Ovlivnění chování formujícího životní styl má proto velký preventivní potenciál. Kognitivní teorie a modely chování ozřejmují psychologické faktory participující na behaviorální změně a umožňují vytváření efektivních preventivních strategií, které jsou v souladu s bio-psycho-sociálním pojetím zdraví a nemoci. Behaviorální intervence se uplatňují v primární i sekundární prevenci, mají efekt na individuální a populační úrovni, zvyšují kvalitu života a snižují náklady na zdravotní péči. Lifestyle plays an important role in their complex aetiology. Affecting the behaviour forming the lifestyle therefore has a strong preventive potential. Cognitive theories and behavioural models illustrate psychological factors participating in the behavioural change and enable to create effective preventive strategies that are consistent with the bio-psycho-social concept of health and disease. Úvod: Jedním z onemocnění, které je potenciálním rizikovým stavem pro vznik chronické pankreatitidy, je pankreatitida akutní. Materiál: Z celkového počtu 231 osob, které bylo sledovaných s diagnózou chronické pankreatitidy, 56 nemocných bylo iniciálně léčeno s akutní pankreatitidou (24,2 %). V intervalu 12–24 měsíců od prvé ataky akutní pankreatitidy jejich stav postupně progredoval do obrazu pankreatitidy chronické. Osoby zařazené do studie po 1. atace akutní pankreatitidy abstinovaly, během období jejich sledování nebyla recidiva akutní pankreatitidy prokázána. Výsledky: V etiologii akutní pankreatitidy převládal alkohol (55,3 %), biliární etiologie byl prokázána v 35,7 %. Dle revidované Atlantské klasifikace byla u 69,6 % nemocných stanovena pankreatitida těžká, ostatní nemocní splnili kritérium formy intermediární, nemocní s lehkou formou nebyli zařazeni. Závěr: Významnými přítomnými rizikovými faktory bylo kouření a obezita a u 18 %, resp. u 25,8 % nemocných byl zjištěn pankreatogenní diabetes mellitus. Přitom 88,1 % nemocných trpících akutní pankreatitidou byli kuřáci. Největší počet osob s chronickou pankreatitidou po atace pankreatitidy akutní byl v produktivním věku, tj. 25–50 let. Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie., Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Within an interval of 12- 24 months from the first attack of acute pancreatitis, their condition gradually progressed to reached the picture of chronic pancreatitis. The individuals included in the study abstained (from alcohol) following the first attack of acute pancreatitis and no relapse of acute pancreatitis was proven during the period of their monitoring. Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. Úvod: Cieľom štúdie bolo analyzovať výsledky činnosti transplantačného centra v Košiciach. Metódy: Do transplantačného programu boli zaradení pacienti v kritickom štádiu obličkového zlyhávania. Na transplantáciu obličky bolo indikovaných 457 pacientov. Z metodík bola použitá plastika tepny, našitie tepny samostatne a našitie na spoločnom terčíku. Transplantácia (Tx) obličky vyžaduje predovšetkým techniku cievnej anastomózy - chirurgického spojenia ciev. Výsledky: Transplantácie obličiek sa v Transplantačnom centre Košice realizujú od roku 1988. Plastiku tepny sme vykonali u 22,3 % (102) pacientov. Z toho tepny našité samostatne boli u 5,5 % (25). Na spoločnom terčíku ich bolo 9,4 % (43). V 7,4 % (34) bola plastika realizovaná ako implantácia pólových renálnych artérií do hlavného kmeňa. Záver: Transplantácia obličiek je život zachraňujúca metóda., Introduction: The aim of the study was to analyze own results from Transplant Center Kosice, Slovak Republic. Methods: In total 457 patients in the end-stage renal failure were included to the kidney transplant program. Following techniques were used: arterioplasty, individual renal artery anastomosis and multiple renal arteries anastomosed on a common patch. The kidney transplant (Tx) especially requires surgical technique of a vascular anastomosis. Results: Renal transplantation has been established in Transplant Center Kosice since 1988. The arterioplasty of renal artery was performed in 102 (22.3%) and individual artery anastomosis in 25 (5.5%) patients. Multiple artery anastomosis on the patch was done in 43 (9.4%) and implantation of pole renal arteries to the main trunk performed in 34 (7.4%) cases. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions.
