Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
Cíl: Uzávěr bazilární tepny jako příčina akutní ischemické cévní mozkové příhody je relativně vzácný s výskytem okolo 5-6 %. Neléčený má vysokou letalitu blížící se až 90 %. Cílem naší retrospektivní studie bylo zhodnotit bezpečnost a efektivitu revaskularizační terapie kombinující iniciální intravenózní trombolýzu s přímo navazující mechanickou trombektomií a srovnat její výsledky s dříve používanou intravenózní a kombinovanou intravenózní a intraarteriální trombolýzou v léčbě pacientů s akutní ischemickou cévní mozkovou příhodou (iCMP) ve vertebrobazilárním povodí. Metodika: Soubor tvoří všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem arteria basilaris (AB), kteří byli v období od června 2010 do prosince 2013 léčeni standardně intravenózní trombolýzou, na kterou bezprostředně bez čekání na odezvu navázala u všech nemocných mechanická trombektomie systémem Solitaire. Tento soubor pacientů byl srovnán s výsledky dvou souborů pacientů, u kterých byla k léčbě použita buď pouze intravenózní trombolýza (IVT) nebo kombinace intravenózní a intraarteriální trombolýzy (LAT). Rekanalizace byla hodnocena pomocí TICI škály. Výsledný klinický stav byl zhodnocen pomocí modifikované Rankinovy škály (mRS), a to po 90 dnech. Dobrý klinický výsledek byl definován jako 0-3 body v této škále. Výsledky: Ve sledovaném období bylo léčeno celkem sedm nemocných s prokázaným uzávěrem AB pomocí kombinované revaskularizační terapie. Rekanalizace (TICI 2-3) bylo dosaženo u všech sedmi nemocných (100 %). Výsledný klinický stav byl hodnocen jako dobrý u pěti (71 %) pacientů. Ve srovnání s pacienty léčenými pouze IVT nebo kombinací IVT a IAT měli tito nemocní vyšší počet rekanalizací a lepší výsledný klinický stav (p = 0,142, p = 0,029). Z porovnávaných údajů je patrná významná tendence k obecně lepšímu výsledku ve skupině pacientů léčených mechanickou trombektomií v kombinaci s intravenózní trombolýzou. Závěr: Kombinovaná revaskularizační terapie akutní iCMP v zadní cirkulaci sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Ze získaných údajů je patrné, že tento způsob léčby má jednoznačnou tendenci k nejlepším výsledkům. K definitivnímu zhodnocení je nutný větší soubor nemocných., Aim: Occlusion of basilar artery (BAO) is a relatively rare cause of an acute ischemic stroke (AIS) with the incidence of 5-6%, however it is associated with a high mortality up to 90% if no treatment is used. The aim of our retrospective study was to assess the safety and efficacy of combined revascularization therapy including initial intravenous thrombolysis (IVT) followed directly by a mechanical thrombectomy and compare the results with IVT alone and with combined thrombolysis (IVT + intraarterial thrombolysis, IAT) in patients with AIS in vertebrobasilar territory. Methods: Study set consisted of consecutive AIS patients with BAO, who were treated with standard IVT and following direct mechanical trombectomy using a stent retriever without waiting for any clinical or radiological improving during IVT. The results were compared with historical controls treated with IVT alone or with combined IVT + IAT. Recanalization was scored using TICI scale and clinical outcome using mRS scale; good outcome was score as 0-3. Results: In total, 7 patients were treated with combination of IVT and stent retriever. Recanalization was achieved in 7 (100%) patients and good 3-month clinical outcome was in 5 (71%) patients. These patients had higher recanalization rate and better outcome than patients treated with IVT alone or with IVT + IAT (p = 0,142, p = 0,029). Thus, apparent trend for better clinical outcome was observed in a group of patients treated with intravenous thrombolysis with a subsequent mechanical thrombectomy. Conclusion: Combined revascularization using IVT and stent retriever seems to be safe and effective treatment for AIS patients with BAO. It is evident that this method of treatment has a clear tendency for the best results.
