Cieľ: Cieľom štúdie bolo preskúmať, ako sestry vnímajú svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb (DSS) a identifikovať, či existujú významné vzťahy medzi úrovňou dôstojného prístupu k pacientom v DSS a demografickými znakmi (vek sestier, dĺžka praxe, vzdelanie a pod.). Metodika: Výskumný súbor tvorilo 106 sestier pracujúcich v domovoch sociálnych služieb v zriaďovateľskej pôsobnosti Trnavského samosprávneho kraja. Výskum prebiehal v období december 2011 až február 2012. Na zistenie úrovne dôstojného prístupu sme využili autorizovaný dotazník: Posudzovanie etických problémov – dôstojný prístup k pacientovi. Tvorí ho 5 subškál (informovanosť pacientov; autonómia; dodržiavanie dôvernosti a ochrany informácií; princíp spravodlivosti; intimita). Štatistická analýza bola realizovaná softvérom SPSS, verzia 15.0. Výsledky: Svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb hodnotili sestry ako primeranú. Medzi vnímanou úrovňou dôstojného prístupu sestier a demografickými údajmi boli nízke korelácie. Signifikantne vyššiu úroveň dôstojného prístupu sme zistili u sestier s vyšším vzdelaním (Bc.). Najnižšiu úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb sme zaznamenali v subškále zameranej na autonómiu pacienta. Záver: Na základe poznania týchto výsledkov môžeme podporovať zmeny, ktoré budú prispievať k zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti v domovoch sociálnych služieb., Aim: The aim of the study was to examine nurses’ perception of their own level of dignified approach to patients in social care homes and to identify potential statistically significant relationships between the level of dignified approach to patients in social care homes and demographic data (nurses’ age, duration of clinical experience, education etc.). Methods: The sample consisted of 106 nurses working in social care homes run by the Trnava Region. An authorized questionnaire called Assessment of Ethical Issues – Dignified Approach to a Patient was used to determine the level of dignified approach. The questionnaire consisted of five subscales (patient awareness; autonomy; respecting confidentiality; principle of justice; intimacy). Statistical analysis was carried out using the SPSS version 15.0 software. Results: The level of dignified approach to patients in social care homes was self-assessed by nurses as adequate. There was a low correlation between the perceived level of dignified approach to patients and demographic data. Nurses with higher education (bachelor’s degree) demonstrated significantly higher levels of dignified approach. The lowest level of dignified approach was found in the subscale focused on patient autonomy. Medicína je specifickým oborem, který čerpá nejen z empirických poznatků přírodních věd, ale též z principů věd humanitních, i skutečnosti, že zdraví a nemoc nejsou jen kategorie výlučně přírodovědné, ale i hodnotové a proto je nutno zachovávat proporcionalitu mezi oprávněnými požadavky biomedicínského výzkumu a etickými nároky na něj a mezi morálními imperativy lékařské praxe. To je právě prostor pro etiku a etický přístup k řešení nejasných nebo sporných otázek našeho konání, otázek veřejností dnes velmi citlivě vnímaných, často vedoucích ke ztrátě důvěry nejen ve zdravotníky a zdravotnictví, ale i v celý společenský systém. Principy etického rozhodování v medicíně, hygienu a epidemiologii nevyjímaje, spočívají na dvou základních předpokladech: prvým je profesionální etika odpovědnosti, která zajišťuje kvalitu péče s vysokým stupněm odbornosti lékaře při poskytování pomoci a zábraně možných škod (primum non nocere) a druhým je právo na sebeurčení (autonomie jedince) s právem na informovanost - to je souhlas jedince informovaného pro něj srozumitelným způsobem nejen o plánovaném diagnostickém, léčebném a příslušném preventivním opatření, ale i v případě epidemiologických studií vztahu prostředí a zdraví jeho informovaného souhlasu k jeho účasti ve studii, jejíž by měl být subjektem. Článek se zabývá metodickými otázkami etických přístupů ke zdravotním problémům s důrazem na hromadně se vyskytující infekční a chronické nesdělné nemoci z hlediska šancí jejich prevence., Medicine is a specific area that draws not only on empirical knowledge of sciences but also on principles from the humanities, and the fact that health and disease are not only exclusively categorised as natural science but as value based entities where it is necessary to maintain proportionality between the legitimate requirements of biomedical research and ethical requirements and the moral imperatives of medical practice. It is here that we find the niche for ethics and ethical approaches to solving unclear or disputed issues of our actions; questions that are very sensitively perceived by the public, often leading to a loss of confidence in both health care professionals and the health system but also in the whole social system as such. Ethical principles in medicine, including hygiene and epidemiology are based on two basic assumptions: the first is the professional ethic of responsibility, which ensures quality of care with a high degree of expertise from assisting physicians and prevention of possible harm (primum non nocere) and the second is the right to self-determination (individual autonomy) with the right to be informed. It likewise comprises informed consent based on clear information regarding planned diagnostic procedures, therapeutic and preventive measure and informed consent regarding epidemiological studies in which patient's cooperation is requested. Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. V současnosti umírá v České republice ročně asi 110 000 osob. Ve více než 80 % úmrtí se jedná o osoby v geriatrickém věku. Proto je nezbytné se seriózně zabývat problematikou paliativní a terminálni péče v geriatrii. V přehledném článku autorka definuje obsah a náplň geriatrické paliativní péče a její hlavní charakteristiky. Na vlastních výsledcích demonstruje vysoký výskyt obtěžujících symptomů a jejich nedostatečné zvládnutí u geriatrických pacientů s terminálni prognózou. V závěru se zamýšU nad možnostmi zlepšení současné situace. Je třeba identifikovat bariéry v českém systému zdravotní a sociální péče a cílevědomě usilovat o jejich odstraňování na úrovni systémově, jednodivých poskytovatelů péče i konkrétních pečovatelů., Currently, the yearly death rate reaches 110 000 inhabitants in the Czech republic. More than 80 % of deaths occurs in geriatric age. Therefore it seems unavoidable to deal seriously with the palliative and end-of-life care m geriatrics. In this review article the author defines content of geriatric palliative care and its main characteristics. Based on her own results she demonstrates high occurrence of distressing symptoms and its inadequate management in geriatric patients with terminal prognosis. In conclusion she is rethinking possibilities of improvement of the current situation. Otázka terminálních stavů a doby bezprostředně předcházející úmrtí není u nás dosud uspokojivě vyřešena. Přitom stoná-li nemocný delší dobu a jeho nemoc má nepříznivý vývoj, přemýšlí o konci života a vytvoří si určitou představu, jak a kde by se měl konec jeho života odehrát. Je-li skutečnost podstatným způsobem odlišná, vyvolává to pocit dyskomfortu. Základní složkou péče v terminálni fázi by tedy měla být informovanost o přáních nemocného, o odmítaných procedurách apod. Například 60 % nemocných starších 65 let odmítá „život na přístrojích", 33 % odmítá život se stálou bolestí, 24 % umělou výživu apod. Podstatnou roli hraje předpokládané místo úmrtí, i když ne každý si přeje zemřít doma, ale ani těm, kteří si to přejí, se toto přání mnohdy nesplní. Dále jsou v článku rozebírány rysy nemocniční péče o terminálne nemocného a péče mimo nemocnici. Je obava, že faktory, které ovlivňují konečnou podobu terminálni fáze ze strany nemocného, nebude možno v domácím prostředí zvládat, a vzniknou tak komplikace, a nemocný bude na obtíž svým blízkým. Rodina a pečující tým má obavu, že nerozpozná ještě řešitelnou komplikaci a způsobí předčasný odchod svého blízkého. Z právního hlediska je podstatné týmem konstatované a do dokumentace nemocného zaznamenané ukončení kauzálni léčby a vyjasnění situace v podávání opioidů. V závěru článku je uvedena kazuistika términální fáze nemocného s bronchogenním karcinomem plic., The situation of dying people during the time immediately preceding the death is not satisfactory solved until now. The patient creates his vision of the end of his life, if he is ill for longer time and the course of his disease is unfavourable. He feels quite uncomfortably, if the reality is rather different. The information concerning end of life decisions should be one of the basic steps in the terminal care. For example - 60% of seniors above 65 years of age consider the life supporting activities „worse as a death", 33% the life with permanent pain, 24% the arteficial nutrition etc. Substantial is the supposed place of the expected death - not everybody wants to die at home, but even these, they want, freíuently die at the hospital. Factors how the dying patient influences the final course of the end of life are the fear from complications, which could not be settled et home and the wish not to be the burden for family members. The fami¬ ly members and caregivers are affraid of causing the untimely death by unrecognized still solvable complication considered as the final one. From the law point of view there is substantial by team to establish and into patient's documents to sign the end of causal therapy. The opioid administration from the lawers point of view is another important i issue in terminal care.
