Pseudomyxoma peritonei je vzácný klinický syndrom; nádorový proces, pro který je charakteristické hromadění mucinózního ascitu a peritoneálních implantátů v břišní dutině. Primárním zdrojem je téměř vždy low grade mucinózní tumor apendixu. Jedná se o pomalu progredující, nízce agresivní, ale smrtící onemocnění s častými recidivami. Optimálním léčebným postupem je kompletní chirurgická cytoredukce (CRS) v kombinaci s peroperačně aplikovanou intraperitoneální hypertermickou chemoterapií (HIPEC) – tzv. Sugarbakerova metoda., Pseudomyxoma peritonei (PMP) is an uncommon clinical syndrome characterized by the slow and progressive accumulation of peritoneal implants and mucinous ascites, intraabdominal gelatinous collection (jelly belly). PMP is reported to originate from the low grade appendiceal mucinous neoplasm. Pseudomyxoma peritonei often recurs after treatment and may eventually cause death by abdominal visceral dysfunction via compression with mucinous ascites. Cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy comprising hyperthermic intraperitoneal chemotherapy (HIPEC) with or without postoperative intraperitoneal chemotherapy (EPIC) has been regarded as the standard of care in specialized centers. It is often referred to as the „Sugerbaker procedure“. Conventionally, PMP is considered resistant to systemic chemotherapy. Even though complete cytoreduction is associated with prolonged overall survival, recurrence of disease is common and multiple operations are frequently required. Patients may enjoy sustained period of remission, free of symptoms, but long – term disease free survival is distinctly uncommon. The pathologic subtype remains the dominant factor in survival. Kazuistika pojednává o třicetileté pacientce, která byla opakovaně hospitalizována pro levostrannou ileofemorální trombózu. Pacientka byla indikována k lokální farmakologické trombolýze. V průběhu trombolýzy bylo však vyjádřeno podezření na možnost komprese cév zvenčí. Následovala další vyšetření, která odhalila měkkotkáňovou nebolestivou formaci na mediální straně stehna jako příčinu na léčbu rezistentní trombózy. Rezistence se nacházela v těsném kontaktu s nervově-cévním svazkem na stehně. Dle histologie se jednalo o bifázický synoviální sarkom. V době diagnózy byly již přítomny vzdálené plicní metastázy I přes intenzivní neoadjuvantní léčbu dochází k progresi zejména plicního postižení. Chirurgická léčba není toho času indikována. Pacientka umírá 6 měsíců od stanovení diagnózy., Case report deals with a 30-year-old female patient, who was repeatedly admitted to hospital with left sided ileofemoral thrombosis. The patient was scheduled to local farmacological thrombolysis. During the thrombolysis a suspicion of compression of the vessels from outside was expressed. A further examination revealed an extensive soft-tissue non-painful mass on the medial side of the thigh as a cause of the thrombosis, which was not responding to the treatment. The lesion was in close contact with the femoral neurovascular bundle. The histological examination proved biphasic synovial sarcoma. At the time of diagnosis, distant lung metastases were already present. Despite intensive neoadjuvant therapy the disease, especially the lung affection progressed. Cíl: Otoskleróza je autozomálne dominantné ochorenie s variabilnou penetráciou a expresiou, charakterizované dystrofiou enchondrálnej vrstvy kostného puzdra labyrintu. Ochorenie sa typicky manifestuje prevodovou poruchou sluchu za celistvou blankou bubienka, ale môže sa manifestovať aj percepčnou alebo zmiešanou poruchou sluchu. Metódy a výsledky: Otosklerózu rozdeľujeme na dve základné formy, fenestrálnu a retrofenestrálnu (kochleárnu). Diagnóza otosklerózy sa stanoví na základe anamnézy (rodinná anamnéza), spektra audiologických vyšetrení a zobrazovacími metódami. Dominantné postavenie v zobrazení má HRCT spánkových kostí. HRCT pomáha potvrdiť diagnózu otosklerózy, vylúčiť iné príčiny prevodovej poruchy sluchu za celistvou blankou bubienka a určiť rozsah postihnutia labyrintu. Záver: Predmetom skúmania je vzťah rozsahu postihnutia labyrintu a stavu sluchu pr, Aim: Otosclerosis is an autosomal dominant disease with variable penetrance and expression. It is characterized by dystrophy of enchondral layer of otic capsule. Otosclerosis typically presents with conductive hearing loss and intact tympanic membrane. Sensorineural or mixed hearing loss may be present in some cases. Methods and results: Otosclerosis is categorized into two major groups, fenestral and retrofenestral (cochlear). The diagnosis of otosclerosis is based on history, series of audiologic tests and imaging modalities. HRCT is imaging modality of choice. HRCT helps to establish diagnosis of otosclerosis, excludes other cause of conductive hearing loss with intact tympanic membrane and allows grading of otosclerosis.
