průměrný věk pacientů byl 63 let, průměrná délka hospitalizace 18 dní, během hospitalizace zemřelo 18 pacientů (15 %). Průměrná hodnota MPI skóre byla 19, což odpovídá predikci mortality 18 %. Ve skupině A 68 pacientů bez sterkorální peritonitidy byl průměrný věk 59 let, průměrná délka hospitalizace 12 dní. Z této skupiny zemřeli během hospitalizace 2 pacienti (3 %). Průměrná hodnota MPI skóre byla 9, což odpovídá predikci mortality 9 %. Ve skupině B 56 pacientů se sterkorální peritonitidou byl průměrný věk 67 let, průměrná délka hospitalizace 25 dní. Z této skupiny během hospitalizace zemřelo 16 pacientů (29 %). Průměrná hodnota MPI skóre byla 29, což odpovídá predikci mortality 31 % pro tuto skupinu pacientů. Závěr: Z porovnání obou skupin vyplývá, že u skupiny bez sterkorální peritonitidy je podstatně nižší mortalita, menší počet komplikací a kratší délka hospitalizace než u skupiny se sterkorální peritonitidou. K predikci mortality u peritonitidy lze použít základních klinických dat, stejně jako sofistikovaných skórovacích systémů. V naší studii se osvědčil jednoduchý skórovací systém Mannheim Peritonitis Index., Introduction: Peritonitis is a life-threatening disease with high mortality and morbidity. The prognosis depends on patient factors, the nature of the disease, and on diagnostic and therapeutic methods. The goal of our study was to perform a retrospective analysis of a group of patients with secondary peritonitis and to compare patients with non-stercoral peritonitis and those with stercoral peritonitis, and finally, to determine the prognostic value of the MPI prognostic index. Methods: We analysed 124 patients who had undergone surgery for secondary peritonitis during the years 2012–2013. We divided the patients into two groups. Group A comprised patients with non-stercoral peritonitis and group B consisted of patients with stercoral peritonitis. We compared the two groups and predicted the peritonitis outcome using the Mannheim Peritonitis Index (MPI). Results: The complete sample of 124 patients consisted of 70 men and 54 women. The average age of the patients was 63 years and the average length of hospitalization was 18 days. In total, 18 patients (15%) died during their hospitalization. The average MPI score was 19, which correlates to an 18% mortality rate prediction. In group A, which consisted of 68 patients with non-stercoral peritonitis, the average age was 59 years and the average length of hospitalization was 12 days. Two patients (3%) from this group died during hospitalization. The average MPI score was 9, which correlates to a 9% mortality rate prediction. In group B, which consisted of 56 patients with stercoral peritonitis, the average age was 67 years and the average length of hospitalization was 25 days. 16 patients (29%) from this group died during hospitalization. The average MPI score was 29, which correlates to a 31% mortality rate prediction. Conclusions: The outcome of our comparison between the two groups is that group A with non-stercoral peritonitis had a significantly lower mortality, lower number of complications and a shorter length of hospitalization. Both basic clinical data and sophisticated scoring systems can be used for mortality prediction in peritonitis. Popisujeme případ multicentrické formy Castlemanovy choroby, s generalizovanou lymfadenopatií a splenomegalií, provázenou typickými B symptomy – úbytek hmotnosti 15 kg, horečky neinfekčního původu, noční pocení, symptomy anémie. Histologické vyšetření uzlin s nejvyšší akumulací fluorodeoxyglukózy, odebrané torakoskopicky z mediastina, prokázalo plazmocelulární typ Castlemanovy choroby. Vyšetření HIV a lidského herpesviru 8 (HHV-8) bylo negativní. Stanovení diagnózy nemoci předcházely 3 recidivy herpes zosteru, což svědčilo o alteraci imunity. Léčba byla zahájena kombinací talidomidu, dexametazonu a cyklofosfamidu. Po 2 měsících léčby nebyla zřetelná léčebná odpověď a pacient tuto léčbu špatně toleroval. Proto byl tento způsob léčby ukončen. Pro léčbu 2. linie byla zvolena léčba R-CHOP (Mabthera – rituximab, cyklofosfamid, adriamycin, vinkristin a prednison. V průběhu 2 cyklů této léčby se zmenšila lymfadenopatie a splenomegalie, ale léčbu provázely závažné infekční komplikace. Proto dále pokračovala pouze monoterapie přípravkem Mabthera v dávce 375 mg/m2 v 28denních intervalech. Tato léčba se ukázala jako účinná a dobře tolerovaná. Při kontrolním PET-CT vyšetření vymizela lymfadenopatie a splenomegalie a normalizovala se akumulace fluorodeoxyglukózy. Mabthera v monoterapii se v tomto případě ukázala jako účinný a velmi dobře tolerovaný lék. Pro multicentrickou formu Castlemanovy nemoci existuje v současnosti více účinných léčebných alternativ: léčba rituximabem v monoterapii či v kombinaci, léčba imunomodulačními léky (talidomid nebo lenalidomid), léčba protilátkami proti interleukinu 6 (siltuximab) nebo proti jeho receptoru (tocilizumab). V případně neúčinnosti jedné léčebné alternativy je nutné otestovat další alternativu. V tomto případně nebyla úspěšná léčba založená na talidomidu, zatímco léčba založená na přípravku Mabthera dosáhla vymizení projevů nemoci., We describe a case of multicentric Castleman disease with generalized lymphadenopathy and splenomegaly, accompanied by typical B symptoms - loss of 15 kg, fever of non-infectious origin, night sweats, symptoms of anemia. Histological examination of the nodes with the highest accumulation of fluorodeoxyglucose, taken from mediastinum by thoracoscopy, revealed plasmocellular type of Castleman disease. Tests for HIV and human herpesvirus 8 (HHV-8) were negative. Three recurrences of herpes zoster indicating an alteration of immunity preceded the diagnosis of disease. Treatment was initiated with combination of thalidomide, dexamethasone, and cyclophosphamide. The response after 2 months therapy was not clear and patient doesn´t tolerated the therapy well. Therefore, this treatment was terminated and R-CHOP (Mabthera - rituximab, cyclophosphamide, adriamycin, vincristine, and prednisone) was selected as a second-line therapy. Lymphadenopathy and splenomegaly were reduced during the 2 cycles of treatment, however, serious infectious complications accompanied the therapy. Therefore, only use of Mabthera monotherapy 375 mg /m2 was administered in 28-day intervals. This treatment has shown efficacy and tolerability. PET-CT scan has demonstrated disappearance of lymphadenopathy and splenomegaly, in addition, normalized accumulation of fluorodeoxyglucose. Monotherapy with Mabthera has proved to be effective and well tolerated drug in this case. Currently, there are more effective therapeutic alternatives in multicentric Castleman disease: treatment with monotherapy of rituximab or in combination therapy with immunomodulatory drugs (thalidomide or lenalidomide, treatment with anti-IL-6 (siltuximab) or against its receptor (tocilizumab). In the case of ineffectiveness of one treatment option must be tested other alternative. AIM/BACKGROUND: Pistacia terebinthus is used as a coffee substitute in the East and Southern Anatolia regions of Turkey. It contains unsaturated fatty acids, tocopherols, polyphenols and carotenoids. P. terebinthus has anti-inflammatory and potential antioxidant activity. In this study we evaluated the protective effects of P. terebinthus coffee (PTC) on thioacetamide (TAA)-induced liver injury in rats. MATERIALS AND METHODS: Twenty-eight male Sprague-Dawley rats were equally randomized into four groups. Chronic liver injury was induced with TAA (100 mg/kg i.p. three times weekly). The first group of rats served as control and received only tap water (G1), and the remaining groups of rats received PTC, p.o (G2); TAA (G3); TAA plus PTC, p.o (G4), respectively.
